California State University, Stanislaus Study Abroad Course and Program Planning and Approval Process Course and Program Requirements Both state support and self-support Study Abroad courses and programs at CSU Stanislaus are under the direction of the Office of International Education in Academic Affairs. Course and Program approvals are made through the Office of the Provost, in consultation with the University Risk Management Officer and the Office of International Education (OIE), followed by review by the Chancellor s Office and signature by the President. The CSU Stanislaus Study Abroad Course and Program Requirements are formed by and adhere to the most current CSU Executive Orders (Appendix G) and WASC policies. Guidelines for Program Planning The following guidelines are to be observed when planning Study Abroad courses or programs and are sourced from the WASC Principles of Good Practice in Overseas International Education Programs for non-us Nationals, http://www.wascsenior.org/findit/files/forms/goodpracticeinternational.pdf and reference WASC Standards and Criteria for Review (CFR). General 1. All off-campus and other special programs providing academic credit are integral parts of The institution and maintain the same academic standards as regular campus programs (Standard 1). 2. The faculty of the accredited institution exercises central responsibility for the academic programs, quality, and character of overseas programs. The faculty has the major role in design and implementation of the curriculum. (CFR 3.11) 3. Each overseas program shall have a core of fulltime faculty whose primary employment obligation is to teaching and research at the institution. (CFR 3.2) 4. All undergraduate programs shall include a general education segment. (CFR 2.2 and Guideline) 5. Overseas programs provide library services and hold readily available basic collections at all program sites. Interlibrary loan or contractual use arrangements may be used to supplement basic holdings, but are not used as the main source of learning resources. (CFR 3.6) Institutional Mission 6. Study Abroad courses and/or program are rooted in the CSU Stanislaus stated mission and purposes. 7. The faculty and administration of CSU Stanislaus understand the relationship of the Study Abroad courses and/or program to the institution s stated mission and purposes. Authorization 8. Study Abroad courses and/or program must receive all appropriate internal institutional approvals and external approvals where required, including University Risk Management Officer, Office of International Education, Office of the Provost, Chancellor s Office, and Office of the President. Instructional Programs 9. The Study Abroad courses and/or program must specify the academic prerequisites and the educational needs to be met by the courses and/or program. 10. The Study Abroad undergraduate courses and/or program will reflect a commitment to general education. 11. The Study Abroad courses and/or program will be taught by faculty with appropriate academic preparation and language proficiencies. 12. Colleges or Departments identify and maintain a current listing of all course or programs that include a Study Abroad component. 13. Student achievement in the Study Abroad courses and/or program will be equivalent to the standard of student achievement on the CSU Stanislaus and Stockton campuses. 14. The Study Abroad courses and/or program, where possible and appropriate, will be adapted to the culture of the host country. Rev 4/2013 Page 1 of 29

15. The Office of International Education and the International Education Advisory Committee will evaluate each Study Abroad course and programs. The evaluation data will be reviewed and appropriate changes will inform the regular planning process. Resources and Environment 16. Ideally, a pre-trip evaluation includes a pre-trip site visit. The written evaluation should be retained by the qualifying department and be available for review. The pre-trip site visit can be bypassed if the trip coordinator can demonstrate and document sufficient knowledge of the field trip site. This could be accomplished by review of online and/or published materials, or by contacting the site to discuss the visit. 17. The Study Abroad courses and/or program will ensure, as described in a Memorandum of Agreement, the use of adequate physical facilities, including classrooms, offices, libraries, and laboratories, and provides access to computer facilities where appropriate. 18. The Study Abroad courses and/or program will provide a supportive environment for student development consistent with the culture and mores of the international setting. Academic Records 19. All college-level academic credits earned in the Study Abroad courses and/or program are to be applicable to degree programs at CSU Stanislaus. Academic credit awarded must adhere to the current University, CSU, and WASC policies on Credit Hours. 20. CSU Stanislaus will maintain the official records of academic credit earned in Study Abroad courses and/or program. 