Instructions for Filling the Application Form Forms must be filled in by the candidate in his / her own handwriting neatly and legibly in English using capital letters, except for signature. Overwriting, cutting, erasing in the form may lead to rejection and should be avoided. Paste your recent photographh (not older than three months) at appropriate place on the application. The photograph must be signed by the candidate at the edge and should not be defaced. Name of the Applicant: Write name in capital letters as it appears in your mark sheet of the qualifying examination. Name of the Parent / Guardian: Write the name of parent / guardian (Any person, other than the parents of the candidate, can become guardian only if the parents are not alive or are insane). Date of Birth: Enter the date, month and year of birth as recorded in class 10th certificate. Address for Correspondence: Write the complete postal address including pin code on which the candidate is to be communicated. Also write the telephone number with STD / ISD code and Fax / E-mail address, if any. Declaration: Candidate and the Parent / guardian must sign with date the declaration to authenticate the information provided by them as per format on a non judicial stamp paper of Rs. 10/- duly stamped by the notary public. Details of passing qualifying examination i.e., MBBS- write the month and year, name of the Medical College and the University from where you have passed or appeared at MBBS examination. Marks obtained in MBBS: Write maximum marks, marks obtained and the percentage subject-wise in the appropriate column for first, second, third Professional examination including supplementary exams, if any and number of attempts in each professional examination separately. Acknowledgment card and Interview call letter: Write your communication
address correctly on the acknowledgment card and interview call letter. Affix postal stamp of Rs. 5/- separately at appropriate places on the acknowledgment card as well as the interview call letter. NRI Seats Students from other countries should possess passport, visa and exchange permits valid for the period of their course of study in this Institution and should also observe the regulations of both central and state governments regarding residential permits and obtain no-objection certificate. The candidate should have a provisional "Student Visa". If he comes on any other visa and is selected for admission, he will have to first obtain a student visa from his country and then only he will be allowed to join the course. Eligibility Every student, selected for admission to postgraduate medical course shall possess recognised MBBS degree or equivalent qualification and should have obtained permanent Registration with the Medical Council of India, or any of the State Medical Councils or should obtain the same within one month from the date of his admission, failing which the admission of the candidate shall be cancelled; Completed satisfactorily one year's rotatory internship or would be completing the same before 31.03.2012 after passing 3rd MBBS Exams. In the case of a foreign national, the Medical Council of India may, on payment of the prescribed fee for registration, grant temporary registration for the duration of the postgraduate training restricted to the medical college/institution to which he is admitted for the time being exclusively for postgraduate studies; however temporary registration to such foreign national shall be subject to the condition that such person is duly registered as medical practitioner in his own country from which he has obtained his basic medical qualification and that his degree is recognised by the corresponding Medical Council or concerned authority. Few lectures or other type of exposure to human behavior studies. Basic understanding of pharmaco-economics. Introduction to the non-linear mathematics. Selection of postgraduate students
NRI Quota 15% of the total seats are earmarked for Foreign National / PIO / NRI / Ward of NRI / NRI sponsored candidates and the remaining 85% seats will be filled by the management of the institution on the basis of merit. For the year 2013-14 academic merit shall be assessed as per P.G. regulation 2000 (amended 2008) by MCI, as under. (I. On the basis of the individual's cumulative performance at the first, second and third MBBS examination. or II. On the basis of merit as determined by the competitive Entrance test conducted by the University or III. (Combination of (i) and (ii) Please note: - The admission policy may be changed according to the law prevailing at the time of admission in that academic year. *Note : University shall be free to take decision about above methods of determining merit on the basis of number of applications received and / or law prevailing at the time of admission. Counseling / Interview I. Candidates in order of merit will be called for Counseling/Interview and for verification of original documents and identity by personal appearance. Counseling will be performed and the placement will be done on merit-cum-choice basis. No change in the choice will be allowed later on. Candidates coming late during Counseling may be considered but placement will be done only on the existing vacancy at that time. II. The same will also be available on MGUMST website www.mgumst.org Period of Training The period of training for the award of various postgraduate degrees or diplomas shall be as follows: Doctor of Medicine (M.D.) / Master of Surgery (M.S.) The period of training for obtaining these degrees shall be three completed years including the period of examination. Provided that in the case of students having a recognized two year postgraduate diploma course in the same subject, the period of training, including the period of examination, shall be two years. University Enrolment All the selected candidates admitted to the MD / MS / M Sc Medical Courses at MGMCH under any category are required to apply to the Mahatma Gandhi University of Medical Sciences & Technology, Jaipur on a prescribed proforma with the required fee for enrolment as per rules and regulations of the MGUMST, Jaipur. The admitted students must apply for enrolment as per the notification of the MGUMST.
Migration No application for migration to other Medical Colleges will be entertained from the students admitted to the MD / MS course at this Institute. Note: fee will be deposited by a Demand Draft in the name of Mahatma Gandhi Medical University of Medical Sciences & Technology, payable at Jaipur Cancellation and Refund If a student withdraws / gets cancelled his admission after joining the course but before the last date of admission prescribed in this regard and if such vacant seat is filled by another candidate by the last date of admission then the fees paid shall be proportionately deducted (which will be approximately 10% of the tuition fees) for the period the student has attended the college along with proportionate deduction of hostel fees and the balance fees along with original certificate will be returned to the student. However, if no other candidate is available and student leaves the course, entire fee of course will be confiscated. Hostel Facilities P.G. training is a full time residency program as per MCI Regulations and it is mandatory for all the PG residents to stay in the campus. There is a separate hostel for PG Residents available in the campus. accommodation for Married students is allowed in the Hostel. Hostel accommodation to all the residents admitted to the Post Graduate Medical Course in the Institution is compulsory so as to maintain regularity, standard of medical education and discipline. Hostel fee for total course duration shall be payable at the time of counseling / admission through a crossed demand draft drawn in favour of Mahatma Gandhi University of Medical Sciences & Technology payable at Jaipur. Subsequent payment must be made on the dates notified. * Every room is self contained with twin beds, attached toilet & bath and kitchen. All rooms have a separate sub-meter for electricity readings. AC facility is optional and available on charged basis. A few rooms without AC on single occupancy basis are also available. For these rooms, the hostel fee is Rs. 70000.00 per annum and electricity charges will be collected as per separate meter readings for the room.
