WHAT IS CERTIFICATION? Certification is a process whereby the team or association will file with the League, a complete roster of players / spirit participants for the regular season schedule. Certification will be accomplished within the official roster. All rosters must be completed on line through the new roster software. All Football rosters must be approved prior to the associations first game by the association. All football players shall meet the age and weight requirements specified on the Pop Warner age/weight schematic
REGISTRATION OF PLAYERS A candidate cannot begin practice with a team until he/she has officially registered with Pop Warner. All candidates must furnish the following in order to be registered and begin practicing: > 4 required Documents: A: Proof of Age B: Medical Examination C: Parental Consent/ Parent Contract D: Scholastic Fitness Note: All coach trainee s and student demonstrators must be certified/registered in the same manor as the participants out lined above
ID CARDS All Associations are required to have a picture ID card for each rostered Individual using the Regional ID outline PLAYERS COACHES
PROOF OF AGE 1) A certified copy of the birth certificate bearing the seal of the issuing office of the state of birth. The copy must be readable and not altered in any way 2) A current Pass Port is acceptable Note; The Hospital BC is not Acceptable!
MEDICAL EXAMINATION Each Child must have a physical by a licensed examiner dated after January 1 st of the current year( Medical Dr., Registered Physician assistant, Registered nurse practitioner etc.) The first page of the medical form MUST be filled out in full by the Parent or Guardian of the Participant, the second page filled out, signed and dated or stamped by a Licensed examiner. The National Pop Warner medical form is to be used as the physical document. If a School Physical form other then the National form is used, Page one of the National form must be attached to the School Physical Form filled out in full. The Physical form can not be altered in any way! The Medical Form can be found on the National Pop Warner Web Site

PARENT CONTRACT FORM The National parent contract form needs to be completed by either parent or Legal guardian, stating that the child has signed up to participate This form is to be filled out in full Parent Consent Form The parent consent form needs to be completed by either parent or Legal guardian, stating that the child has his or her permission to participate The Parent Consent form must be filled out in full BOTH FORMS CAN BE FOUND ON THE NATIONAL PW WEB SITE

SCHOLASTIC FITNESS Proof of Satisfactory progress in school is required. A 2.0/70% or the equivalent shall be the minimum GPA. If the GPA is below 70% or a report card can not be submitted, A National Scholastic Eligibility form is required. A progress report must then be submitted filled out in full and dated between the dates stated on the Eligibility form for the child to participate The report card or Progress Report can not be altered in any way. The Eligibility Form can be found on the National PW Web Site
COACHES CERTIFICATES All Spirit and Football Coaches must complete and pass the on line Coaches education courses and Testing program to be eligible to Coach. All Football and Spirit Volunteers that are on the roster must complete the Regional Certification course and print certificates Spirit Head Coaches and Assistant Coaches must Complete the YCADA course and print Certificates Football Head Coaches and Assistant Coaches must complete the USA Football course and print Certificates
HIGHLIGHT SPECIFICS Each item on the certification document listed below must be highlighted to show the documents were checked before the League Certification. Birth Certificate: Birth name and Birth date of the participant Medical History: Parents name and date, Doctors name/stamp and date Parent contract form: Parents or Guardian name Parent consent form: Participants name, Parents signature and date Report card: Participants name, Grade currently in and Year
STEPS IN CERTIFYING A TEAM 1) Participant Sign ups, Collect the 4 Documents required 2) Highlight the specific items specified by the Region 3) Enter the Participants information in the Roster Software 4) Once a Team is created, Print the Roster and place it in the first sleeve of the Certification book 5) Coaches Certificates are place after the roster 6) Using the Roster, start placing the participants documents in the Certification Book by Roster order and document Order as required by Nationals. 7) Coach Trainee Documents will be placed in the back of the book after the last participant on the roster.

BACK GROUND CHECKS All coaches, Board of Directors members and any other persons or volunteer workers who have repetitive access to or contact with players and/or spirit participants, must complete and submit an official Pop Warner Volunteer Application to their local Pop Warner Association or League before assuming their duties for the current season.
CPR/FIRST AID All practices must be attended by One person holding a Red Cross Community CPR and First Aid certificate or the P.R.E.P.A.R.E. course by the National Center for Sports Safety or its equivalent, if not by an EMT or volunteer physician p
UNLIMITED WAIVER Any League who will be fielding Pilot Unlimited programs MUST collect a Waiver from each player in order to participate for the current season. It will be the Leagues responsibility to collect the waivers from the associations and submit a copy from each player to Region before the first regular season game. Originals must be kept in the team certification book.
