Metropolitan State University School of Law Enforcement and Criminal Justice 2018 APPLICATION - LAW ENFORCEMENT SUMMER SKILLS PRACTICUM Due: Noon Friday December 8, 2017 PART 1 APPLICANT INFORMATION Last Name First Name Student Tech ID Metro State Email: @metrostate.edu Mailing Address: Home Phone ( ) Work Phone ( ) Mobile Phone ( ) PART 2 LEARNING MODULES The new learning objectives mandated by the Minnesota Board of Peace Officer Standards and Training Board (POST Board) are embedded in the revised law enforcement curriculum that took effect Fall Semester 2017. It is important to note that the number of learning objectives has been greatly expanded from what was previously required before this change. There are a number of law enforcement students who started their degree under the previous curriculum, but will now be completing their coursework under the revised curriculum and taking the Minnesota peace officer licensing exam based on the new POST learning objectives. Please review the student notification letter attached to this application, which was circulated in April 2017 to law enforcement students impacted by the transition to our newly revised law enforcement curriculum. For students affected by this transition, an interim pathway is being created to help bridge this gap through noncredit, no-cost modules. The number of modules a transitional student needs to take depends on how much coursework was completed under the previous curriculum and learning objectives. Taking these module(s) minimizes students having to take additional coursework under the revised curriculum in order to fulfill POST Board requirements. The 2018 Skills Application is one of several factors being used to determine on a case-by-case basis which students need to take one or more of the five gap modules. Students identified as needing modules will be informed via their Metro State email and automatically enrolled in the D2L site, which becomes active Spring Semester 2018. Once finishing a self-directed module, students need to complete a competency statement regarding their understanding of the content. Students have until 11:59 p.m. on Tuesday, April 24, 2018 to successfully complete their designated modules. Failure to meet this deadline may result in their tentative acceptance into the skills practicum being rescinded for Summer Session 2018. 2018 Skills Application Version 1 ( 11/03/2017) Page 1 6

PART 3 COMPLETE CHARTS(S) - SKILLS PREREQUISITE COURSES Fill out below those Professional Peace Officer Education (PPOE) courses you have already taken under CHART 1 and/or are in progress or pending under CHART 2. You must receive a grade of C- or better on each of the required PPOE courses in order to be counted. All these PPOE courses must be completed before the start of the 2018 summer skills practicum. It is highly recommended that you connect with your Academic Advisor for assistance in completing this section, given the realignment and revision of courses that took effect Fall Semester 2017. PART 7 STUDENT ACKNOWLEDGEMENT AND UNDERSTANDING (Please note Metropolitan State University is abbreviated under this section as Metro State) READ AND INITIAL EACH OF THE FOLLOWING STATEMENTS I confirm having submitted a Major Declaration Form and receiving formal notification of my admission to the Law Enforcement Major at Metro State. I understand that if I am admitted to the skills practicum, but do not complete all the required coursework with a C- or better grade, my admission to the skills practicum will be rescinded. I understand this application does not guarantee my acceptance into the skills practicum and that incomplete information will result in my application being rejected. I understand that if my application is approved, I will be required to attend one of the Skills Orientation Sessions in February 2018. I further understand that failure to attend one of these mandatory orientation sessions could result in my conditional acceptance into the skills practicum being revoked. I understand that I will normally be placed in the Day Skills Practicum unless circumstances dictate otherwise. This will be determined by the Metro State POST Coordinator. I further understand that I can request to be placed in the Night/Weekend Skills Practicum, but there is no guarantee my request will be granted. I understand that if I decide to drop from the skills practicum once accepted, I will notify the Metro State POST Coordinator via email as soon as possible. I further understand it is my responsibility to officially withdraw from the courses that comprise the skills practicum. I understand that normally most of the content and instruction for the skills practicum is delivered through a contractual arrangement with Hennepin Technical College (HTC) Customized Training. I further understand this partnership arrangement between Metro State and HTC is subject to modifications or dissolution if circumstances warrant alternative arrangements being made for the delivery of the skills practicum. I understand that as part of the skills practicum, I will need to provide my own transportation and lodging if desired while attending the two-day Emergency Vehicle Operation Course (EVOC) at the Minnesota Highway Safety and Research Center in St. Cloud, Minnesota. Although unlikely, I further understand that the provider and location of this training could change. I understand that I will need to provide documentation from Driver and Vehicle Services (MN or other state) that my driver s license is valid prior to the start of the Skills Practicum. I further understand that my driving status must be valid for the duration of the Skills Practicum and that I am obligated to notify as soon as possible the Metro State POST Coordinator of any changes to that status. I understand that I must receive advanced approval from a licensed physician and psychologist to attend the skills practicum. This approval needs to be documented on the forms provided by Metro State. I further understand that during the skills practicum I am obligated to report to the Metro State POST Coordinator any changes in my physical and/or mental status that adversely affect my ability to participate in the program. I understand that it is my responsibility to prepare for and maintain a level of physical fitness that will allow me to fully participate in the physical activities and testing that are part of the skills practicum. I understand that I am responsible during the skills practicum to act in a professional manner and adhere to the Student Code of Conduct for both Metro State and HTC. I further understand that skills instructors may impose additional expectations that must be followed due to safety concerns and/or other considerations. I understand that any safety violations I commit may result in my immediate removal from one or more of the courses that comprise the skills practicum and forfeiture of my tuition. I understand information, correspondence, admission status, and updates concerning the skills practicum will normally be sent to my Metro State email account. I further understand that I am responsible for ensuring all my personal data is current and valid (legal name, home address, phone number(s), student email address, etc.) with both the Metro State POST Coordinator and Admissions Office. I understand that during the skills practicum I am responsible for regularly checking my Metro State email account and the designated D2L site. I understand that both the Day and Night/Weekend Summer Skills Practicums are programmed to be delivered within the confines of Summer Session 2018 (May August).