21. The official transcripts of record issued by CSU Stanislaus will follow the University s practices in identifying by site or through course numbering the credits earned in the Study Abroad courses and/or program. Student Applications 22. The Study Abroad courses and/or program will be open to the broadest number of qualified students. In the case of disabled students who apply to participate, all questions should be referred to Disability Resource Services. If reasonable accommodation can be made to allow specified disabled students to participate safely in the program, Disability Resource Services will make such recommendation to the Program Director. 23. Academic and personal qualifications for student participation must be defined in the selection criteria. 24. Proficiency requirements will be clearly specified, including language proficiency requirements when appropriate, as well as defined methods of testing proficiency. 25. A student orientation must provide, at a minimum, the following: a. student emergency contact information prior to the Study Abroad trip; b. a plan to accommodate students with special needs; c. training for any equipment that may be used on the activity; d. medical requirements (e.g., immunizations, physical challenges of the program or its environment); e. procedures to follow in reporting accident, illness, or injury; f. safety/health issues relevant to the instructional site, including traffic hazards, the bad part of town, animals, water, food, avoidance of political demonstrations, use of unvented heaters, and other issues that might be unfamiliar to students; g. home stay behavior and responsibilities; h. behavior appropriate to the local culture; i. what is or is not considered sexual harassment in the local culture; of special concern are actions or dress that might be interpreted as inviting unwanted sexual overtures; j. estimated expenses beyond the basic program cost, e.g. optional side trips, admissions (to museums, concerts, etc.), special food costs, taxis, and incidentals. Students should also be made aware of the availability of ATMs and other ways to access money from home; k. essential items for students to bring, including linens or sleeping bags, if necessary; l. use of local transportation; m. how to get in touch with a program leader or responsible alternate in case of emergency, 24 hours a day; and Rev 4/2013 Page 2 of 29

Faculty n. how to use the telephone system, including purchasing a phone card, at the program site. 26. A debriefing activity will occur after return, providing for student reflections on their experiences. Evaluation activities, both through discussion and through written evaluations will occur at this time also. Faculty and student evaluations are to be submitted to the Office of International Education within 90 days of the completion of the program. 27. In order to take a group of students out of the country, faculty must be appointed in writing by the Provost. For projects involving a faculty team, each faculty member must have a separate letter of appointment. 28. Faculty from other institutions who will provide formal leadership or instruction in this program must go through the review process also. 29. Faculty are required to participate in a pre-departure discussion of procedures and liability issues to be scheduled by the Office of International Education. 30. For programs that do not use the services of a host campus that provides counseling and supervisory services, the faculty director may be required to function in that role. 31. Faculty will inform students in the in Study Abroad courses and/or program as to services that will or will not be provided. 32. An emergency is defined as the following: a. any physical or mental condition requiring hospitalization of a student or faculty member; b. any local condition (e.g., weather, political tension, epidemic) which is capable of causing harm to your students whether or not such harm has occurred (this provision will allow the university to answers questions of families, friends and the press); c. any incident involving law enforcement; and d. any incident which causes a faculty member to dismiss, or seriously consider dismissing, a student from the program. 33. Faculty are required to take a copy of campus phone numbers and a file of student emergency contact numbers to call in case of emergency. Campus emergency contact numbers: During business hours: Director, Office of International Education: 209-667-3117 After business hours: Risk Manager Amy Thomas: 209-620-6492 The principle to follow is that campus officials are a resource in an emergency and that they are to be kept fully informed of anything about which students families or the press will be concerned. When in doubt, call. A campus telephone tree will ensure that other essential campus officers are informed. 34. Faculty will take the following documents: a. file of the students emergency contact information; b. copy of campus phone numbers, including campus emergency contact numbers; c. photocopy of each student s passport; and d. copy of the signed Student Off-Campus Program Agreement for each student. Public Disclosure and Ethics 35. The Office of International Education will ensure that all media presentations about the Study Abroad courses and/or program are factual, fair, and accurate to include academic, financial, and personal commitments required from students. Recruitment materials must include a statement of nondiscrimination. 