HOW TO ACCESS GOIEP 1. LOG INTO INFINITE CAMPUS AS USUAL. 2. SELECT SLDS ON THE INDEX TAB. 3. SELECT IEP IN THE SLDS TASK BAR. NOTE: IN ORDER TO SEE THE IEP BUTTON, USERS MUST BE ASSIGNED A ROLE. CONTACT THE SPECIAL EDUCATION GOIEP ADMINISTRATOR FOR ASSISTANCE IF THE IEP BUTTON IS NOT VISIBLE TO YOU. 3

GOIEP HOME PAGE/NAVIGATION LOGGED IN USER YOUR NAME. ROLE THE ROLE(S) IN GOIEP THAT HAVE BEEN ASSIGNED TO THE USER. THERE ARE ADMIN AND APPLICATION ROLES. o SYSTEM ADMINS CAN ASSIGN ROLES, ASSIGN STUDENTS TO CASE MANAGERS, VIEW ALL INFO ON ALL STUDENTS, CREATE AND EDIT IEPS, AND CREATE REPORTS. CONSULTANTS o SCHOOL ADMINS CAN ASSIGN ROLES TO ANYONE IN THEIR SCHOOL, ASSIGN STUDENTS IN THEIR SCHOOL TO CASE MANAGERS IN THEIR SCHOOL, VIEW ALL INFO ON ALL STUDENTS IN THEIR SCHOOL, CREATE AND EDIT IEPS, AND CREATE REPORTS. ISCs o MOST USERS WILL ONLY HAVE AN APPLICATION ROLE: SPECIAL ED TEACHER, GENERAL ED TEACHER, SCHOOL READ ONLY, SYSTEM READ ONLY. o SPECIAL ED TEACHER - MAY BE ASSIGNED AS A TEAM MEMBER OR ASSIGNED A CASELOAD. CASE MANAGERS AND TEAM MEMBERS CAN CREATE AN IEP, INITIATE, SAVE, RECORD THE RESULTS OF THE MEETING, ATTACH ELECTRONIC DOCUMENTS TO THE IEP. o A COURSE NUMBER IS AVAILABLE FOR THE PURPOSE OF CONNECTING SLPS, OTS, PTS, AND OTHERS TO A GROUP OF STUDENTS THROUGH THE STUDENT COURSE PROFILE IN ORDER TO ALLOW ACCESS TO SLDS AND GOIEP. o GENERAL ED TEACHER CAN VIEW ANY STUDENTS WITH IEPS THAT HAVE BEEN ASSIGNED TO THEIR CLASS OR FOR WHOM THEY HAVE BEEN ASSIGNED AS A TEAM MEMBER. CAN ONLY VIEW FINALIZED ACTIVE IEP FOR THE STUDENT. o SCHOOL AND SYSTEM READ ONLY CAN ACCES ANY ACTIVE IEP IN THE DISTRICT, CAN CREATE REPORTS, CANNOT MAKE ANY CHANGES. o DEFAULT READ ONLY ROLE WITH NO SPECIAL PERMISSIONS. 4

