Page 12 Other Services Transitional Care (Swing Bed) Program features a service known as transitional care, which is a short-term, extended care program that patients may be eligible for following hospitalization for an acute problem. If your condition has stabilized, but you are not ready to return home, you may be eligible for this program. In order to be admitted to the transitional care unit, you must meet the following criteria: Your transitional care must be related to an acute hospital stay You must need skilled nursing or skilled rehabilitative services on a daily basis You must have spent three overnight stays in acute care during the past 30 days The following treatments are offered in Transitional Care: Occupational therapy Physical therapy Speech therapy Intravenous therapy Patient education (such as learning about living with diabetes or how to care for a colostomy) To find out more about Transitional Care, call Patient Coordinator Pam Tavernier at 217-322-4321, ext. 295, or via email at: ptavenier@sdcmh.org Senior Life Solutions Senior Life Solutions focuses on the health and well-being of aging adults.