36. Personal relationships between faculty and students are to be guided by the principles outlined in the CSU Stanislaus 13/AS/11/FAC/SEC Power Disparity Policy. Travel Planning 37. The program must provide for round-trip transportation beginning and ending at the CSU Stanislaus campus. Students who choose to arrange their own transport for any segment of the trip must sign a waiver of liability form (included in Student Off-Campus Program Agreement). Provide these students with instructions as to how/where to meet the group if they are traveling independently outbound. 38. Each student must sign a Student Off-Campus Program Agreement (Appendix F). This agreement includes a general waiver of liability, guidelines for refunds, parental permission for medical treatment of students under 18, and adult students permission for medical treatment, in case a student should Rev 4/2013 Page 3 of 29

become unconscious as a result of accident or illness. This contract must also be signed by the Director of Procurement Services and filed with the Office of International Education. 39. Lodging: Faculty leaders are required to survey lodging provided by the program for safety (including fire safety) and cleanliness. If students are required to find their own lodging, such must be made clear in program information materials. 40. Itinerary: Turn in a chronological itinerary to Office of International Education prior to departure, including contact information (especially telephone numbers), at each location. The University must have a reliable way to get in touch with the faculty director in case emergency contact should be necessary. Any alteration to the itinerary while abroad, including change of hotel or telephone numbers, must be communicated to Office of International Education immediately. Risk Management: 41. Emergency services information: For each stop on the itinerary, provide the following to Office of International Education: a. Address and telephone number of the nearest full-service medical facility and how to arrange to have an injured student taken there; b. The world-wide number provided by the overseas medical insurance company to call (in case a student needs medical evacuation). c. The number and the name of a contact person at the nearest U.S. Embassy or Consulate (in case a student should disappear, die, or be arrested). 42. Signed copies of the Student Off-Campus Program Agreement (Appendix F). The faculty director should take copies when leaving the country. 43. Insurance covering medical care, air travel, evacuation, and repatriation is required. All students must register for the Foreign Travel Insurance Program (FTIP). For information, read and follow the steps on the CSU Stanislaus Risk Management website: http://www.csustan.edu/riskmanagement/internationaltravel/foreigntravelinsprogram.html 44. For trips longer than 30 days, travel health insurance must also be purchased. The cost of insurance must be included in the program fee. Information about insurance is available at the Office of International Education. Three weeks before departure, the Program Director will submit completed copies of the Foreign Travel Insurance Registration Form for each student. For trips longer than 30 days, the Program Director will also submit evidence that each student has purchased insurance. 45. A photocopy of each student s passport (or other acceptable documentation) will be on file with the Office of International Education before departure. The faculty leader must keep a copy as well. Financial Management: 46. All programs must be self-supporting. 47. Faculty-led study abroad programs must be administered in one of two ways: a. by the academic department of the faculty leader, or b. by University Extended Education (UEE). 48. The administering office performs the following: a. contractual arrangements with program providers, b. maintenance of program budget, c. completion of study abroad approval and risk management process, d. publicity and recruitment, and e. student application process. 49. Three weeks prior to department submit an authorized travel request sent to Accounts Payable/Travel. 50. Keep and turn in all receipts as required for travel reimbursement for financial accounting purposes. 51. For a semester or year-long program, make arrangements for access and distribution of financial aid. Approval Process 52. The International Education Advisory Committee will review all proposals, including a. Ensuring that appropriate resources are available for the proposed activity. b. Evaluation of the risks of the proposed activity, and the agreement and balancing the benefits of the proposed activity against the risks. c. Review of applicable host country legal requirements for the proposed activity. Rev 4/2013 Page 4 of 29

