SYLVANIA SCHOOLS CODE OF CONDUCT FOR EXTRACURRICULAR ACTIVITIES/ATHLETICS Participants are expected to conduct themselves in a manner that will not bring discredit to themselves or their school. They should not be involved in the use of alcoholic beverages, illegal drugs, or tobacco. Failure to abide by the code of conduct, or other behavior derogatory to the individual or the program during the calendar year, will lead to denial of participation. No student shall possess, conceal, use, convey to another, be in the presence of illegal drug/alcohol activity, or offer for sale, or attempt to purchase any alcoholic beverage, illegal drugs, non-prescribed drugs, prescribed drugs illegally used, look alike drugs, or any mind altering substance. A student will abide by the rules, regulations, and policies of the Sylvania schools and the laws of the community at large. Included in the prohibition are any substances represented as controlled substances, drug paraphernalia, and tobacco. A violation of the policy by any student who participates in any extracurricular activity shall be deemed in violation of the Code of Conduct, and the student shall be subject to the penalties described herein. Penalties for violation of this Code of Conduct shall apply to both athletic and extracurricular participation concurrently. This Code of Conduct must be signed before the student participates in any extracurricular tryout, practice, game or event. The student will be subject to the Code of Conduct for the entire calendar year. The Code of Conduct shall be deemed to have been violated under the following circumstances: (1) when a student is observed in violation of the Code of Conduct by school personnel or (2) when a student has been charged by any law enforcement officer with an activity prohibited by this code, or (3) by an admission of violation by the student or his parents, or (4) when a student brings discredit to himself/herself or the school (including but not limited to being at an underage party where drugs/alcohol are present, vandalism, fighting, theft ). SELF REFERRAL/PARENTAL REFERRAL Any student and parents may employ a self referral procedure, allowing the student an avenue to seek guidance, assessment/counseling and/or treatment in regard to use of alcohol, drugs and tobacco. Voluntary or parental referrals do not carry punitive consequences. A selfreferral/parental referral however is not to be used by students and parents as a method to avoid consequences once an alcohol and/or drug policy violation has occurred. All self-referrals by a student/athlete must begin by informing a staff member, coach/advisor, or Athletic Director/Administrator. The Sylvania Schools believes it is extremely important to keep the lines of communication open between parents, students, coaches, teachers, and administrators. Parents and students shall be encouraged to discuss problems in a confidential manner with school personnel without punitive consequences, regardless of a student s level of participation in extracurricular and/or athletic activities. Sylvania Schools will maintain a current list of community treatment and evaluation providers, which will be available to parents and students employing this self-referral procedure, as well as to any student found in violation of this Code of Conduct. The cost of assessment and/or treatment is the sole responsibility of the student, parents or guardians.

SYLVANIA SCHOOLS CODE OF CONDUCT FOR EXTRACURRICULAR ACTIVITIES/ATHLETICS (cont.) PENALTIES FOR VIOLATIONS OF THE CODE OF CONDUCT: ATHLETICS (a) First violation: The student shall not participate in any athletic activity for one year. Students who elect to receive professional assessment and assistance will be denied participation in 20% of the games in the current season as defined by the published schedule of games. Fractions of games shall be rounded to the nearest whole number. If the violation occurs at the end of the season, such that the full 20% penalty cannot be served, the remaining penalty shall be carried into the next season in which the student is a participant. The student will be denied the privilege to drive to school for 20% of the remaining semester. (b) Second violation: The student shall not participate in any athletic activity for one calendar year. Students who elect to receive professional assessment and assistance will be denied participation in the remainder of the season in which they are presently competing, and in addition, the sports season immediately following. [A second violation occurs any time in a participant s career after a first violation. Junior High (grades 7-8) and High School (grades 9-12) are considered separate careers.] The student will be denied the privilege to drive to school for the remainder of the school year. (c) Third violation: A third violation of the code of conduct will result in termination of athletic eligibility for the remainder of the student s career. A student who has received professional assessment and assistance may petition the Code of Conduct Committee at the conclusion of one calendar year from the date of the code violation for permission to participate in athletics the following year. (The Code of Conduct Committee will consist of the Assistant Principal and four building staff members.) The student will be denied the privilege to drive to school for the rest of the student s career in Sylvania Schools. (d) Fourth violation: A fourth violation will result in termination of athletic eligibility at Sylvania Schools for the remainder of the student s career. Participation in Practice Pursuant to the stated philosophy of prevention and rehabilitation, Sylvania Schools recognizes the benefit of continuing involvement in structured, positive activities for its students. Therefore, a student who is denied participation from athletics under this Code of Conduct for a first violation must practice with the team and attend games (out of uniform). Coaches are encouraged to allow full participation in practice. A student who is denied participation for a second violation under this Code of Conduct shall not be permitted to participate in practice, attend any post-season banquets, or receive any awards.

