INSTRUCTIONS TO THE CANDIDATES 1. a) Candidates who have been allotted through centralized counseling by the MCC of DGHS, New Delhi for admission to MDS Degree courses in Sree Balaji Dental College and Hospital for the academic year 019-0, are advised to report and complete the admission process within the date and time specified in the Allotment Letter issued by the DGHS. The candidates should bring the following certificates/documents in Original (along with 3 sets of photocopies of the same): - 1. Allotment letter issued by the MCC of DGHS, New Delhi.. Admit Card issued by NBE. 3. Result/Rank Letter issued by NBE. 4. Aadhaar Card 5. BDS Degree Certificate/Provisional Certificate. 6. BDS Internship Certificate. 7. Permanent Dental Council Registration Certificate issued by DCI or any State Dental Council. 8. BDS Mark Statements (First Year to Final Year). 9. BDS Transfer Certificate. 10. Migration Certificate. 11. SSLC/10 th Mark Sheet. 1. HSC/ 1 th Mark sheet. 13. Community Certificate. 14. Conduct Certificate issued by the Head of Institution Last Studied. 15. Five Passport size & Two Stamp size colour photographs (same as in NEET admit card) and one Passport size photograph of the candidate s Father and Mother and also of the local guardian, if applicable.
16. PAN Card 17. Any one of the identification proofs (ID Proof as mentioned in the information Bulletin published by the National Board of Examinations. i.e., (Voter ID/ Passport). 3. i. Candidates without original certificates/documents shall not be allowed to take admission in Sree Balaji Dental College and Hospital. ii. Candidates who have deposited their original documents with any other institute/college/university and come for admission with a Bonafide certificate stating that their original certificates are deposited with institute/college/university shall not be allowed to take admission in the allotted Dental College. 4. Allotment letter given by DGHS for admission is subject to fulfilment of all eligibility criteria prescribed by the DCI and medical fitness of the candidates. Those who do not fulfil the prescribed eligibility criteria and not found medically fit will not be admitted to the course of study to which they are allotted by the DGHS. 5. Other Fees Details: Sl.
