INSTRUCTIONS Please read the following information carefully and ensure all fields in this form are complete. The application form and all supporting documents (refer Checklist) must be scanned and saved in PDF format as one file, then emailed to: ES@mn.catholic.edu.au Please attach the saved PDF file to your email using the subject format: Surname Application. All documentation must be copies that have been certified by a Justice of the Peace or other Authorised Officer. If documents are submitted in your maiden name or a name different to your proof of identify documents, you must provide a certified copy of your marriage certificate or other supporting documents for verification purposes. Once a complete application with all documents has been received, you will receive an acknowledgement email within ten working days. SECTION 3: NESA NSW EDUCATION STANDARDS AUTHORITY (FORMERLY BOSTES) To teach in the Diocese of Maitland-Newcastle, teachers are required to be accredited with NESA (NSW Education andards Authority). If you are commencing teaching in NSW or you are returning to teaching in NSW after an absence of five (5) years or more, you will need to be accredited with NESA. Please note your application will not be accepted until you are registered with NESA. Existing teachers who have worked in a NSW school before 1 October 2004 and have not had a break from teaching of more than 5 years, do not currently need to be accredited. If this is applicable to you, please indicate below: I am an existing teacher i.e. qualified and teaching in NSW before and since October 2004 and without a five year teaching absence in the meantime. If you hold current registration from another Australian teacher registration body, and have the right qualifications to teach in NSW, your interstate registration can be recognised by NESA. You must apply for accreditation with NESA and provide evidence of full registration in another Australian state or territory. To maintain accreditation with NESA, teachers are required to pay the annual fee and meet the requirements of accreditation. As demanded by law in New South Wales, failure to maintain accreditation with NESA will forfeit a teacher s right to teach in the Diocese of Maitland-Newcastle. 1. Are you a member of NESA? APPENDIX 2 Registration for Employment Collection Notice 2017 1. In applying for registration you will be providing the Diocese of Maitland-Newcastle Catholic Schools Office (CSO) with personal information. We can be contacted at 841 Hunter reet, Newcastle West NSW 2032, phone (02) 49791200. 2. If you provide us with personal information, for example, your name and address or information contained on your resume, we will collect this information in order to assess your application for registration for employment. We may keep this information on file. 3. The CSO s Privacy Policy sets out how you may seek access to your personal information and how you may complain about a breach of the APPs.