PENALTIES FOR VIOLATIONS OF THE CODE OF CONDUCT: CLUBS AND ORGANIZATIONS (a) First Violation: The student shall not participate in any extracurricular activity in which he or she is involved for one calendar year. The penalty will be modified for any student who elects to receive professional assessment and assistance. Following the due process hearing, the student will be denied participation in all performances or activities as defined by each club or organization for up to twenty (20) consecutive school days (including weekends). The student will also be denied the privilege to drive to school for 20 consecutive school days following the due process. (b) Second Violation: The student shall not participate in any extracurricular activity or performance for one calendar year. Any student who elects to receive professional assessment and assistance will be denied participation in the remainder of the performances or activities in which they are involved for the remainder of the quarter as well as the following quarter. (A second violation occurs any time in a participant s career after first violation. Junior High (7-8) and High School (9-12) are considered separate careers.) The student will also be denied the privilege to drive to school for the remainder of the school year. (c) Third Violation: A third violation of the Code of Conduct will result in the termination of all eligibility for performances and activities for the remainder of the student s career. Any student who has received professional assessment and assistance may petition the Code of Conduct Committee at the conclusion of one calendar year from the date of the violation for permission to participate in performances and activities for the following year. (The Code of Conduct Committee will consist of the Assistant Principal and four building staff members.) The student will be denied the privilege to drive to school for the rest of their career in Sylvania Schools. (d) Fourth Violation: A fourth violation of the Code of Conduct will result in the termination of all eligibility for performances and activities for the remainder of the student s career in Sylvania Schools. At this point, no petition for reinstatement may be made. DUE PROCESS When an Athletic Director, Principal, or Assistant Principal has been notified of a student violation of this Code of Conduct, the following shall occur: 1. The Athletic Director and/or Assistant Principal shall hold a meeting with the student to determine if a violation of the Code of Conduct has occurred. The Athletic Director/Assistant Principal shall make every reasonable attempt to contact the parents or guardians of the student before holding this meeting and parents may attend. 2. When a determination of a violation of the Code of Conduct has been made by the Athletic Director/Assistant Principal, the parents/guardians of the student shall be notified in writing. Parents/Guardians shall be considered notified if they are present at the first meeting with the Athletic Director/Assistant Principal. This does not waive the obligation of written notification by regular mail.

Any student, parent, or guardian may appeal a violation decision by notifying the Athletic Director/Assistant Principal of their intent to appeal. Such notification must be made in writing to the Athletic Director/Assistant Principal, within 5 days of the date of determination of violation of the code. The Athletic Director/Assistant Principal will convene the Code of Conduct Committee, and will establish a date for the appeal hearing. 4. After hearing the appeal, the Code of Conduct Committee will forward its recommendation to the building Principal, who shall have the authority to either accept or overrule the recommendation of the Committee. It shall be the responsibility of the building Principal to notify the student and his/her parents/guardian of the decision of the Code of Conduct Committee and the determination of the building Principal. 5. Any further appeals shall fall under the guidelines as established by the Board of Education. I understand the above policy and will abide by them and/or agree to enforce them.

