By choosing a network provider you will save money. That s because network providers have agreed to deliver services to members for less than their usual fees. Please know that if you go to health care providers or hospitals outside of your selected network, it will result in higher costs for you, often significantly higher. Where to get the most current information. The information in this directory was compiled on: 08/27/12. To get the most up-to-date look at who is in your selected network, please use the online search tool at mymedica.com > Find Physician or Facility. The online tool is updated weekly and allows you to search for network health care providers by location, name and specialty. Know your network: Members of the UPlan Choice Regional plan, your network is Medica Choice. Members of either the UPlan Choice National or UPlan HSA plan, your network is Medica Choice with UnitedHealthcare Options PPO (also called Medica Choice Passport). Specialist: A provider whose medical specialty is not in the field of family or general practice, internal medicine, or pediatrics. The Medica Choice and Medica Choice Passport networks are always being updated. As providers are added to or drop out of the networks, this printed edition of the directory may not include the most current information. Before receiving services, check the online directory or call Customer Service to verify that your provider is still in your selected network. This health plan may not cover all your health care expenses. Please refer to your Summary of Benefits for detailed information about your benefits. Going to providers or hospitals outside of your selected network will result in higher costs for you, often significantly higher. How to use this Provider Directory Use this directory to find a provider that is conveniently located for you. Inside, providers are arranged: (1) by specialty; (2) next, by state, (3) then, by city, (4) and finally, by clinic in alphabetical order beneath each specialty. To find a provider: 1. Choose a specialty. 2. Choose the state where it would be convenient for you to see a provider. 3. Choose a city where it would be convenient for you to see a provider. 4. Choose a clinic under that specialty. Each listing includes the clinic address and phone number, and a list of the physicians who practice at that clinic. Sometimes, a clinic in the network may choose not to accept new patients. However, there are some situations where you might be able to go to that particular clinic, such as: (1) You are already a patient at the clinic, but are changing health plans; (2) You are already signed up at the clinic, but have not been seen yet; (3) You are an immediate family member of someone who is a patient at the clinic. To find out if the clinic will accept you as a new patient, contact the clinic directly. Enrolling in this health plan does not guarantee you can see a particular provider listed in this directory. If you want to make sure you can receive care from a specific provider, you should contact Medica Customer Service to ask whether that provider still participates in your selected network (refer to contact information at the bottom of this page). Tips for choosing a provider. Medica encourages you to establish a long-term relationship with a physician you can trust. A doctor who knows you and your needs can be one of your most valuable health resources. The most common types of primary care physicians practicing in clinics include: Family practice physicians. These doctors specialize in taking care of the general health needs of people of all ages, from infancy through adulthood. They receive a broad range of training in: pediatrics, internal medicine, obstetrics and gynecology, psychiatry and geriatrics. Internal medicine physicians. These doctors specialize in diagnosing and treating diseases in adults. They do not perform surgery, deliver babies or treat children. Obstetricians/Gynecologists (OB/GYN). These doctors specialize in pregnancy and childbirth and in the diseases and routine care of the reproductive system of women. Pediatricians. These doctors specialize in taking care of the general health needs of children, from birth through approximately age 16. It is important to choose the right physician to provide for the health care needs of you and your family. When choosing a doctor, you may want to consider: Training and experience. Find out whether the doctor is board-certified and whether they have any special training. You can check a doctor s qualifications by using the resources listed below. Accessibility. You want your doctor to be available when needed. Ask about office hours, how long it takes to schedule a visit and how patients after-hours needs are handled. Some providers offer extended hours. Contact your clinic for more information. The doctor s approach to preventive medicine. You want a doctor who emphasizes staying healthy. A good indicator of this is if the doctor asks about your eating and exercise habits, smoking, alcohol use and any other health risk factors.
