Classroom instruction will be a minimum of 3 weeks in length. BTW instruction shall not begin until the student has received a minimum of 4 hours of classroom instruction. BTW instruction must be completed no later than 3 weeks after the classroom instruction has been completed. Notice: This provider is required to be certified by the Secretary of State. If you have any complaint, which cannot be settled with this provider, write: Michigan Department of State, Driver Programs Division, Lansing, MI 48919. Completion of driver education instruction does not guarantee qualification for a driver license. Terms: In Order To Take And Pass Driver Training, A Student Must: 1. Return application form with payment to the high school office. 2. Be 14 yrs & 8 months old at least by the start of class (verification by birth certificate required). 3. Attend and pass 24 hours of classroom and 6 hours of behind the- wheel instruction. 4. Must have proof of an eye test showing visual acuity is at least 20/40 corrected, within the last year before class starts. Class and Group Assignments: Pick a Classs (Blue or ), Pick a Drive Group (A or B) Note: You must attend all of your classes, the drive group determines the days you drive Students must pick ONE Class time: Blue, or Students must pick ONE Drive group: Monday/Wednesday OR Tuesday/Thursday Classes: Students must pick one class time. Blue Group or Group Class will be EVERY day regardless of drive group! Drive times are THREE HOURS PRIOR to class. Students must pick one drive group. Group A Group B Absences: **** IMPORTANT**** Classroom time is setup with a sequential order of lessons that should NOT be taken out of order. Students who are late or absent may result in dismissal. According to State Law: PA 384 of 2006, Student can only attend two hours a day but if a student misses a class it must be made up within the current session. This makes virtully impossible to reschedule a class time that is missed and may result in students having to re-enroll and pay again. It is important to look at the classroom dates before signing up. If a student is absent from driving, that time must be made up. We can usually find time for one make-up drive during the program if the absence is planned in advance before the start of the session with the instructor; it is entirely up to that instructor. An additional $50 fee may be assessed depending on availability of instructors and driver education car. FEES: $315 is due when signing up online through RevTrak. Students who drop out of the program after they sign up may not be entitled to ANY refund unless a replacement student can be found prior to the start of class. We are a nonprofit program. In order to keep our cost low, we count on a full class of paid students. Check the dates BEFORE signing up. Replacement permits cost $5.

PLAINWELL HIGH SCHOOL DRIVER EDUCATION RULES & REGULATIONS Driver Education is one of the most important, if not the most important, class a student takes in high school. Everyday we hear or read about people that have been killed in automobile accidents. The major reasons for these accidents are the ignorance, carelessness, discourtesy and recklessness of students; therefore, our policies are strict. The following is a list of the policies for driver education. Please initial that you have read each point below. If you have questions, please call the high school at (269)685-9554. Ask for Secretary Mrs. Tyria or the Director of Driver Education, Mr. Badalamente. Parents / Guardians Initial each of the following to indicate that you have discussed these with your child. 1. The State of Michigan requires twenty-four (24) hours of classroom instruction, six hours of behind-the-wheel training, and observation hours in a driver education car. Special assistance can be provided to those who qualify to participate in the classroom. Notification of needs must be made in writing and turned in with this form. In the past six months a student can not have had any fainting spells, blackouts, seizures, loss of consciousness; or has a mental condition that would affect their ability to drive safely. If yes, do not initial. 7. Is the student taking any medications that may affect their ability to drive a motor vehicle safely. If yes, do not initial. 8. Students are not allowed to sleep during class, range, or road time. On the first incident, the student will be warned, second time the student will be sent home counts as an absent and will be dismissed from class with no refund. 9. ALL SCHOOL RULES APPLY! No drugs of any kind, drinking or smoking is allowed on school grounds either before class, during break, or after class. Violation will result in removal from the class with no refund. 10. A student is required to bring pencils, paper, and driver education book to class each day. 11. A student is expected to wear closed toed shoes to range, and road; not flip-flops. 12. A student will be issued a certificate following the successful completion of the course. A student must pass class and the state test (with 70% or better) in order to be issued a driver education certificate to receive credit for the course. 13. Replacement permits will cost a set fee. 14. If a student drops out or is removed from the program, the student will have to pay again the necessary tuition fees. 15. Behind the Wheel Agreement: PLEASE FILL OUT AND SIGN ONE form below. During instruction time in the driver education car we will have three students scheduled to drive together. From time to time conflicts arise, and for one reason or another students need to be released from a drive. On RARE occasions this results in only the instructor and a student left in the car for their drive time.
