Registration Requirements Completed registration form (page 3 of this document to be signed and returned). Non-refundable annual registration fee of $30 per family (CASH or cheque) dated the day of registration. Post-dated cheques made payable to Toronto Mathematics Academy for September 1 st, 2018, October 21 st, 2018, December 9 th, 2018, February 10 th, 2019, April 14 th, 2019. Name of child and level should be included in the memo portion of each cheque. Read and understood the Toronto Mathematics Academy Policies and Procedures. The parents must come in person to bring the registration forms and cheques. Please do not send the forms and cheques with your children. Important Notes! 1) In order to register, all of the following items are necessary: Registration form (signed and dated) $30 annual non-refundable registration fee dated the day of registration (cash or cheque) (all the students, returning and new, must pay this fee). Five post-dated cheques (the dates are specified on the registration form, please do not change the dates with other dates). 2) No registration will be accepted if any item listed above is missing. Seats are not reserved unless all the above items are received. 3) The registration is made on a first come - first served bases. We do not reserve seats by phone or email. The parents or an adult representing the parents must come in person to register their children. 4) We reserve the right to cancel a class due to insufficient enrolment. It is very unlikely that we will offer classes for which the enrolment is less than eight students. 5) If there are a large number of requests for a specific level, new classes may be added. Policies and Procedures 1) The registration is complete and students are enrolled with the Toronto Mathematics Academy once the office receives all of the followings: the registration form, the registration fee, and all five post-dated cheque. Seats will not be reserved until the registration is complete. 2) The registration is made on a first come - first served bases. 3) Toronto Mathematics Academy reserves the right to cancel a class due to insufficient enrolment or for other reasons determined by the principal. 4) The withdrawal procedure does not allow for partial refunds for a session in progress. If you would like to withdraw from the program, please inform the school IN WRITING prior to the next payment date and the cheques will be returned to you. A session represents an interval of time covered by one payment (7 lessons for each cheque). 5) There is NO refund in case of absence. Four (4) free make-up lessons per year are possible, but only with a similar level that runs the same lesson as the one missed, and only whether there is space. Any additional make-up lessons are subject to an additional amount of $ for a 1.5 hour classes or $30 for a 2 hours class and that is payable at the beginning of the lesson. Private lessons are NOT provided as a free make-up lesson. 6) A make-up lesson can be taken only with a class of the same level. 7) In case a make-up lesson is required, the parents must inform the school at least 24 hours prior to the lesson they wish to make the switch for. If the school is informed during or after the lesson has passed, no make-up lesson can be arranged. 8) If a student is absent from class due to illness or other circumstances, it is the responsibility of the student and/or parents to pick up any missed work from school. Teachers and staff are not permitted to email and/or fax handouts to the students. 9) There is allowed one permanent switch per school year, provided that there is room, at no cost. Any additional permanent switches are charged an administration fee of $50. 10) All tuition cheques returned to the office Non-Sufficient Funds or for any other reason given by the bank, will carry a $30.00 service charge to be paid to Toronto Mathematics Academy. 11) Classes are offered once per week for each level, according to the calendar provided in the registration package. 12) Level 7, 8, 9 and Calculus are 2-hour classes. Levels 2, 3, 4, 5, 6 are 1.5-hour classes. Level 1 is a 1- hour class. The students come once per week for 35 weeks, according to the School Calendar. 13) The parents are responsible to accompany their children to and from the classroom every lesson. Parents will accept full responsibility for their child/children until their class begin or when it ends. Toronto Mathematics Academy cannot assume responsibility for students who arrive before classes begin or after the classes end. 14) The students and the parents are responsible to hand in the homework completed on time. 15) The students that fail to hand in the homework two times will be put on probation. If a third homework will not be handed in on time, the student may lose their seat in the program. In case a student is asked to leave the program for not keeping up with the work, no refund is given for the session in progress. All the remained cheques for the school year will be returned to the parents. 16) The parents must inform the school in writing if the students cannot bring the homework on time. The information note must be sent no later than the beginning of the lesson. In that case, the students will be given one-week extension to present the completed homework. 2018 Toronto Mathematics Academy Page 3

17) NEW! NO soft copies of the work (class work, homework, tests, contest prep, etc.) will ever be sent by email. 18) NEW! All the students will receive a package with all the Class Work, Homework, Contest Prep (less test/quizzes/drills), at the beginning of each payment session, after the bank cleared out the cheque. The students must bring with them the work packages, every lesson. If a student loses their original work package and require a new package, a $20 fee per package will be charged. The fee must be received before the new package is handed out. 19) NEW! NO answers/solutions to the homework and/or class work will ever be sent by email. The students will receive a package with answers at the beginning of each payment session. The parents should keep the Answers Package and check the work with the student to make sure it is ready to be handed in for marking at the required due date. 20) NEW! The TMA materials are copyrighted materials. Under no circumstances, may the actual contents or individual problems and/or solutions be published and/or distributed in any form, including on the internet, secure websites, email or printed formats. 21) Please note that each post-dated cheque covers a certain number of lessons. In case of withdrawal, the school must be informed in writing prior to the following dates: Session 1 : no later than August 20, 2018 Session 2 : no later than October 14, 2018 Session 3 : no later than December 2, 2018 Session 4 : no later than February 3, 2019 Session 5 : no later than April 7, 2019 2018 Toronto Mathematics Academy Page 4