USER PROFILE - ALL USERS HAVE BEEN GIVEN A BASIC USER PROFILE INCLUDING F/L NAME, WORK NUMBER AND EMAIL SO THEY COULD BE ASSIGNED A ROLE IN GOIEP. USERS HAVE THE OPTION TO CLICK ON GREEN USER PROFILE TO ADD ADDITIONAL PROFILE INFORMATION IF DESIRED. CASELOAD/TEAM MEMBER - EACH CASE MANAGER IS ASSIGNED A CASELOAD BY THE ISC. THERE IS ONLY 1 PRIMARY CASE MANAGER FOR EACH STUDENT. o AFTER LOGGING INTO GOIEP, THE CASE MANAGER MAY SELECT THE CASELOAD LINK (GREEN TASK BAR) AT THE TOP OF THE PAGE TO VIEW A LIST OF ALL STUDENTS ASSIGNED. BASIC INFO SCHOOL, GRADE, DOB, IEP LAPSE DATE AND ELIGIBILITY LAPSE DATE - ARE DISPLAYED. SELECT STUDENT NAME TO GO TO STUDENT FILE. CASELOAD CHANGES GO THROUGH THE SCHOOL ISC. o BENEATH THE CASELOAD LIST IS A LIST OF STUDENTS THAT THE USER HAS BEEN ASSIGNED TO AS A TEAM MEMBER. TEAM MEMBER CHANGES GO THROUGH THE STUDENT S CASE MANAGER. PROGRESS REPORTS SELECTING THIS LINK WILL DISPLAY THE PROGRESS REPORTS DASHBOARD. THIS IS WHERE PROGRESS IS EVALUATED FOR EACH STUDENT. A LIST OF ALL CASELOAD STUDENTS AS WELL AS A LIST OF ALL TEAM MEMBER STUDENTS WILL SHOW. COLORS SIGNIFY WHETHER THE PROGRESS REPORT IS COMING DUE, OVERDUE, OR THAT THE REPORT HAS BEEN COMPLETED. IF A PROGRESS REPORT IS OVERDUE, TWO LINES WILL BE PRESENT FOR EACH STUDENT ONE FOR THE OVERDUE REPORT AND ONE FOR THE CURRENT REPORT. DASHBOARD TERMS MAY BE FOUND IN THE HELP DOCUMENT. ONCE PROGRESS HAS BEEN ENTERED FOR ALL GOALS AND OBJECTIVES, THE CREATE REPORT LINK WILL BECOME AVAILABLE ALLOWING THE CASE MANAGER TO PRINT THE PARENT COPY TO SEND HOME. SEE INDIVIDUAL PROGRESS REPORT HANDOUT. REPORTS - THERE ARE MANY REPORTS AVAILABLE IN GOIEP. EXPORT ALL REPORTS TO EXCEL TO PRINT. SEE INDIVIDUAL REPORTS HANDOUT. HELP DOCUMENT - AT THE END OF THE GREEN TASK BAR YOU WILL SEE THE LINK TO THE HELP DOCUMENT. THIS DOCUMENT IS EXTENSIVE AND WILL BE UPDATED BY THE GADOE AS NEEDED. THIS DOCUMENT WILL SERVE AS YOUR FIRST GO-TO RESOURCE. o DO NOT CONFUSE THE SLDS AND GOIEP HELP DOCUMENTS. THE HELP& TRAINING LINK IN THE BLUE BAR AT THE TOP IS FOR SLDS ONLY. 5

HOW TO ACCESS AND UPDATE YOUR USER PROFILE 1. LOG INTO GOIEP. 2. SELECT USER PROFILE. 3. AT A MINIMUM, ALL USERS SHOULD ENTER FIRST NAME, LAST NAME, WORK EMAIL ADDRESS, AND WORK TELEPHONE NUMBER. THESE FIELDS PRE-POPULATE FORMS CREATED LATER IN GOIEP. 4. SELECT SAVE PROFILE. 6

HOW TO SEARCH FOR A STUDENT IN GOIEP 1. LOG INTO GOIEP. 2. SELECT ADVANCED SEARCH TO ACCESS THE LOOKUP SCREEN SHOWN BELOW. ALL STUDENTS (REGULAR AND SPECIAL EDUCATION) WILL APPEAR IN YOUR SEARCH RESULTS. 3. ENTER FIRST INITIAL, LAST NAME. (THE FIRST 50 STUDENTS MATCHING THE CRITERIA WILL APPEAR IN THE SEARCH RESULTS. YOU MAY FURTHER LIMIT BY ENTERING THE ENTIRE FIRST NAME, IF NEEDED.) 4. YOU MAY LIMIT SEARCH RESULTS BY SELECTING STUDENTS WITH CASE MANAGER OR STUDENTS WITH DISABILITIES (SWD). 5. SELECT LOOKUP STUDENT. NOTE: IF THE STUDENT HAS A CASE MANAGER ASSIGNED, THE STUDENT S NAME WILL BE PRINTED IN GREEN (LINKED) AND YOU WILL BE ABLE TO SELECT THE STUDENT S NAME TO GO TO THE STUDENT PROFILE PAGE. IF THE STUDENT DOES NOT HAVE A CASE MANAGER ASSIGNED, THE STUDENT NAME WILL BE PRINTED IN BLACK (NOT LINKED) AND A CASE MANAGER MUST BE ASSIGNED BEFORE MOVING ON TO THE PROFILE PAGE. SEE DIRECTIONS FOR ASSIGNING A CASE MANAGER. 7

