Faculty Development: Junior Faculty Professional Development Conference (October 29, 2010 - October 31, 2010): The annual Compact for Faculty Diversity Institute on Teaching and Mentoring (Institute), the nation s largest gathering of minority Ph.D. scholars, has successfully helped its attendees to earn their degrees and find employment at the collegiate level. The purpose of the Junior Faculty Professional Development Conference is to expose junior faculty to strategies, which will help them earn tenure. WKU FACET Preparatory Camp for Online Teaching (August 3, 2011 - August 9, 2011: The purpose of the WKU FACET Camp is to provide instruction and training on the design/conduct of online classes and on the quality matters requirements. The sessions also focused on introductory and advanced Blackboard techniques. The University of Alabama at Birmingham Lister Hill Center for Health Policy: Research Methods Workshop: (October 21, 2011). Workshop which focused on logistic regression modeling and interpretation. Workshop conducted by Edward Norton, University of Michigan. SERVICE ACTIVITIES: Professional Service: WKU CHHS: New Emerging Health Professions Ad Hoc Committee-Member (August 22 nd 2013- Present): The members of the committee are task with accomplishing the following goals: 1. To review the literature on the need for new emerging health professions such as patient navigators, patient advocates, scribes, and community paramedics; 2. Plan, organize, and conduct a program of leading experts in the health professions arena to be held at WKU (speakers might include William Frist with Aspen Healthcare in Nashville, Steve Williams at Norton s in Louisville, Connie Smith with CHC, David Gray with Baptist Health Systems, and/or others); 3. Prepare a report on the feasibility of implementing new health professions programs (courses, certificates, degrees) that link courses with competencies needed by individuals in new emerging health professions, and 4. Report to CHHS faculty/staff on January 2014 7

AUPHA Corris Boyd Scholarship Selection Committee (May 16 th 2013-Present): Hospital Corporation of America (HCA) sponsors the HCA Corris Boyd Scholars Program. This program was established in honor of a healthcare leader who dedicated his life to diversity and excellence in leadership especially among people of color and women. The HCA Corris Boyd Scholars Program offers full-tuition scholarships to two deserving students of color entering a Master s Degree program at a university of the student s choice. WKU Recertification (March 30 2012- Present): Chaired committee which was responsible for creation and the submission of the 2012 AUPHA undergraduate re-certification document. The document was successfully submitted in November, 30 2012 and re-certification meeting conducted on 06/19/2013 in Monterey, CA. The AUPHA review panel recommendations were released on 08/29/2013. Health Behavior Committee-Member (August 2011-December 2011): The committee was successfully able to identify a Health Behavior candidate (Gretchen Macy). The candidate started position in January 2012 Kelsii Gerber's Thesis: Multi-level Examination of Influenza Vaccination Disparities from the 2009 Behavioral Risk Factor Surveillance System. WKU Sociology Department (May 2011-December 2012): Assisted the student in several brain storming sessions. Reviewed the candidate s thesis and offered suggestions for improvement. Kelsii successful defended her thesis in November 2012. Research Assistant Professor Search Committee-Member (May 15, 2011 - August 1, 2011): Traveled to various universities to advertise for the Research Assistant Professor position. Met with the administrative delegation from Jackson State University. Reviewed and analyzed applications. The committee was successfully able to identify a Research Assistant Professor candidate (Akihiko Michimi ). The candidate started position in October 2011 Epidemiology Search Committee-Chair (October 2010- April 2012): Organized search committee meetings with the faculty. Responsible for the marketing of the epidemiology position. Function as the initial and most immediate contact for all epidemiology candidates. Committee was successfully able to identify an Epidemiology candidate (Helen Zhu). Candidate started position in August 2012 Community Service: Evaluation Report for the Ohio County Hospital Health Improvement Program-Chair (February 2011-Janurary 2012): Performed an evaluation of Ohio County Hospital s Health Improvement Program. The evaluation focused on the activities and process measures of the Health Improvement Program. The Health Resources and Services Administration (HRSA) required than an independent body perform the evaluation. 8
