2 Aims This logbook is designed to capture the value of your work placement and how it contributes to your post school journey. There are sections that cover: Before your placement During your placement After your placement Your level of response in your Learning Plan will determine the level of the Award you will receive: Level 4; answers will be mostly straightforward and you will need support to complete your Learning Plan Level 5; you will provide more detailed answers and require minimum support to complete your Learning Plan Level 6; your reflections and answers will be more complex and you will work independently; this booklet is not enough to gain a Level 6 Award you will be expected to use your notes in this booklet to create a version that covers in detail all the areas required.

Target Setting Level 4-6 ALL 7 Using your Personal Achievement Journey or My Strengths in My World of Work identify 2 or 3 skills where you need to improve your level confidence and success. SETTING SUCCESSFUL TARGETS For example I wish to improve my ability to talk to others confidently. My target is to sit with the other staff at break and lunchtime and start conversations with different people each day of my placement. AFTER Level 5 outcome 2d Work Place Project - REVISITED Level 6 outcome 3 24 Once you have carried out your work place project you will need to evaluate the outcome. Remember for Level 6 this table will simply form your notes a more detailed evaluation is required. Did I achieve the aim of my project? 31 Final Supervisor Checklist We would be grateful if you could ensure the following was up to date prior to the pupil leaving their placement. Thank you in advance for your co-operation. Complete Employer Report on pages 28-30 Page 12 is signed to ensure accurate information has been given Page 26 is signed to show you have agreed with pupil s evaluation of their Targets. Parental Checklist We would be grateful if you could ensure the following was up to date prior to your child handing in their Work Placement Learning Plan to their teacher. Thank you in advance for your co-operation. Ensure all parts of the Work Placement Learning Plan have been completed; discuss with your child what they have learned and how this relates to their future. Ensure your child submits this Work Placement Learning Plan to their teacher. Sign page 26 to show you have agree with your child s evaluation of their Targets. Comment: Now that you have completed your placement you will participate in a final Work Placement Workshop to allow the following: o Thank you letter You can use the template to generate a letter thanking your employer for having you on the placement. Include a copy of the template Certificate issued by West Lothian Council to employers as well as a copy of your Level 6 Project. o Submit your Work Placement Learning Plan to your House Head (week placement) or Miss McTiernan (extended placement) o Consider how you can support future work placement students could you give a short summary from your Work Placement Learning Plan to your teacher to go onto the school website? o Carry out the West Lothian Work Placement Survey your teacher will provide details o Update Didbook think of all the skills and experiences you have gained.