d. Coordination with the systemwide Office of Risk Management to ensure that appropriate insurance has been procured for the activities involved. 53. The International Education Advisory Committee will review all proposals, considering the health, safety and security of students, staff, and faculty as a central feature of planning and operation and must: a. Not operate in countries where there is a U.S. State Department Travel Warning unless an exception is granted by the chancellor. b. Provide a training session for staff or faculty who will lead programs abroad. This training should include emergency response training, communication from abroad, student conduct code, alcohol and drug policy, and disciplinary procedures in addition to other country or program specific information. c. Provide prospective students with information about the program, including location, duration, academic program, and total cost. d. Include a pre-departure orientation about the destination including health, safety, security, specific legal exposure or political restrictions related to their status in the country, CSU or campus policies and procedures for study abroad, and financial information. e. Require students to carry medical insurance including medical evacuation and repatriation coverage that will be valid in the host country(ies). f. Advise appropriate host country authorities of the program where appropriate or required, including locations, duration, and academic program. 54. Upon approval by the International Education Advisory Committee, the proposal will be reviewed and approved by the Provost. 55. Upon approval by the Provost, the Office of International Education will submit the proposal to the Chancellor s Office for review. 56. Upon successful review by the Chancellor s Office, the proposal will be submitted to the President for review and approval. California State University, Stanislaus Study Abroad Course Program Planning and Approval Process One (1) year prior to departure: 1. The faculty leader will submit an initial proposal to the appropriate College Dean. 2. The Dean will forward the proposal to the Director of International Education who will review the proposal and make a recommendation for initial program approval to the Provost. 3. Initial approval of the proposal authorizes the Program Director to begin program planning. Initial approval does not give authorization to take students out of the country. Program Approval Packet: The following documents are to be submitted to the Office of International Education (OIE) at the intervals noted prior to departure. Faculty leaders are strongly advised to consult with the OIE Director in the process of gathering the required documentation. Eight (8) months prior to departure: Submit the following to the Office of International Education: 4. Study Abroad Program Approval Checklist (Appendix A) (To be completed by the Office of International Education as documents are submitted) 5. Study Abroad Program Cover Sheet with approving signatures of department chair and college dean (Appendix B) 6. Faculty Application Form (Appendix C) 7. Detailed Narrative Description of the Program (Appendix D) Rev 4/2013 Page 5 of 29

Seven (7) months prior to departure: 8. Review by the campus Risk Manager 9. Review by the International Education Advisory Committee 10. Upon approval by the Risk Manager and the International Education Advisory Committee, seek approval by the Provost Six (6) months prior to departure: 11. Office of International Education submits proposal to the Chancellor s Office for review Three (3) months prior to departure: 12. Complete Study Abroad Program Authorization, including, Chancellor s Office approval, and President s signature. One (1) month prior to departure: 13. Study Abroad Program Authorization (Appendix E) 14. Provost letter(s) of faculty appointment 15. Complete travel itinerary, including in-country emergency services contact information 16. A detailed program budget 17. Outline of a plan for evaluating all aspects of the program (e.g., academic, logistical, and cultural). The evaluation data will be reviewed and will inform the regular planning process, including the decision to repeat the program, should that be requested. Three (3) weeks prior to departure: 18. Student Off-Campus Program Agreement (Appendix F) between each student and the University 19. Completed insurance application forms from all students 20. A photocopy of each student s passport (or other legal document) 21. Authorized travel request submitted to Accounts Payable/Travel Appendices: A. Study Abroad Program Approval Checklist B. Appendix F STUDENT OFF-CAMPUS PROGRAM AGREEMENT CALIFORNIA STATE UNIVERSITY, STANISLAUS This Agreement is entered into on the day of 201_ by and between the Trustees of the California State University, on behalf of California State University, Stanislaus, hereafter called "University"; and, hereafter called Student. It is the intention of the University to provide for the participation by students in the Off-Campus Program at hereinafter called Program. Student desires to participate in such Program under the terms and conditions hereinafter set forth and parties hereto agree as follows: 1. VOLUNTARY PARTICIPATION Participation in this Program is VOLUNTARY. Participation in this Program is not mandatory for completion of degree requirements. 