Timberstone Junior High Athletic Department Parental & Student Consent Form READ ALL SECTIONS CAREFULLY BEFORE SIGNING Student Athlete Pledge As a student, I know I am a role model. I understand the spirit of fair play while playing hard. I will refrain from engaging in all types of disrespectful behavior, including inappropriate language, taunting, trash talking, and unnecessary physical contact. I know the behavior expectations of my school, my conference, and the OHSAA, and I hereby accept the responsibility and privilege of good sportsmanship and of representing this school and community as a student athlete. Parent Pledge As a parent/guardian, I acknowledge that I am a role model. I will remember that school athletics are an extension of the classroom, offering learning experiences for the students. I must show respect for all players, coaches, spectators, officials and support groups. I will participate in cheers that support, encourage and uplift the teams involved. I understand the spirit of fair play and that good sportsmanship is expected by our school, our conference and OHSAA. I hereby accept my responsibility to be a model of good sportsmanship that comes with being the parent/guardian of a student athlete. Risk of Injury I acknowledge that I have been properly advised, cautioned and warned by the administration and/or coaching staff of Sylvania Schools that by participating in interscholastic athletics, I am exposing myself to the risk of serious injury. This could include, but is not limited to sprains, fractures, ligament and/or cartilage damage which could result in temporary or permanent, partial or complete, impairment of limbs, brain damage, paralysis or even death. I do desire to participate in athletics. Code of Conduct I have read, am aware of and understand the rules and regulations that govern the conduct of participants in the Sylvania Schools Athletic Program. The receipt of this code is my first warning, and it is in effect 365 days a year. If I choose to violate these rules and regulations, I understand I will be disciplined according to policy. I will abide by the Athletic Department Code of Conduct. By signing, I acknowledge that I understand my responsibilities and have read the rules with my parent/guardian. Ohio Department of Health Concussion Information I acknowledge that I have received a copy of the concussion and head injury information sheet prepared by the Ohio Dept. of Health. I understand concussions and other head injuries have serious and possibly long-lasting effects, and that I have a responsibility to report any signs or symptoms of a concussion or head injury to coaches, administrators and the student s doctor. I understand that coaches, referees and other officials have a responsibility to protect the health of the student athlete and may prohibit the student from further participation in athletic programs until the student has been cleared to return by a physician or other appropriate health care professional. Parent Equipment Contract The athletic equipment listed below may be issued to your child/ward by the athletic department. By signing this contract, you and your child/ward agree to accept responsibility for this equipment and will return the equipment in good condition at the end of the season or pay the replacement cost as listed below. Insurance/Assumption of Risk & Release As parent/guardian, I give my permission for my child/ward to participate in athletics at Sylvania Schools. I understand the school district will make every effort to supervise my child/ward during practices and games so that s/he may participate without being injured, but acknowledge injuries including serious and permanent ones, and even death, are a possibility in interscholastic athletics. Understanding the risk involved, I consent to have my child participate in athletic department, programs and waive and forever release the Board of Education of Sylvania Schools, its officials, agents and employees from all liability for wrongful death, bodily injury or property damage that may result to my child during or as a result of interscholastic athletics. I understand the school assumes no financial responsibility in case of any injury. Timberstone Extra-Curricular Activity Eligibility Rights and Responsibilities: Students at Timberstone Junior High School are fortunate to have a wide variety of extra-curricular activities in which to participate during the school year. All members of Timberstone extra-curricular groups are expected to obey all of the school rules as well as common sense rules of sportsmanship, respect for others, and regard for the property of Sylvania Schools as well as other schools encountered. The Timberstone Student Handbook is clear as to the expectations of all Timberstone students during any activity that is related to school. These rules and regulations are enforced al all school functions: before, during and after school hours. The school and our staff members have a right to expect reasonable and responsible behavior from all of our students. Our Timberstone students can expect to be treated fairly and in a reasonable fashion by all staff in our school. Timberstone students and parents can anticipate abundant communication from extracurricular advisors and coaches. Extra-Curricular Code: Violating any of the following may result in denial from participating in an activity sponsored by Timberstone Junior High School. This denial may include a probationary time period, the remainder of a season, or the remainder of the trimester. 3. Academic probation is used when at interim a student-athlete has more than one F or percentage grades below 60%. The studentathlete remains a participant but grades are checked weekly for a three (3) week period. If the student-athlete still has more than one F or percentage grades below 60%, he/she will be denied participation until such time as grades improve. Students are granted ONE (1) probationary period during the junior high years. 4. In order to participate in an extra-curricular activity, the student must be in attendance for at least three hours and one half hours (3 ½) on the day of the activity. 5. Any student suspended from school is automatically suspended from attending and participating in all extra-curricular activities. 6. Students must remain in Good Standing in their classes and in the activity. Good Standing in Classroom or Activity Good Standing is a term for academic and social attitude displayed by a student throughout his/her day at Timberstone. Good Standing can include doing homework regularly, attending school regularly, exhibiting a positive attitude in class and being cooperative consistently in class. 1. Extra-curricular participants must follow the eligibility guidelines of the OHSAA and the policy of our Board of Education. All students must pass 75% of all classes the prior trimester to be eligible. 2. All Code of Conduct rules apply to any school related activity; before, during, and after school hours as stated in the Student Handbook given out to students on the first day of school. The same rules that you follow in school are to be followed as you leave Timberstone. Remember, you are a guest at the other school. Remain in your seat on the bus at all times. Do not stand or switch seats at any time. Do not use profanity at any time. There will be no eating allowed on the bus. Sit together as a team during the other grade s game.
