Supplies Required: The following are required supplies for the Basic Welding program. You may purchase from the school a kit including a welding hood, welding jacket with leather sleeves, welding gloves, safety glasses, grinder, needle nose pliers, welder's cap and other miscellaneous tools at a nonrefundable cost of $750.00 to the student and in addition to Tuition cost. Leather steel-toe boots are also required and are not available to purchase from the school. To assist students who are financially not capable of paying the full tuition up front, the school will offer third party lender financing, subject to qualification and approval. Date Credit Card Security Code or Funding Agency: Tuition Refund Policy Three-Business-Day Cancellation: I understand that if for any reason I am unable to enter, all monies paid will be refunded if requested within three business days after signing an enrollment agreement and making an initial payment. Cancellation After the Three-Business-Day Cancellation Period but Before Commencement of Classes by the Student: Tuition fees collected in advance of entrance and if the student does not begin classes, not more than $150.00 shall be retained by the institution. Refunds for a student who does not begin classes shall be made within 30 days of the start of the quarter, term, or semester. I understand that the WITHDRAWAL AFTER COMMENCEMENT OF CLASSES REFUND POLICY SHALL BE: 1. After a student has completed less than 15% of the course, the institution shall refund at least 80% of the tuition, less the registration fee, thereafter, 2. After a student has completed less than one fourth of the course, the institution shall refund at least 70% of the tuition, less the registration fee, thereafter, 3. After a student has completed one fourth but less than one half of the course, the institution shall refund at least 45% of tuition, less the registration fee, thereafter, 3 of 7

4. After a student has completed one half or more of the course, the institution may retain 100% of the stated course price. There is no refund for any equipment, books and supplies received by the student. Fees for books and supplies are subject to cost change. Special Cases: In case of prolonged illness or accident, death in the family, or other circumstances that make impractical for the student to complete the program, the school may, at its discretion, make a settlement it considers to be reasonable and fair to all parties. Holder in Due Course Statement: Any holder of a consumer contract involving the financing of tuition, required school supplies and other administrative costs is subject to all claims and defenses which the debtor could assert against the seller of goods or services obtained pursuant hereto or with the proceeds, hereof Recovery hereunder by the debtor shall not exceed amounts paid by the debtor (FTC Rule effective 05-14-1976). Procedure for withdrawal/withdrawal date: 1. A student choosing to withdraw from the school after the commencement of classes is to provide written notice to the Director of Admissions of the school. The notice is to indicate the expected last date of attendance and be signed and dated by the student. 2. For a student who is on authorized Leave of Absence, the withdrawal date is the date the student was scheduled to return from the Leave and failed to do so. 3. A student will be determined to be withdrawn from the school of the student has not attended any class for eight (8) consecutive class hours, without notifying the school before the absence. THE STUDENT UNDERSTANDS: 1. The School does not accept credit for previous education, training, work experience or College Level Examination Program (CLEP), if applicable. 2. The School does not guarantee job placement to students upon program/course enrollment/completion or upon graduation. 3. The School reserves the right to reschedule the program start date when the number of students scheduled is too small. 4. The School will not be responsible for any statement of policy or procedure that does not appear in the School catalog. 5. The School reserves the right to discontinue any student s training for unsatisfactory progress, nonpayment of tuition or failure to abide by School rules. 6. This document does not constitute a binding agreement until accepted in writing by all parties. HOLD HARMLESS AGREEMENT: Sparx Welding and Technology Institute and student acknowledge that there is some risk of accident or injury associated with use of equipment and other aspects of the courses of study of welding, including but not limited to direct care and contact of other students, clients, visitors, 4 of 7

employees and instructors at the training facility site. Student does hereby waive, release and discharge Sparx Welding and Technology Institute of any and all liability and all claims for damages for death, personal injury or property damage which may have or which hereafter accrue to student as a result of participation in the program and courses whether or not caused by negligence or fault of Sparx Welding and Technology Institute. This release is intended to discharge the school and its officers, employees, representatives, students, volunteers and agents from and against any and all liability arising out of or connected in any way with student s participation in the training, internship/externship, hands-on activities, practice or other activities. Knowing risks exist, nevertheless, student hereby agrees to assume those risks and to release and to hold harmless all persons or agencies mentioned above that might otherwise be liable to student or his/her heirs or assigns for damages. Student further understands and agrees that this waiver, release and assumption of risk are to be binding on my heirs and assigns. In addition, student gives permission to receive, if necessary, emergency medical services by authorized personnel and that any cost incurred as a result of such medical treatment will be the student s responsibility.