HOW TO ASSIGN/REASSIGN A CASE MANAGER IN GOIEP 1. SCHOOL OR SYSTEM ADMINS MAY ASSIGN CASE MANAGERS. LOG INTO GOIEP. 2. SEACH FOR THE STUDENT. ICON AND NAME PRINTED IN GREEN (LINKED) INDICATES THAT THE STUDENT HAS BEEN ASSIGNED A CASE MANAGER - CLICK ON THE ICON TO CHANGE THE CASE MANAGER. (CASE MANAGERS MAY NOT BE DELETED ONCE ASSIGNED.) ICON AND NAME PRINTED IN BLACK (NOT LINKED) INDICATES THAT THE STUDENT HAS NOT BEEN ASSIGNED A CASE MANAGER CLICK ON THE ICON TO ASSIGN A CASE MANAGER. 3. CLICK ON THE ICON TO ASSIGN/REASSIGN A CASE MANAGER. 4. SEARCH FOR CASE MANAGER BY LAST NAME OR BY SCHOOL. 5. SCROLL DOWN TO THE DESIRED CASE MANAGER OR SEARCH FOR THE CASE MANAGER BY LAST NAME AND SELECT THE BLUE ASSIGN BUTTON. ONCE ASSIGNED, CASE MANAGERS MAY NOT BE DELETED. A NEW CASE MANAGER MAY BE ASSIGNED IF SOMEONE WAS ASSIGNED IN ERROR OR IF THE CASE MANAGER HAS CHANGED BY SELECTING THE ICON AND SELECTING CHANGE CASE MANAGER. 8

THE PROFILE TAB 1. Student and School Information Sections 1 and 2 auto-populate from the previous June Student Record data submission and SIS. GTID/Name corrections should be reported to system tech admin to be changed. The address section may be modified in GOIEP. Notify the school SIS clerk of address changes so that Campus may be updated, as well. Save. 2. Parents/Guardians Select ADD PARENT/GUARDIAN/SURROGATE to add parent information. 9

At a minimum, add FIRST/LAST NAME and RELATIONSHIP. This information populates team member drop-downs later in the GOIEP application. Save. Parent information may be UPDATED by selecting the parent name or DELETED by selecting the X. Select ADD PARENT/GUARDIAN/SURROGATE to add as many parents/guardians as appropriate for the selected student. 3. Assessment Data State assessment data auto-populates from SLDS. When available, all assessment data will be visible in chronological order with most recent administration at the top. (NOTE: Only the most recent 3 years of data imports into the IEP application.) 10

Until GKIDS and ACCESS results are available through SLDS, case managers may enter these results manually. o Select ADD GKIDS or ADD ACCESS. o Enter ASSESSMENT DATE, LANGUAGE, and RESULT. o Save. 4. Environment Code The Environment will auto-calculate based on information entered in GOIEP. 11

TEAM MEMBERS TAB Once assigned as the case manager by the ISC, case managers must add IEP team members for each student on their caseload. Team members will be able to view IEPs of students they are assigned to and will autopopulate meeting notices; therefore, appropriate staff members who will need to view IEPs and be invited to IEP team meetings should be added to the list of team members at the beginning of each year. Notice that students are automatically added to their IEP team. 1. To add a team member who is a GOIEP SYSTEM USER: Select user type SYSTEM USERS. Select team member name from the EXISTING USERS drop-down. Assign team member the appropriate IEP TEAM TITLE using the drop-down. Select ADD TEAM MEMBER. 2. To add a PARENT: (Parent must have been added on the Profile Tab.) Select user type PARENT/GUARDIANS. Select parent name from the existing users drop-down. Notice that IEP team title automatically changes to PARENT. Select ADD TEAM MEMBER. o If no parent name appears, return to the profile tab to add parent information. 3. To add other team members who are not listed above: Select user type OTHER. Enter at minimum LAST NAME AND FIRST NAME. Assign team member the appropriate IEP TEAM TITLE using the drop-down. Select ADD TEAM MEMBER. The case manager should delete IEP team members when they no longer have a need to view or edit the IEP or be invited to a meeting for the student. Team members may be deleted by selecting the red DELETE in the last column. 12

