Your non-refundable tuition deposit of $500 must be included with this form to reserve a place for your child in the 2015-2016 academic year. This deposit will be deducted from your last tuition payment. Please retain a copy for your records. The average true cost of a St. Francis education is $6,620 per student. In our efforts to keep tuition affordable for all students, we offer tuition at the tuition rates in the table below, granting a subsidy for qualifying Catholics and parish families. Each family is encouraged to pay the unsubsidized tuition which will qualify for a tax-deductible contribution. This will assist the school in its efforts to bridge the tuition gap while also providing critical tuition assistance to students in need. Section I. Fees and Payment (There are three tuition plans available): In Full Option 1: Catholic Tuition: In 4 Payments In 10 Payments Kindergarten $6,576.00 $1,644.00 $657.60 Grades 1-8 $5,600.00 $1,400.00 $560.00 * In order to apply for this tuition rate the child attending school needs to be a baptized Catholic. There is no tax deductible contribution in this plan except for families that qualify for the parish subsidy and elect to forego the subsidy by paying the Catholic Tuition rate. Option 2: Catholic Parish Subsidized Tuition (requires approval by Pastor): Kindergarten $5,576.00 $1,394.00 $557.60 Grades 1-8 $4,600.00 $1,150.00 $460.00 *There is no tax deductible contribution in this plan. However, any amount paid above the tuition is a tax-deductible donation. This option is available only to Catholic parishioners who apply for the parish subsidy and are approved by the parish Pastor. In order to apply for this subsidy each family must complete and turn in the parish subsidy form by the parish deadline. Please see the Parish Subsidy section below for more details. Option 3: Unsubsidized Tuition: Kindergarten $6,982.00 $1,745.50 $698.20 Grades 1-8 $6,006.00 $1,501.50 $600.60 * This represents the full unsubsidized cost of educating your child at St. Francis School. Non-Catholics are required to pay this tuition. Each qualifying Parish or Catholic family is encouraged to pay the unsubsidized tuition rate, which will assist the school in its efforts to bridge the tuition gap while also providing critical tuition assistance to students in need. This option includes a tax-deductible donation in the amount of $1406 for parishioners that qualify for the parish subsidy and a donation in the amount of $406 for families that qualify for the Catholic tuition rate. Payments will be billed via monthly statement and are due on the 20 th of each month starting in August. Unconditional Promise to Pay: I understand that the overhead expenses of the school do not diminish with the departure of a student during the course of the school year and that my obligation to pay the full annual tuition and other fees for the academic year is unconditional after acceptance of this contract by St. Francis School or July 31, 2015, whichever is earlier. All decisions not to return must be in writing to the principal and received in the office prior to July 31, 2015 to avoid responsibility under this contract. I further understand I am responsible for paying the full tuition and that no portion of the full annual tuition cost paid or outstanding will be refunded or canceled even in the event of a withdrawal of the above student. Collection of the full annual tuition cost and other fees may be waived only upon the sole discretion of the Pastor and Principal in the following cases: (1) the family relocated their residence more than 50 miles from St. Francis School; (2) the student is expelled by the school; (3) financial hardship that extends beyond actively seeking financial assistance and is approved by the Pastor and Principal of St. Francis; and (4) a disability for which the Pastor and Principal determine the school cannot meet the needs of the child. I further agree to pay, to the extent permitted by law, the school s expenses of enforcement and collection of the tuition, fees and related expenses, including, without limitation, attorney s fees and costs if I fail to make timely payments. Section III. Rules and Regulations: I agree to accept the rules, regulations and policies adopted by St. Francis School and make them a part of this contract. I understand that the student s disregard of the rules, regulations and policies of St. Francis School may be deemed sufficient cause for expulsion. St. Francis reserves the right to make changes to its rules and regulations and policies at any time. It is my responsibility to be familiar with the rules, regulations and policies and to ensure that my child adheres to them. Section IV. Understanding and Acceptance: I have initialed each page of this contract to signify that I have read and understand it. I hereby accept the place at St. Francis School reserved for the 2015-2016 school year. By signing below I agree to the terms stated in this Enrollment Contract. I may only cancel this contract in writing and delivering it to the school office on or before July 31, 2015. I understand that my registration fees are non-refundable. Additional Conditions: Policy on Share Hours In order to control tuition costs, all parents are expected to be involved in helping care for the school community. Each hour worked counts as one share hour. Each family is expected to contribute the required hours of share time and submit the report to the School office. The Share hour requirements are as follows: a) A 2-parent