Develops goal-oriented treatment plans, interventions, educational materials based upon evaluation results and patient input, establishes functional, measurable and attainable goals and performs reassessment and updating/modification of goals based on client progress. 4. Clinical Treatment: Administers individualized and effective treatment procedures through engagement of client using interpersonal dynamics (including home exercise and selftreatment plans and family education) to assist patient towards goal achievement Provides information regarding diagnosis and prognosis of therapy, recognizes contradictions or adverse responses to therapy and adjusts treatment accordingly, provides recommendations and discharge planning as appropriate for the anticipated needs of the patient. 5. Maintains files and/or records (e.g. progress reports, billing information, treatment plans, required documentation, quarterly reports, screening results, etc.). and/or other written materials (e.g. activity logs, correspondence, memos, etc.) as required. 6. Makes appropriate recommendations and referrals for additional services. Consults and collaborates with a variety of groups and/or individuals (e.g. parents, teachers, physicians, other rehabilitation disciplines, team members, other professionals, etc.) for the purpose of coordination of treatment strategies, communicating information, resolving issues and providing services. 7. Maintains and increases professional competency by participating in continued education and annual review. Provides in-service for staff after attending courses and conducts in service training programs in the community regarding speech therapy services. 8. Participates in team meetings and client treatment planning meetings. 9. Licensure: Current Indiana License (or eligible) as a Speech-Language Pathologist Experience: ASHA CCC preferred with prior experience in a Clinical Setting, but CFY encouraged to apply.