TIMELINES TAB The Timelines page lists all state and federally mandated event dates for each student, when available. The timelines page is the GO TO page to quickly check student status and it drives the rest of the GOIEP application by determining what meetings need to be held, when IEPs should be written, and other timelinerelated tasks. The timelines page itself is not editable by the case manager, but is auto-populated from other fields within the GOIEP application and SR information from the DOE. 1. Student Information o IEP lapse date/primary & Secondary Disability fill from IEP. 2. Countdown o Provides live countdown of days left to complete evaluation/re-evaluation/iep. 3. Override Dates o This can only be completed ONCE! This allows the case manager to correct any incorrect dates from SR submission. DO NOT submit until all dates are verified to be correct. View only access after submission. 4. Referral Reason o Shows initial referral reason if completed in application, otherwise blank. 5. Consent for Evaluation Form: Date sent, date signed, and date received. o New referrals completed in GOIEP: 3 dates fill from the application. o Others: Received date imported from student record, others blank. 6. Special Ed Eligibility: Date of Initial Evaluation, Date of Initial Special Ed Elig in GA, Did the Initial Special Ed Elig meeting determine that the student has a disability? o New referrals completed in GOIEP: All fill from the initial eligibility. o Others: Imported from student record. 7. IEP Meeting: Initial IEP Meeting Date, Initial IEP Meeting Attendance Parent/Student, Date parent signed consent for initial placement in Special Ed in GA. o New referrals completed in GOIEP: All fill from application. o Others: Meeting date imported from student record, others blank. 8. Date of Initiation of Special Ed Services Initiation in GA: o New students: Filled from implementation date of initial IEP in application. o Others: Date imported from student record. 9. Annual Review: IEP Date, IEP Service Start Date, and IEP Service End Date. o New students: Fill from IEP application. o Others: Dates imported from student record or mock IEP. 10. Re-Evaluation: Eligibility Date, Lapse Date, End Date. o New students: Fills from Eligibility application. o Others: Imported from student record or mock eligibility. 13

HOW TO VERIFY IMPORTED TIMELINE DATES AND COMPLETE THE OVERRIDE TIMELINES PAGE THE TIMELINES VERIFICATION PAGE WAS ADDED JULY, 2013 TO ALLOW SYSTEMS AN OPPORTUNITY TO EDIT EVENT DATES SUBMITTED IN STUDENT RECORD THE PREVIOUS YEAR OR ADD EVENT DATES WHICH WERE NOT SUBMITTED AS PART OF STUDENT RECORD. ALL EVENT DATES SHOULD BE VERIFIED FOR ACCURACY USING THE STUDENT S PAPER OR ELECTRONIC FILE. IT IS VERY IMPORTANT TO ENSURE ACCURACY IN THE VERIFICATION PROCESS BECAUSE ONCE SUBMITTED, THE DOE MUST BE CONTACTED IN ORDER TO MAKE ANY CHANGES. 1. BEFORE BEGINNING AN IEP OR ELIGIBILITY IN GOIEP, THE OVERRIDE TIMELINES PAGE MUST BE SUBMITTED EITHER WITH OR WITHOUT CHANGES. 2. CASE MANAGERS SHOULD COMPLETE THE IEP CASELOAD VERIFICATION FORM AND IF NO CHANGES ARE REQUIRED ON THE TIMELINES OVERRIDE PAGE, THE CASE MANAGER AND/OR ISC MAY SUBMIT THE FORM WITH NO CHANGES. IN THIS CASE, THERE IS NO REQUIREMENT FOR A SYSTEM ADMIN TO VERIFY THE SUBMISSION. 15

