INSTRUCTIONS To be considered for admission to Oregon State University (OSU), you must do the following: APPLICATION FORM: Complete the attached application form (front and back). All questions must be answered in full. Your answers must be legibly printed or typed. Please remember to sign the form. Your admission process will be delayed if your application is not complete. NOTE: Your name on the application must appear exactly as it appears on your passport to ensure that your visa documentation is properly prepared by OSU. TOEFL TEST SCORES: Please be sure that you include one unofficial copy of your TOEFL test scores when you submit your application form to your home university exchange coordinator. If you have not yet taken the TOEFL test, make sure your score report is sent directly to OSU using the OSU school code: 458 Scores cannot be more than 2 years old at the time of your program start date. Undergraduate student requirement: TOEFL score of at least 80 (ibt) with minimum score of 16 on each section, 550 (PBT), 5 (IELTS). Graduate student requirement: TOEFL score of at least 80 (ibt) with minimum score of 18 on each section, 550 (PBT), 5 (IELTS). OFFICIAL TRANSCRIPTS, DEGREES, AND CERTIFICATES FROM ALL COLLEGES/UNIVERSITIES: (IN English): A minimum GPA of 5 for undergraduates and 75 for graduates is required For graduate students: An equivalent of a U.S. Bachelor s degree. The 3 year Bologna degree is also acceptable. ACADEMIC RECORDS: You must submit an official transcript of your college level coursework with your application. Please be sure to include the type of degree/certificate you are earning and the date this degree/certificate will be awarded under Academic Record on the application. FINANCIAL DOCUMENTATION: See page 2 of instructions for detailed information about the Declaration of Finances. PASSPORT: Attach one legible copy of your passport identification page to this application. Make sure that your passport is valid for at least 6 months after the end of your academic program at OSU. If you hold dual citizenship with the U.S., you must enter the U.S. on your U.S. passport (in this case make sure that the attached copy of your passport identification page is from your U.S. passport). ENGLISH TRANSLATIONS: Make sure all application materials are accompanied by an English translation. Official transcripts and financial documents should be submitted in their original language and in English translation. DEADLINE: Each exchange program has a different deadline. Be sure to confer with your home university exchange coordinator for further information. It is important not to delay submitting a complete packet to your exchange coordinator since the admission process, issuing of the Form DS 2019, and obtaining a visa can take several months. RETURN APPLICATION: Send the original application and all materials mentioned above to your home university exchange coordinator. Do not send these materials directly to OSU. DECLARATION OF FINANCES OSU tuition is waived on all exchange programs as part of the exchange agreement. Other student fees may or may not be waived depending on your university s agreement with OSU. You must list the sources of your support and complete the Affidavit of Support if you are sponsored by a person other than yourself. All supporting financial documents and the affidavit(s) of support must show the numerical amount required for the entire period of your proposed study. Financial documentation showing a non numerical description, e.g., mid six figures is NOT acceptable. All supporting financial documents should be written in English or include an English translation. Funding should be shown in U.S. dollars. Your financial documentation must be no more than 9 months old at the time of enrollment. Assistantship/Scholarship/Home Country Sponsorship Award Letters must list the terms/time period when you intend to study at Oregon State University. These letters issued for a past term are not acceptable. Health Insurance OSU requires all non resident international students and their dependents (living in the U.S.) to be covered by health insurance. The health insurance must meet federal and OSU requirements. All international students who are registered for at least 1 credit hour on campus in Fall, Winter and/or Spring terms will be automatically charged for the OSU International Health Insurance. If a student has other insurance, he/she may submit a waiver and a summary of the insurance for approval. If approved, the charge for OSU insurance will be removed.
