This form must be used for applicants for need-based grants, applicants for merit-based scholarships, or applicants for both grants and scholarships. PLEASE NOTE THE APPLICATION DUE DATES ON THE WEBSITE I. ELIGIBILITY: The complete eligibility and other requirements for educational award assistance from the Armed Forces Children s Education Fund of A Million Thanks, Inc. ( AFCEF ) is found within its Educational Award Policies and Procedures, a copy of which may be found at [www.amillionthanks.org]. Any summary information provided herein is for informational purposes only, and does not alter the requirements found in those policies and procedures, as they may be amended from time to time in writing by AFCEF. Generally, an applicant s parent(s) or legal guardian(s) must have died in the course of their active duties for the U.S. Armed Forces in connection with the Global War on Terrorism on or after October 7, 2001 to be eligible for any educational assistance from AFCEF. The applicant must be either a high school senior or a high school graduate/ged, or a current student at a post-secondary educational institution who intends to be enrolled in a tax-exempt post-secondary educational institution in the coming academic year. Please note that those planning to enroll in an online degree program or at a for-profit institution are not eligible for an AFCEF award. Those affiliated with AFCEF or a selection committee of AFCEF, as well as their family members, are also ineligible. II. The transcript must show the applicant s cumulative grade point average (or other academic marking system), class rank (if available), and must bear the official stamp or seal of the issuing high school or post-secondary educational institution. If you are enrolled at a post-secondary institution but have yet to complete a full academic year, then please submit as much information as possible from your current school, but also provide all information below from your high school. The essay should be no more than one thousand (1,000) words, but not less than two hundred and fifty (250) words, as measured by the applicable Word processor, and the word count must be clearly displayed. Please submit copies of all financial information and forms submitted to the post-secondary educational institution in which you are enrolled or intend to enroll, including (but not limited to) a copy of the Applicant s completed Free Application for Federal Student Aid, available at http://www.fafsa.ed.gov/ (the FAFSA ). Grant applicants must submit a completed FAFSA to AFCEF even if they have not or do not intend to submit the FAFSA to the educational institution in which they are enrolled or intend to enroll. VIII. ADDITIONAL INFORMATION FOR SCHOLARSHIP APPLICANTS (If you are only applying for a need-based grant, then you may skip this Section VIII.) Please submit written evidence of all standardized test scores received, including but not limited to the Scholastic Aptitude Test, the American College Testing examination, Advanced Placement or International Baccalaureate examinations, or tests used by graduate schools (e.g., the LSAT, MCAT, GMAT, etc.). Additional rules and conditions applicable to A Million Thanks s Fund-A-Scholar program shall be: 1. AFCEF Awards are for undergraduate study at an accredited, nonprofit institution of higher education within the United States, except when the recipient is from a possession of the United States, in which case he/she can select an otherwise eligible educational institution in that possession or in the United States. 2. The recipient must be accepted or enrolled as a full-time student in order for the funds to be awarded to the institution. 3. The number and amount of the awards will be determined at the sole discretion of AFCEF. 4. Award funds may only be used to defray necessary costs of the student's education (i.e. tuition, books & supplies, fees and equipment required by the student's particular course of study or school). 5. The award recipient may reapply for the scholarship annually. The amount of the fund request cannot exceed the cost of education expenses. 6. Students may make only one (1) request for funds per semester or quarter. 7. Award checks will be issued upon receipt of a "Request for Funds" form. It is the normal and customary procedure that the scholarship check will be a two-party check made payable to the student and school. Thank you Revised 12/2015