2. SERVICES PROVIDED BY UNIVERSITY a. University shall enroll Student in the Program if Student otherwise qualifies for University enrollment. b. University shall provide appropriate academic supervision for the Program offered and shall grant academic credit if applicable. Program Director has visited the Program location and/or has sufficient knowledge of the area to be the Program Director. c. University offers the Program as set out in Attachment A (Program Details) which is hereby incorporated in this contract by reference thereto. d. University will supply foreign travel liability insurance policy including (but not limited to): primary general liability, primary/emergency medical expense, emergency medical evacuation, trip cancellation. 3. SERVICES NOT PROVIDED BY UNIVERSITY Student acknowledges and agrees that University is not furnishing and is not responsible for any matters other than as specified in Section 2. The University is not furnishing and is not responsible for and assumes no liability for the following: a. Guarantee or assurance of safety of students and/or elimination of all risks from the environment. b. The safety of personal property during participation in the program. Student agrees that the University (fully referenced above), and each and every officer, agent and employee of each of them shall not be responsible for any injury, damage, or loss to Student or Student s property which occurs from any cause beyond the control of the University or which does not occur from the sole negligence of the University. c. Monitoring and/or control of all the daily personal decisions, choices, and activities of the individual students. d. Assurances that home-country culture values and norms will apply in the host country. 4. STUDENT RESPONSIBILITIES Student agrees as follows: a. Student shall qualify for admission as a student for credit or audit in the University extension or academic year course(s) offering this Program by satisfying all requirements including payment of fees. b. Student shall pay University by the dates specified the amounts set out on Attachment A. Except as otherwise provided in this Agreement, any refund may be made at the sole discretion of University. Student shall pay sums in addition to those specified in Attachment A as may be necessary due to increases in charges by the host country, host university, accommodation changes, fluctuation in United States dollar exchange rates, or commitments made by Student while out of the country that are subsequently discharged by University, and increases in fees or other charges relating to enrollment in the CSU. Student shall pay any additional sums within 30 days notification by University. c. Student shall pay the costs outlined in Attachment A under Participation Cost Requirements. These costs include program tuition, transportation in the foreign country, insurance and other fees. d. It is understood that maximum participation in any group transportation arrange for the Program is necessary to obtain the most favorable rates, departure dates, and arrangements upon arrival at the Program location; therefore, when group transportation is arranged, Student is expected to participate unless Student informs University in writing of independent travel arrangements. e. Student understands that he/she is responsible for payment of any expenses he/she incurs during the course of the Program, including, but not limited to, medical costs or expenses. Student should keep receipts for all such payments. f. Student shall obtain and provide all materials, meet all deadlines, and otherwise comply with all participation requirements established by the Program and the University International Education Office including but not limited to passport, visa, health insurance, and foreign travel insurance. g. Student agrees to expend his/her best efforts in successfully completing the academic requirements of the courses in which the Student enrolls. h. Student agrees to submit a written health statement (Attachment B). Student understands and agrees that it is Student s responsibility to determine, in consultation with competent medical authorities, whether or not Student should participate in the Program. Student understands that visiting or living in a foreign country may involve health and other personal risks. Student further agrees to hold harmless, defend and indemnify the State of California, the Trustees of The California State University, California State University, Stanislaus and their Auxiliaries, employees, officers, directors, volunteers and agents of each of them (collectively University ) from any and all loss, damage, or liability that may be suffered or incurred by University, caused by, arising out of, or in any way connected with negligence on the part of Student, his or her agents, or employees, in the performance of this agreement, irrespective of whether such liability is also due to any negligence on the part of the State. 