3. IF INCORRECT OR MISSING (REQUIRED) DATES ARE IDENTIFIED THROUGH THE VERIFICATION PROCESS, CASE MANAGERS AND/OR ISC SHOULD SELECT ADD/CHANGE, ENTER THE CORRECT DATE IN THE OVERRIDE SR HISTORY DATES POPUP, SELECT THE REASON FOR THE ADDITION OR CHANGE, AND SAVE. A COMMENT IS REQUIRED FROM THE CASE MANAGER OR THE ISC AND THE APPROPRIATE SYSTEM ADMIN MUST BE CONTACTED TO (CONSULTANT/PROGRAM SPECIALIST) TO VERIFY EACH CHANGE. 16

MEETINGS Meetings are the heart of GOIEP. All meetings are started from the Timelines page. Available meetings will be displayed as links on the timeline page. To get started, select the link to the meeting that you want to create. In the case of initial eligibility, once the Consent for Evaluation Form is complete, the START INITIAL ELIGIBILITY link becomes available. 1. Select START INITIAL ELIGIBILITY. The MEETINGS page will come up automatically. 2. Select Modify Meeting Date/Time. Enter meeting date/time. Save. Close. 3. Select Modify Meeting Location. Select location or select other to be able to enter a more specific location. Save. Close. 4. Select Modify Meeting Purpose. Select appropriate purpose(s). Save. Close. The Meeting Purpose auto-populates for some meetings, but you may need to select additional purposes. (Annual Review may auto-populate but you will need to also select Develop/Update Transition Plan, if appropriate.) Only the appropriate meeting purposes for the type of meeting selected will show in the drop-down. You will not be able to change the purpose of the meeting that was selected from the Timelines page. (Notice the gray check-mark.) 17

5. Select Modify Parental Rights. Select applicable option. Save. Close. 6. MEETING ATTENDEES. All IEP team members listed on the Team Members page automatically appear in this list. You may delete any team members who will not be attending this meeting. Add missing Meeting Roles by selecting EDIT. Select role. Save. Close. o An attendee s Meeting Role may be different than Team Title depending on the needs of the student. (The IEP Case Manager may serve as the Special Education Teacher and School Psychologist for the meeting, for example.) o Ensure all required team roles are filled. Add additional attendees by selecting ADD ATTENDEE. 7. Select SUBMIT when complete. Correct any errors (warnings) by reopening that section. PARENT RESPONSE/RESCHEDULING MEETINGS Once created, all meetings related to a student are listed on the MEETINGS TAB along with the date and time of the meeting and the status of the meeting. A meeting can be rescheduled or modified at any time before the meeting is held. 1. Select Meeting Name from list of scheduled meetings on Meetings Tab. 2. Select Response/Reschedule Meeting button. 18

3. Select appropriate option from the choices (only 1 may be selected): Meeting will proceed as scheduled. o Enter parent response. o Save. Close. Reschedule Meeting. o Enter reason for rescheduling the meeting. o Save. Close. o Modify the Meeting Information. Submit. Reopen Meeting to Change Information (use this option to make corrections). o Save. Close. o Modify the Meeting Information. Submit. GOIEP keeps a running list of each notice submitted and each parent response entered. 19

PARENTAL CONSENT FOR EVALUTION FORM Initial referrals to special education will have the START or INITIATE CONSENT TO EVALUATE link available on the Timelines page. 1. Enter Referred By and Date Sent. Future dates may not be entered. o Notice that the contact name, telephone number, and title are auto-populated by case manager information entered in the user profile. All fields are editable. 2. Save. Once saved, the PRINT PARENT CONSENT link becomes available on the Timelines page. Print form for parent. 3. Once the signed consent is received by school personnel, update GOIEP with response information. 4. Select CONSENT TO EVALUATE IN PROCESS link. 5. Enter updated response information: o Consent received? Y/N (add any parent comments in open text box) o Enter name of parent who signed, telephone number, date signed, and date received by ANY school personnel (may include teacher, administrator, or office support staff). 6. Select Save Parent Consent. 20

Once saved, the Consent link on the Timelines page changes to VIEW CONSENT TO EVALUATE, indicating that no further changes may be made. The link to START INITIAL ELIGIBILITY also becomes available at this point. (Once the meeting is started, Initial Eligibility and Initial IEP may be in-process simultaneously.) 21

