R. E. FRENCH FAMILY EDUCATIONAL FOUNDATION SCHOLARSHIP APPLICATION The R. E. French Family Educational Foundation was created by the R. E. French Family to provide scholarships for high school graduates who plan to continue their education after high school, beginning in the semester following their high school graduation. The final decisions and selections will be made based upon the applicant s high school grades, test scores, financial need, chosen course of study, and the committee s assessment of the applicant s motivation and character. Awards will be paid after the successful applicant has been accepted for admission to a college, university, vocational technical training school, school of nursing, or licensed training program. Applicants are required to submit a recent photograph with this application, a complete OFFICIAL high school grade transcript, (including First semester of Senior year) copies of ACT and/or SAT test results, and if available, high school attendance records. The complete address of the applicant must be included on the application including street, city, state, and zip code. This application may be reproduced in any manner convenient for the applicant. HOWEVER, all questions must be answered and all essays must be completed for the application to be considered for a scholarship award. If a required enclosure is not included, then an explanation must be provided, otherwise the application will not be considered. French Family Educational Foundation Applicant s Name ESSAY Please attach to this application an essay telling the trustees - at whatever length you think appropriate - the information requested below plus anything else you believe might help convince the trustees to grant you a scholarship award. The essay should be divided into at least the three distinct parts indicated here. Part I Introduce yourself. Tell us who you are. Describe your family. Give your background. Describe who or what has been the greatest motivating factor in your life and tell what effect it has had on you. Tell us what obstacles, difficulties, circumstances have you encountered and how you dealt with them and what effect they have had on your life. Part II What goals have you set for yourself in the past? What are your goals now that you are graduating from high school? What do you plan to study in college or the other training program you have selected? Explain what you expect to achieve with that training and the effect you think it will have on your future life. What are your career plans? In what ways do you anticipate your life will affect others? Part III Please tell us your views about the following: (a) What is of the greatest importance to you in life? (b) What are the highest expectations you have for yourself? (c) What does success mean to you? (d) What would it take for you to be successful? (e) What would it take for you to be happy? REQUIREMENTS OF THE R. E. FRENCH FAMILY EDUCATIONAL FOUNDATION COMPLETING THE APPLICATION All questions must be answered and all attachments requested must be included for the student s application to be considered for a scholarship. The grade transcript must include grades for the First Semester of the student s Senior year of high school. BE SURE TO INCLUDE THE ENTIRE HOME ADDRESS OF THE STUDENT. Completed applications, with all of the requested attachments, are to be either delivered to our office or postmarked not later than March 6, 2015. The mailing address is R. E. French Family Educational Foundation, P. O. Box 203, Gridley, KS 66852. REQUIREMENTS The student is required to maintain at least a 3.0 grade point average on not less than 12-college (degree) credit hours per semester. College/Degree credit hours are hours earned and credited toward the student s degree. Classes bearing a class code beginning with 0 are termed as support, remedial, developmental, etc. These classes do not count as college/degree credit hours. If you are enrolled in any class with a class code beginning with 0, be sure you are enrolled in 12 hours that are college/degree credit hours, or 100 or above level classes for which a grade will be received. An OFFICIAL, CERTIFIED grade transcript is required at the end of each semester that the student receives the award, regardless of the grade point average earned for the semester. This applies to the semester of the student s graduation, also. A CERTIFIED copy of the enrollment/class schedule for the current semester is required. The class schedule printed from the Internet is acceptable ONLY when a signed letter from the registrar supports it. A CERTIFIED copy of the fees paid receipt showing that the fees have been paid for the current semester is required at the beginning of each semester. A signed letter from the cashier is acceptable. THE NAME of the STUDENT and the NAME of the INSTUTION the student is attending must appear visibly on each required document. ALL of the THREE required documents MUST be received by the R. E. French Family Educational Foundation before the award check is issued to the student for any semester. And, they MUST be received by the Foundation by FEBRUARY 28 th for the Spring Semester and OCTOBER 15 TH for the Fall Semester each year. The scholarship award check will not be issued to the student until all OFFICIAL, CERTIFIED documents have been received. The scholarship award check will be issued in the student s name and sent to the home address. Only upon request, by the student, will the scholarship award check be sent to the student s college address. It is your responsibility to keep us informed of where you want your scholarship award check sent.

COMMUNICATION is VERY important. Unless the student communicates with the Trustees of the Foundation, the Trustees do not know that he/she is experiencing difficulty. The difficulty might be acquiring the proper documents to receive the scholarship award check, enrolling in and finishing the required number of college/degree credit hour classes in any semester, illness, family situations; whatever might arise. Communicating the student s progress and keeping the Trustees informed of the student s graduation date will assure that the student will retain scholarship status through his/her final semester. The scholarships are CONTINUING SCHOLARSHIPS. The original amount is available to the student each semester he/she continues his/her education as a full-time student and maintains the academic standards set by the Foundation. This includes not only two and four year degrees, but the pursuit of degrees to other fields, such as medicine, law, etc provided that the student continues in the graduate program in the semester following graduation from the undergraduate program. The student must receive approval from the Trustees prior to entrance into the graduate program to retain scholarship status.
