Checking Document Status In order to be eligible for need-based aid, all documents must be marked as Received or Waived under the Documents tab in your MyFinancialAid portal. All documents must be received by April 1 st, 2017. FAFSA Login using your FSA ID and password. If you do not have a new FSA ID, you can create one here. Complete the 2017-2018 FAFSA using school code E00079. You will only be required to provide student/spouse financial information to process the FAFSA (not parental information). **Please be sure to provide an email address in the Student Demographic Information section** 4
CWRU Financial Aid Application Prior to completing the CWRU Application, you must submit your social security number in the iapply portal. You will be provided with your CWRU ID and pin in your iapply portal approximately 48-72 hours after you submit your SSN (slightly longer timeframe during high volume periods). Use your ID and pin to activate your CWRU ID. Once you have activated your ID, you will have access to the MyFinancialAid Portal within 24-48 hours. Login to MyFinancialAid here by selecting: Application Procedures Login to MyFinancialAid, from the side menu. Once logged in, select the CWRU Financial Aid Application tab The application consists of demographic questions used to award institutional and outside scholarships, when available. 5
Proof of Citizenship The Department of Education requires our office to verify some students' citizenship status. You are required to submit appropriate documentation, most often a birth certificate, certificate of naturalization, Permanent Resident Card, or a valid U.S. passport. Please submit this document through OnBase, the secure document imaging system. More information regarding OnBase can be found on slides #12-13. **Please see slide #11 for submission guidelines** 6
CSS Profile Complete the 2017-2018 CSS PROFILE using the CWRU School of Medicine code 1104 You may view this video and follow the instructions here to complete the application. Both student and biological parents information are required. If your parents are divorced or separated, your custodial parent should complete the PROFILE. Your non-custodial parent must complete the Non-custodial PROFILE (NCP). If you wish to have College Board email the NCP link to your non-custodial parent, you must provide your non-custodial parent's email address on the CSS PROFILE application. **Please follow the Instructions on slide #11 for submission guidelines** 7
2015 Federal Tax Returns & W-2s Copies of verified 2015 tax returns (including all applicable schedules: A,B,C,D, etc.) and 2015 W-2 forms will required to be considered for needbased institutional aid. Tax Waivers If you, your spouse, or your parents did not and will not file a federal tax return for 2015, please sign and date the Tax Waiver Form and submit through OnBase, the secure document imaging system. Contact the Financial Aid Office to receive the Tax Waiver Form. More information regarding OnBase can be found on slides #12-13. **Please see slide #11 for submission guidelines** 9
2017-18 CWRU Loan Authorization Form If you are NOT applying for CWRU institutional need-based aid, which consists of need-based grant and university loan, please complete the Loan Authorization Form to be awarded only Federal Direct Loans. You will need to provide your name and DOB to complete the form. You can find the Loan Authorization Form here. 10

OnBase: Secure Document Imaging System OnBase is a secure document imaging system that we will be utilizing for document submission. We will not accept emailed or faxed documents. Students and families will be able to securely upload tax returns, W-2s, and other requested documents directly to OnBase. You will be required to provide your CWRU Network ID (ex: abc123) to ensure your documents are indexed correctly. AFTER you have been issued your student ID and registration numbers in iapply (see below), you may access the OnBase Submission Form to upload your required tax documents. OnBase: How to Upload Documents 1 Enter your Network ID (ex: abc123). The rest of your information should auto-fill. If the information does not auto-fill, or the auto-filled information is incorrect, please manually enter the correct information. 2 Select Attach (document name) for each document type that you would like to submit. You can submit all of the required documents at one time, or you can return to the form to submit additional documents at a later date. Please allow up to one week processing time for documents to be marked as Received in your MyFinancialAid portal. **You submission has been successful once you receive the blank screen after clicking the submit button** 13