ELIGIBILITY & IEP TABS 1. Eligibility Tab o For Initial Referrals to special education, once the Consent for Evaluation Form is complete and the parent response info is entered and the meeting notice has been submitted, the Eligibility Tab changes from nothing available to View/Update. For reevaluations, the View/Update becomes available once the Re-evaluation has been initiated on the timelines page and meeting has been scheduled/submitted. o Select View/Update to begin the eligibility report. 2. IEP Tab o For Initial Referrals to special education, once the Consent for Evaluation Form is completed, the parent response info is entered, and the meeting notice has been submitted, the IEP Tab changes from nothing available to View/Update. For annual reviews and re-evalutations, the View/Update becomes available once the review has been initiated on the timelines page and meeting has been scheduled/submitted. o Select VIEW/UPDATE IEP to begin the IEP. Once Eligibility Reports and IEPs have been completed using the GOIEP application for a student, they will be available here to view (or update according to your role) along with basic information about the document. (IEP date, IEP Service Start Date, IEP Service End Date, Primary Disability, Status, and Is Initial IEP true/false.) 22

LOG TAB There are 2 types of Logs in GOIEP: LOG AND DOCUMENTS TABS Access Logs display the name of anyone who has accessed the student s IEP record. Access Logs display the most recent 50 people who have accessed the specific student s record and viewed the profile page. Select More Records to see more listed. Contact Logs display contacts between the school and parent. The Contact Log records all contacts made between the school and parent, some automatically, others manually entered by an IEP team member. Add contacts by selecting Add Contact Log. DOCUMENTS TAB The Documents Tab provides a place to attach outside documents to the student s special education file. At this time, only.doc,.xls,.pdf, and.txt document types will be readable once uploaded. Office 2007.docx and.xlsx documents will be corrupted. DOE advice is to convert documents to PDF by saving as PDF or XPS before uploading. 1. To upload a document saved on your computer, select Documents Tab. 2. Select browse to search for your document. Select document and select Open. 3. Give your document a SPECIFIC NAME INCLUDING DOC DATE to make it easier to identify later. 4. Select Document Type from the drop-down. 5. Enter Document Date. 6. Save. Uploaded documents are listed at the bottom in order of upload and include Document Name, Type, Uploaded by, and Upload Date information. 27

COMPLETING PROGRESS REPORTS Select the PROGRESS REPORTS link on the green task bar to display the PROGRESS DASHBOARD which contains a list of all students on the user s caseload as well as all students for whom the teacher has been assigned as a team member. Users may add progress notes to any report listed, as appropriate. The Dashboard shows the frequency progress is to be reported, the frequency dates that are included in the period, the total number of goals (and objectives) in the IEP, the number of goals (and objectives) that have been completed in the specified report, and the last date progress was completed and reported for each student. Currently, the FREQUENCY PERIOD automatically begins the first Tuesday after the start date of the IEP, and ends on the Monday at the end of the frequency period (nine weeks later). NOTE THAT IT IS IN THE GOIEP BACK-LOG TO ALLOW SYSTEMS TO ALLOW LOCAL SYSTEMS TO CHANGE THE REPORTING PERIODS TO MATCH LOCAL SYSTEM GENERAL EDUCATION REPORTING PERIODS. FOR NOW, EVALUATE PROGRESS EARLY ON REGULAR SYSTEM DATES. Each student line is color-coded: Red indicates the report is overdue. Yellow indicates the report is due within a week. Green indicates everything is OK. REPORT DETAILS TAB Select a student s name to show the progress report details for the student. 28

PROGRESS REPORT DETAILS This page lists all goals and/or objectives from the selected student s IEP, along with the evaluation method and measurement component. Select ADD A PROGRESS NOTE to add progress for each goal (and objective). ADDING PROGRESS NOTES Enter the DATE, the correct PROGRESS CODE from the drop-down, and relevant comments (percentages, for example) to each goal (and objective). Remember that any user assigned to the IEP team may enter progress for students. It is the responsibility of the case manager to ensure progress is reported on time for all goals (and objectives) and the report is printed and sent home. If a goal is marked as Functional, an additional option has been added stating that the goal/objective was not assessed during this progress period but was previously reported as functional. REPORT DETAILS Once entered, all progress notes are displayed on the REPORT DETAILS TAB. 29

