COURSE SELECTION WORKSHEETS The worksheets on the following pages may be used by parents/guardians and students when planning next year s class schedule; however, these worksheets are not the final form which must be completed for course selection by the student, parent/guardian, and school counselor. Please follow the instructions outlined below when completing the worksheets and the final course selection forms. There is a worksheet designed for each grade level. Be certain to use the correct sheet for the grade the student will be in for the 2017 2018 school year. 1. Correctly complete the top portion of the course selection form/worksheet. Use ink. PRINT LEGIBLY. 2. Fill in the Credits earned to date section, if known. (9 th graders should attach a copy of their middle school transcripts). 3. Fill in the CLASS SELECTIONS. Make certain that grade-level requirements and prerequisites are met for each class chosen. Also, note that each grade-level worksheet/form already has the required courses a student must take preprinted on them. 4. If a student wants to petition a course, the student must complete the Petition for Course Selection Form, located in the Counseling Office. The student must discuss the Petition for Course Selection Form with the recommending teacher. The student and teacher will discuss the teacher s current recommendation and the prerequisites for the course the student wishes to petition. The parent must understand that a student is expected to stay in the petitioned class the entire school year. A schedule change will not be made for students in a petitioned class until the school year is completed. 5. Selecting ALTERNATES is critical. Be certain to list alternate classes. A chart of courses which offer elective credit has been provided (located after Course Selection sheets). Use this chart as a quick reference to determine the prerequisites and grade requirements for each course. Be certain to read the entire course description written in the COURSE OFFERINGS section of this guide before signing up for a class or an alternate.) 6. Teacher recommendations and signatures are required for the classes listed on the worksheet/forms. 7. Courses that students select this spring are the courses they will be required to attend next year. 8. Make certain to read and understand the Schedule Change Policy. 9. Not all courses listed in this catalog/guide will necessarily be offered next fall. The school Master Schedule is derived from the classes students sign up for this spring and from staffing allocations from the Clark County School District. Due to increased/decreased enrollment, staff changes, and/or course selection numbers, the administration may have to balance, drop, or add course selections during any academic year. 10. We advise that both the student and the parent/guardian sign the final course selection form. Las Vegas Academy Magnet Contract ENROLLMENT STANDARDS: The Las Vegas Academy of the Arts (LVA) becomes your student s home school once enrolled. Withdrawal from LVA is not permitted during the school year for the remainder of the time a student is enrolled in the program. Should you wish to withdraw your child, it must be at the end of the school year. If a student athlete returns to his/her zoned school or makes any other subsequent changes in enrollment, he/she would be ineligible for the remainder of that school year and for 180 school days in any sport as per NIAA (Nevada Interscholastic Activities Association). ACADEMIC STANDARDS: Students must maintain a minimum grade point average of a 2.0 (C average) and a minimum grade point average of a 2.0 (C average) in their Major program. Students who earn a D or F in coursework may be placed on probation as defined by the school. Any student with a grade of F or who fails to maintain a current 2.0 G.P.A. or has a U in citizenship will not be eligible to miss class or go on a school trip. While the school will work with families to ensure the academic success of each student, the student will be required to meet the expectations of the program. A student coming in academically deficient in coursework related to the program to which they applied will be asked to participate in the following which may include, but not be limited to: additional activities, tutoring, and/or additional coursework so that they may be successful in the program. ATTENDANCE STANDARDS: Students must maintain good attendance while attending LVA. Students that earn (5) or more unexcused absences within a semester will be placed on probation. Students accumulating excessive tardies will be placed on probation. BEHAVIOR STANDARDS: Students that receive a suspension will be placed on probation. Students that are referred for expulsion and/or are formally expelled will be dismissed from Las Vegas Academy of the Arts. Students must earn a minimum of S - Satisfactory behavior in all of their classes. Students earning an N - Needs improvement or U - Unsatisfactory will be placed on probation. PROBATION PROGRAM: Students that violate any of the guidelines pertaining to academics, attendance, behavior, or any combination of the three items will be placed on probation. Probationary Process: STEP 1 Students will be placed on probation if they are in violation of the contract. STEP 2 A probation conference will be scheduled with the parent/guardian to discuss the conditions of the probation. STEP 3 Conditions of the probation. A student may be required to partake in the following activities in order to meet the requirements of the probation which may include but not be limited to: summer school, online credit retrieval, after-school tutoring, community service, campus beautification, or other school-based requirement. Students that do not meet the obligations of their probation may be dismissed from the program at the end of the school year or prior to the following school year.
