NORTH CAROLINA HISPANIC COLLEGE FUND 2018-2019 Application North Carolina Society of Hispanic Professionals 8450 Chapel Hill Road, Suite 209, Cary, NC 27513 Telephone: 919-467-8424 - Fax: 919-469-1785 Email: mailbox@thencshp.org - Website: www.thencshp.org The NC Hispanic College Fund established by the NC Society of Hispanic Professionals awards annual scholarships worth between $500 and $2500. These scholarships are awarded to recent graduates of North Carolina High Schools who are of Hispanic/Latino background. Winners of a NC Hispanic College Fund scholarship must enroll or be enrolled in a degree or certificate program at a community college or a 2- or 4- year college/university and be committed to public service and community development. Important Dates Application is posted on our website all year and updated applications are posted at the beginning of each school year. They can be obtained by writing or calling the NCSHP or by downloading an application from the Internet. Final applications must be postmarked before or on January 15 th of each year. Award announcements will be mailed by March 5th. Scholarship Disbursement All scholarship awards will be sent directly to the college or university where the recipient enrolls. Scholarship funds will be designated for tuition and/or room and board. If these expenses have already been met, the award will be placed into an account at the educational institution for use by the student as needed. General Eligibility Criteria For High School Seniors: Applicant is of Hispanic background (one parent must be fully Hispanic or both parents must be half Hispanic). Applicant is a graduating senior from a North Carolina High School Applicant has a four-year cumulative GPA of 2.5 or better on a 4.0 scale (3.5 on a 5.0 scale) For College Students: Applicant is of Hispanic background (one parent must be fully Hispanic or both parents must be half Hispanic). Applicant has graduated from a NC high school within the past 2 years. Applicant is currently enrolled in a college or university and has a GPA of 2.5 or better on a 4.0 scale Page 1 of 10

Eligibility Criteria for Selected Scholarships Duke Energy Scholarship* o Applicant plans to major in and enter the field of: electrical, mechanical, nuclear chemical, or civil engineering. *Recipients of Duke Energy Scholarships will be awarded only to students that officially enroll in an engineering program. Proof of enrollment and claim of major will be requested. DO NOT SELECT THIS SCHOLARSHIP IF YOU ARE NOT PLANNING TO ENROLL IN A COLLEGE OR UNIVERSITY WITH AN ENGINEERING PROGRAM. PLEASE be prepared to provide necessary documentation proving that you are pursuing the fields of electrical, mechanical, nuclear, chemical or civil engineering. Juntos Program Scholarship o Applicant has participated in the Juntos program within the last 2 years in one or more of the following areas: 1. Juntos 4-H Club and/or Afterschool Program 2. Juntos Family Engagement Workshops and/or Juntos Family Night Outs 3. Mentoring and or Success Coaching 4. Juntos Academy 5. Academy Team Captain 6. Juntos College Mentors Selection Criteria: NOTE: All packets must be postmarked before or on the 15 th of January. Faxed or Emailed applications will NOT be accepted. Please submit the following items in one large sealed envelope to the NCSHP. Your envelope must be postmarked before or on January 15, 2018. 1. Two (2) Letters of Recommendation (in two separate signed and sealed envelopes): Please submit 2 letters of recommendation from a pastor, high school teacher, principal, employer or other community leader who can discuss your: Current academic status Academic and extracurricular achievements Personal qualities such as motivation, commitment, leadership, and goal orientation Potential for success and contributions to your community Financial need FAQ: What does it mean that the envelope is signed and sealed? Make sure that the envelope that you receive is completely closed / glued shut. We do NOT accept open envelopes. Make sure the person that makes the recommendation signs the envelope along the closure. This indicates that the envelope has not been opened once delivered. 2. An official high school transcript OR college transcript: The official transcript must be in a signed and sealed envelope and enclosed within the large envelope to be mailed directly to the NCSHP. For students enrolled in a college or university, a current college official transcript must be sent within the packet mailed to the NCSHP. 3. Personal statement: An essay of 500 words addressing briefly each of the following topics: Describe why having Hispanic heritage and family background are important to you Describe meaningful personal and/or academic achievements that you have had and why these achievements are important to you Describe your academic plans and career goals, giving specific examples Describe past and current efforts, as well as future plans, towards making a difference in your community Page 3 of 10 The personal statement is one of the most important selection criteria. It should be written with care and solely for the NC Hispanic College Fund. Title your essay Personal Statement, but do not use a separate cover sheet. Be sure to include the Personal Statement within your packet to be sent directly to the NCSHP. Please note that you will be reviewed based on the aforementioned topics in your personal statement. 4. Completed Application: Answer all questions legibly or by using a word processor. 5. Completed Resume outline 6. Pages 1 and 2 of your parents'1040 US Tax Form (and yours if applicable). Unfold and Remove all staples from all materials you send (except for those enclosed in a sealed envelope). 2. I understand that I must e-mail any changes in my academic status to mailbox@thencshp.org. Changes in academic status may result in award termination or suspension of your award. I understand that all award decisions and notifications are final and exceptions will not be made. I understand that application materials become the property of the NCHCF and will not be returned. I understand that any scholarship funds awarded to me will be distributed directly to the university or college in which I am enrolled. I understand that applicants and their relatives are barred from advocating for themselves by directly approaching selection committee members. Doing so may result in an automatic removal from consideration for a NC Hispanic College Fund scholarship. I understand that incomplete and late application materials will result in ineligibility and exceptions will not be made. I understand that I must reapply each eligible year with no guarantee of renewal if I am a scholarship winner. I give authorization to the NC Hispanic College Fund to share my contact information with scholarship company sponsors; including the name of the university or college I will be attending, and my career mayor/minor, for them to contact me for purposes of letting me know about internship and job opportunities. I give authorization to the NC Hispanic College Fund to publish my name, image, statements and/or writings including any and all photographic images and video or audio recordings in connection with my scholarship application for use in the NCSHP s print, online and video-based marketing materials, as well as other NCSHP publications.