household must contribute a total of 40 share hours, 10 of which must be used for auction related tasks; a single-parent household is required to contribute 20 share hours, 5 of which must be used for auction related tasks. b) Each 5 th grade family is required to contribute a total of 7 share hours towards the Friar Trot. c) The 2015/2016 3 rd grade class will be responsible for overseeing the management of the 2016/2017 Auction. All families within the class must commit to a lead role in the Auction by February 19 th, 2016 or will be assigned a lead role. d) The 2015/2016 2 nd grade class will be responsible for shadowing a lead role in the 2016/2017 Auction. All families within the class must commit to a lead shadow role in the Auction by February 19 th, 2016 or will be assigned a lead role to shadow. e) The 1 st grade parents are responsible for managing the Jog-a-thon and each family is required to contribute hours to this program. f) Any parent not wishing to contribute his/her share hours will be assessed a $50.00 per hour charge for uncompleted time. Auction hours may not be bought out. It is recommended that half of your share hours be completed by January 31. g) All share hours for the current year must be completed by June 1st and the share hour information must be turned in by noon on June 1st. h) Any unfulfilled share hours in which you are charged will appear on your June statement. i) Every family will be required to procure an item(s) for the Auction of at least $100 total value or donate $100 in cash. Procured items, or cash, must be submitted no later than October 2nd, 2015. j) In addition, every class has a Classroom Showcase which is a themed collection that is offered at the Auction. Every school family will be required to procure an item(s) for each of their students classroom showcases of at least $25 in total value or donate $25 in cash. Procured items, or cash, must be submitted no later than October 2nd, 2015. Account Payment Policy All expenses will be billed on a monthly statement and payment must be made at the School office. All accounts for students in grades K-7 must be current in payments in order to register by April for the upcoming year and must be paid in full by June 20 th or the contract for the forthcoming year will be held until paid. No student will be allowed to enroll until the prior year school account balance is paid in full except on approval by the Parish Pastor and the Principal. All accounts for students in 8 th grade must be paid in full prior to graduation or they will not be permitted to graduate. Payment by EFT ( Electronic Funds Transfer ) If your option is to pay monthly, your monthly balance can be paid in full by EFT on the 20 th of the month. If the 20 th is a Saturday, Sunday or Holiday, the EFT will occur on the next business day. Any changes to your EFT account must be made in writing no later than the close of business on the 7 th day of the month in which you wish the change to occur. If you wish to terminate your EFT account, written notice must be given to the School office no later than the close of business on the 7 th day of the month in which you wish the termination to occur. If an EFT is return NSF ( Not Sufficient Funds ), you may be assessed an additional service charge of up to $50 per event and a late fee of $50.00. This is in addition to any charges you may be assessed by your bank. Past Due Accounts and NSF Returns Accounts that are not paid by 5 PM on the 25 th of the month will be assessed a $50 late fee, which will appear on your next statement. Families will receive a letter advising them that if payment is not received by the 1 st Monday of the upcoming month the student(s) may be excluded from school until payment is received. As a courtesy only, a phone call may be made on the last day of the month to remind the family of the non-payment consequences. For NSF checks or EFTs, you may be assessed an additional service charge of up to $50 per event and a late fee of $50. This is in addition to any charges you may be assessed by your bank. Parish Subsidy A qualifying parishioner may apply for a parish subsidy in the amount of $1000 per student. In order to qualify for the parish subsidy the parishioner must submit a written application to the parish office for the parish subsidy and it must be approved by the Parish Pastor. The deadline for submitting the written application to the Parish is March 4 th, 2014. Failure to submit the application by the deadline will result in denial of the parish subsidy. Parishioners are encouraged to donate their parish subsidies to the school in the form of a tax deductible contribution. Special Circumstances The school realizes that families sometimes have unforeseen financial situations which may cause school accounts to go unpaid. If a family finds itself in such a situation, arrangements to pay any outstanding account balance via an approved payment plan should be made with the Principal on or before the due date of the monthly statement to avoid the $50 late fee. An approved payment plan is a written document that is approved by the Parish Pastor and Principal, is signed by both parties and which addresses the student(s) current and future payment responsibilities under this Agreement. Withdrawal of Student(s) If you withdraw your child(ren) prior to the end of the school year and meet one of the exceptions for payment in full of your contract, you will be responsible for payment of tuition through the end of the month in which you withdraw.
