AFROTC FORM 48 YOUR PLAN TO GRADUATE ON TIME PURPOSE: The academic plan is designed to ensure each applicant meets graduation and program completion requirements in a realistic manner. Once the academic plan is established, at a minimum all cadets must have their academic plan reevaluated and recertified by their academic advisor every fall term. You have until the end of your first academic term in the AFROTC program to turn in a completed FORM 48. Failure to turn in an academic plan may result in dismissal from AFROTC program or placement into a special student status. Changes to original Form 48 may only be made for course changes. If you change the year you are graduating or change majors then you must complete a new academic plan form 48. FILL OUT SECTION I IN INK. FILL OUT SECTION II IN PENCIL INSTRUCTIONS: Follow these instructions step by step to complete your Form 48. Your 48 is kept as a permanent record. An appropriate institutional official must review, verify, and sign block 4. Return 48 to our Detachment Cadre after completion. # 1 NAME: Last, First and middle initial # 2 ACADEMIC INSTITUTION & AFROTC DETACHMENT: Enter name of university where you will receive your degree / Det 620 #3 ACADEMIC MAJOR: Enter major field of study. If you change your major you MUST complete a new form 48. NOTE: Remember, if you are on scholarship / contract or have been submitted for a scholarship or enrollment allocation to enter POC, you must obtain approval through Detachment Commander PRIOR to changing your academic major. #4 INSTITUITIONAL OFFICIAL REVIEW: Your Form 48 must be signed and dated by dean, department chairman, faculty advisor, or other appropriate institutional official. His / her signature /date verifies that the plan, if successfully completed will lead to the prescribed degree at the time indicated. A new institutional official review is required if a new AFROTC Form 48 is completed.

SECTION II ACADEMIC PLAN: List all courses you plan to take each term by course number, title, and credit hours. Start with any transfer credits. Then list your first semester in AFRTOC program and list all terms required to complete degree and commissioning requirements from left to right on Form. List summer semester each academic year, you will leave course listed blank if not taking any courses that summer, however you must list summer. List Field Training. A. number Enter the course number as listed in the school catalog B. Title Provide the title of the course C. Enter # of hours specified in the college catalog D. leted This block will be completed by your AFROTC instructor E. This block will be completed by your AFROTC Instructor if there are any hrs that are different from the attempted and completed columns F. All Aerospace Studies courses will be listed in the term they will be taken G. You may list any elective courses as elective if you do not know the course title at the time 48 is being completed. You will add the actual course title when it becomes available. H. Leave all remaining blocks BLANK. They will be filled out each semester during your term counseling with your Aerospace Studies Instructor. Any questions don t hesitate to ask. MAINTENACE, RE-EVALUATION AND REACCOMPLISHMENT: Corrections and revisions are acceptable. Make course changes only. A new 48 is required for change of Major or Change of graduation date. All cadets must have their academic plan revaluated and recertified by their academic advisor every fall term. Recertification will occur during mid term review.