PRINTING PROGRESS REPORTS When progress has been entered for all goals (and objectives), the CREATE REPORT link on the PROGRESS DASHBOARD (first column) will become available. The IEP case manager should verify that progress has been reported for each goal (and objective), select the CREATE REPORT link to bring up the completed progress report, and print to share with parent. CREATING REPORTS There are many reports available in GO-IEP. Following are some examples of the types of reports that can be created and exported to Excel as needed. NAVIGATION: IEP REPORTS - STUDENTS BY CASELOAD Uses: Administrators may use this report to see a list of all students in their system or school with case managers, or to filter for all students on a particular caseload. A full system or school report can be created or reports can be filtered by selecting School or Case Manager from drop down list. The following options are available and only one option can be selected at a time. IEP LAPSED - Reports will be generated for students whose IEP has lapsed. This report will help system and school administrators, as well as case managers, determine whether any student for whom they are responsible has an IEP which has already lapsed and no more current IEP. IEP LAPSE IN () MONTHS - Reports will be generated for students whose IEP will lapse in selected number of months. This report will help system and school administrators, as well as case managers, determine whether any student for whom they are responsible has a current IEP which will lapse is the designated number of months. ELIGIBILITY LAPSED - Reports will be generated for students whose eligibility has lapsed. This report will help system and school administrators, as well as case managers, determine whether any student for whom they are responsible has an eligibility report which has already lapsed and no more current eligibility report. 31

ELIGIBILITY LAPSE IN () MONTHS - Reports will be generated for students whose eligibility will lapse in selected number of months. This report will help system and school administrators, as well as case managers, determine whether any student for whom they are responsible has a current eligibility which will lapse is the designated number of months. GAA STUDENT This report will help system and school administrators, as well as case managers, determine all students for whom they are responsible whose active IEP indicates they will participate in GAA. REPORTS WILL CONSIST OF THE FOLLOWING INFO AND COLUMNS First Name, Last Name, Date of Birth, Grade, IEP Case manager, School Name, IEP Lapse Date, Eligibility Lapse Date IEP REPORTS STUDENT SERVICES A full system or school report can be created or reports can be filtered by selecting School, Case Manager or Student Name from drop down list. This report will help system and school administrators, as well as case managers, determine all services listed in the active IEP for all students for whom they are responsible. This report could be used to compare IEP services to a student's schedule at the beginning of every school year or semester. This report will also be helpful during FTE reporting periods. To limit the report to only services that are in effect on a given date, filter the Service Begin Date for dates less than or equal to that date and then filter the Service End Date for dates greater than or equal to that same date. 32

Once exported to excel, this report can be filtered to identify the school, grade, and/or content areas in which each student will participate. IEP REPORTS - ACCOMMODATIONS A full system or school report can be created or reports can be filtered by selecting a School or Case Manager from drop down list. This report will help system and school administrators, as well as case managers, determine the IEP driven accommodations for state-required assessments for all students for whom they are responsible. Once exported to excel, this report can be filtered by the school, grade, assessment, and/or content area. This report can be used to assist testing coordinators with grouping of students and allow systems to ensure that appropriate accommodations are provided. Accommodation Report will consist of the columns listed at the bottom of the following screen shot: IEP REPORTS EXTENDED SCHOOL YEAR A full system or school report can be created or reports can be filtered by selecting a School or Case Manager from drop down list. This report will help system and school administrators, as well as case managers, determine all students for whom they are responsible whose active IEP indicates that the ESY decision was deferred (and the IEP still needs to be amended to make the ESY decision) as well as those students who will receive ESY services. Additionally, the report will indicate those students who require AT devices and/or services. Assistive Technology Reports will consist of the columns listed at the bottom of the following screen shot: SPECIAL CONSIDERATION - COMMUNICATION This report will help system and school administrators, as well as case managers, determine all students for whom they are responsible whose active IEP indicates the student has communication needs. SPECIAL CONSIDERATION ALTERNATE FORMAT This report will help system and school administrators, as well as case managers, determine all students for whom they are responsible whose active IEP indicates the student requires an alternate format. SPECIAL CONSIDERATION DEAF OR HEARING IMPAIRMENT This report will help system and school administrators, as well as case managers, determine all students for 35
