C0-Teaching Observation Form 2. Level of Questions Form 3. DI Best Practices Inventory GaDOE & ETC Staff Only (Upon completion of all walk-through observations, will enter data into a data management system and will email results back to building administration to use for guidance and support of co- teachers) (These monthly online assignments will supplement the Face-to- Face Co-Teaching Trainings and will primarily focus on Differentiation, Effective Co-Planning, and Co- Teaching. Co-teachers will also develop an on-going student centered or coteaching centered instructional Wiki in the area of reading or math. *Day 2 (TBD late August to early September; at least 1 week following Pre-Training Walk- Through Observation ) Co-Planning and Models of Co-Teaching: Knowing When to Use Which Model Importance of Co- Planning and How to Make It Happen: Introduction to Google Docs One Teach/One Observe One Teach/One Assist Station Teaching (Full Day: ½ day & ½ day planning) *All participating schools must receive onsite on the same Staff Inclusion Team Members Building Level and/or Administrator - recommended

2 Parallel Teaching Team Teaching (Co- Teaching) Alternative Teaching *Bring your laptop and a lesson plan that you and your co-teacher need to develop or have developed to the day *Training Site needs to have wireless internet connection *Day 3 (TBD early to mid October; approx. 4 to 6 weeks from previous ) Differentiated & Specialized Instruction and Progress Monitoring Research-Based Instructional Strategies with a Reading & Math Focus Review and discuss the SBC Co- Teaching Lesson Plan Template & Differentiation: Grouping, Materials, Product, Technology, Other Progress Monitoring: Using Formative & Summative Assessment Data in the Teaching & (Full Day: ½ day & ½ day planning) *All participating schools must receive onsite on the same day Staff Inclusion Team Members Building Level and/or Administrator - recommended

3 Learning Process Introduction to Web 2.0 Tools to Differentiate Instruction and Provide Specialized Instruction to SWD Examine, Assess, & Provide Feedback on Differentiated Lesson Plans, Materials, etc. in small group GaDOE Testing Accommodations & Definition of Non- Reader *Read, Jigsaw, and discuss the article: Web Tools Applied to Teaching located on the Wiki *Bring your laptop and a lesson plan that you and your co-teacher need to develop or have developed to the *Training Site needs to have wireless internet connection ***Day 4 (TBD late October to early November) Mid-Training Co- Teaching Walk-Through Observation to be conducted by Building Level and/or District / Building Level and/or Administrato Building Level and/or Administrator using Co-Teaching Observation Checklist

DI Best Practices Inventory GaDOE & ETC r (Upon completion of all walk-through observations, will enter data into a data management system and will email results back to building administration to use for guidance and support of teachers) *Day 5 (TBD early Feb. to early March) **Day 4.5 Staff Only: Random Sampling/IEP Compliance Review prior to On- Site Training/ needs to be scheduled at Implementing Standards-Based IEPs Examine, assess, & provide feedback on a random sample of IEPs reviewed for compliance, discuss accommodations used in class, discuss results and effectiveness, etc. will provide Compliance Review Like feedback using the GaDOE Compliance Review Due Process Checklist & Other Pertinent Resources (ex. Implementation Manual, etc.) *Co-teachers are to (Full Day: Presentation from Co- Teaching Teams & NW GLRS Reports Out) *All participating schools must receive onsite on the same day Staff Inclusion Team Members Building Level and/or Administrator - recommended

5 least 2 weeks prior to onsite ) (TBD early Jan. to mid Jan.) bring data on the 3 to 5 SWD that they implemented differentiated instructional strategies on using web 2.0 tools and they will provide a 10 15 minute presentation on its impact on increasing student achievement in the area of Reading and/or Math. Teachers will also share their finished Wikispace with the class and will discuss how it assisted them with co-planning, coteaching and/or differentiating instruction with their SWD. *Training Site needs to have wireless internet connection *Training Site needs to have wireless internet connection *All participating schools must receive on-site on the same day

6 *Day 6 (TBD April to May) Post-Training Co- Teaching Walk-Through Observation Staff Only (Upon completion of all walk-through observations, will enter data into a data management system and will email results back to building administration to use for guidance and support of teachers) ***All co-teaching/inclusion team members participating in the Co-Teaching Academy will collect data on at least 3 to 5 students with disabilities served in their co-taught classroom, using a teacher selected formative and summative assessment measure. An example of a data collection form will be provided as an option to collect and report on data and will be located in the Wiki page and in Google Docs. See Attachment for specific details.