5. CONDUCT AND DISCIPLINE It is understood between the parties that the international implications of this agreement are such that the conduct of Student during the course of the program is of utmost importance. Student, therefore, agrees to conform to standards of conduct consistent with the maintenance of a positive reputation of The California State University and to conform to all applicable rules, regulations and policies of the California State University, Stanislaus International Education Office and any partner institution. Student understands and agrees that in the event the Program Director determines, after informing Student of the violation and discussing it with him or her, that the conduct or academic standards of Student are detrimental to the best interests of the program, the Director may terminate the participation of Student from the program. Such termination shall not diminish or otherwise affect Student's obligations to make to University any payments specified in this Agreement. University in no event shall be required to refund to Student any payment made by Student to University, but may make such refunds as are consistent with University policy. If misconduct occurs, no refund will be remitted. Such actions include, but are not limited to, the following: a. Threats or the use of physical violence b. Violating the laws of the country or partner institution c. Openly abusing the customs and mores of the community d. Damage to or destruction of property of others e. Alcohol or substance abuse f. Harassment of any kind g. Eviction from one s lodging h. Obstruction or disruption of teaching or other Program activities i. Unauthorized absence from classes and/or from the Program itinerary j. Exceed the number of unexcused absences from class allowed for the Program k. Academic misconduct (cheating, fabrication forgery, plagiarism, or facilitating academic dishonesty). Appendix F 6. LAWS, REGULATIONS, AND REFUNDS This Agreement is subject to applicable laws and regulations. If performance of this Agreement involves violation of applicable law or regulation thereby making it legally impossible to perform and such illegality is not the fault of Student, University shall refund to Student those payments made pursuant to this Agreement which are authorized to be refunded in Section 41802 of Title 5, California Administrative Code. Upon payment of said refund, all rights of University and Student under this Agreement are waived. 7. TERM This agreement is only for the term of the Program as presented on Attachment A. Upon conclusion of the Program as stated this agreement automatically terminates. 8. AGREEMENT MODIFICATION AND/OR TERMINATION a. This Agreement contains the sole and entire agreement between the University and Student and shall supersede any and all other agreements between the parties. The University and Student acknowledge and agree that any statements or representations that may have hereto been made by either of them to the other are void and of no effect and that neither of them has relied thereon in connection with his or her or its dealings with the other. b. University, in its sole discretion, may modify any terms of this Agreement as necessary before Student s departure (including but not limited to costs, dates, times, methods of travel, schedules, sites, or elements or details of any program) as long as any such modification contains a reasonably comparable substitution for the terms of the Agreement. c. University, in its sole discretion, may end this Agreement, and shall have no obligation to place Student in any institution with which University, through its Program, has contractual relations, provided that University returns to Student payments made by Student, in accordance with Section 6 above, unless the termination is in accordance to the terms of Section 5 above. d. It is expressly understood by the parties to this Agreement that the University reserves the right to terminate the Program at any time it deems the safety or welfare of students is at risk. 9. NON-DISCRIMINATION The University shall not deny the Program to any person on the basis of religion, color, ethnic group identification, sex, age, or physical or mental disability. 10. GOVERNING LAW The laws of the State of California shall govern the interpretation of this agreement. Any action brought to enforce any right or obligation under this Agreement or any action which arises out of or in connection with this Agreement shall be brought in the courts of the State of California. a. Any laws broken while in a foreign country are subject to the law in that country, which may differ significantly from those in the United States and California. I understand that each foreign country has its own laws and standard of acceptable conduct, including dress, manners, morals, politics, drug use and behavior. I recognize that behavior which violates those laws or standards could harm the University s relations with those countries and institutions therein, as well as my own health and safety. I will become informed of, and will abide by, all such laws and standards for each country to or through which I will travel during the Program. B. I also will comply with the University s rules, standards, and instructions for student behavior. I waive and release all claims against the University that arise at a time when I am not under the direct supervision of the University or that are caused by my failure to remain under such supervision or to comply with such rules, standards, and instructions. C. I acknowledge and understand that any violation of the above standards of conduct, could lead to sanctions being imposed on me that are consistent with CSU Student Discipline Policies and Procedures, including, but not limited to suspension or expulsion from the program.
