Albany Middle School Athletic Clearance is as easy as 1,2,3 In order to be cleared for athletic participation the following steps must be taken in order to gain eligibility to tryout/practice for an AMS athletic team. Step 1: Download & print Athletic Clearance Packet from AMS website or pick up from AMS Main Office. Step 2: Complete & Sign the following forms: (Parent & student signatures required) Athletic Eligibility Checklist Form* Consent & Liability Release Form* Donation Form Step 3: Submit the completed forms to the AMS Main Office. (Paperwork will not be accepted if forms are not completely filled out) If forms are not completed & submitted to the AMS Main Office by the deadline then athletic clearance may be denied. Students cannot tryout/practice for any team until the athletic office has verified the student s scholastic eligibility (see reverse side for scholastic eligibility requirements). * Consent & Liability Release Form & Athletic Eligibility Checklist Form must be submitted for each season of sport in which your child is participating.

Scholastic Eligibility Eligibility for participation in after school sports: 1. All student athletes must achieve at least a 2.0 GPA with no F's in the previous grading period. (If the last grading period ended at the 1st quarter, then the 1st quarter grade will count toward eligibility). If a student's GPA is below a 2.0 and they have no F's then the student athlete may be placed on academic probation. 2. Student athletes are required to attend at least 4 classes of their school day in order to be eligible to practice or play in a scheduled match, game, or contest. Athletes who cut a class or have an unauthorized absence may not practice or compete. Academic Probation Students who fail to meet the minimum grade requirements will be placed on academic probation and will be expected to turn in a completed weekly progress report to the Main Office every Friday by 3:15pm in order to continue athletic participation. Their eligibility to participate in athletics will depend on their performance in all classes for the entire week. If a student fails to submit a progress report on Friday or if their progress report is not satisfactory, then they will have to sit out of practices and games the following week. The student will then submit a progress report the following week in order to regain eligibility. Behavioral Eligibility Behavioral criteria for student athletes regarding referrals: 1 referral = player meeting with athletic director & coach to decide consequence 2nd referral = benched for next match/game/contest 3rd referral = possible removal from team as to be decided by the administration and athletic director Suspension If a student athlete is suspended from school, the student athlete will not be eligible to return to the team until a meeting has taken place between the student, parent/guardian, coach and athletic director. During that meeting it will be decided whether or not the student is eligible for, or would benefit from, continued participation on the team.

Although it is not necessary for your child to have a physical examination in order to participate, it is recommended, since injuries are a possibility as a result of athletics. The California Education Code mandates that all participants in any type of interscholastic athletic event must be covered by medical insurance. The Albany Unified School District and Albany Middle School does not provide this coverage, but this coverage is available for purchase at a nominal fee. Information was sent home with your child during the first week of school and can also be obtained from the school office. To the extent permitted by the Education Code, any participant determined to be in violation of behavior standards may be removed from this sport/athletic event or activity. I understand and acknowledge that in order to participate in these activities, I and my son/daughter agree to assume liability and responsibility for any and all potential risks that may be associated with participation in sport/athletic events or activities. Albany Athletics Boosters (AAB) Fact Sheet Who we are: A group of coaches, parents, administrators and Albany community members supporting all of the athletes and athletic programs at AHS & AMS. Our focus: Fundraise to support sports programs, increase Albany Sports Spirit. When we meet: The 2nd or 3rd Monday of each month at 6pm at Albany High School in RM 106. Meeting announcements are posted in the AHS & AMS Daily Newsletter and on the AHS & AMS websites. Fundraising: Need: The minimum cost to operate the Albany Athletic Program is $300,000. The AUSD provides approximately $120,000. AAB (Albany Athletics Boosters) is responsible for raising the other $180,000. Together, these funds pay for equipment, coaches salaries, tournament fees, officials, uniforms, transportation, athletic trainer, etc. to support all of the Albany student-athletes. Per-athlete obligation: To meet this responsibility, each team is expected to raise a minimum of $200 per athlete (AHS), $120 per athlete (AMS), per season. This money goes through the AAB (Albany Athletics Boosters) account and is used to meet the basic needs of our Athletic teams. Anything raised above and beyond the $200/$120 minimum is designated back to the team and is available for extras like warm-up suits, sweatshirts, etc. Fundraising examples: Athlete provides the $200/$120 requested donation at time of sign-up for a sport Concession sales Spirit Wear sales Gift card sale Corporate Sponsor Banners Tournament: Golf, Wrestling, Volleyball, Basketball Team Example: If the Rugby team has 30 players, they are required to raise $6,000 to pay for the operating expenses (salaries, uniforms, transportation, tournament fees, officials, etc.) for our Athletic Program. Any money they raise beyond $6,000 is available for the team to use for things other than basic needs. Teams that consistently fail to meet their fundraising obligations may be dropped as a sponsored sport (simply because we can t afford it). Albany Athletics Boosters Donation Form Raising Funds! Raising Spirit! Supporting all Sports in Albany! Due to budget cuts, voluntary contributions are needed to support the athletic budget in Albany. Therefore, we are requesting a voluntary contribution from all students participating in athletics for the 2015-16 school year. 100% of these contributions will be used to cover the basic expenses of officials, equipment, transportation, uniforms, maintenance and coaching stipends. It is our sincere hope and desire that this athletic contribution causes no hardship to your family. For this reason, we will accept any and all athletic contribution amounts. For us to sustain our programs, the amount requested is $200 (AHS), $120 (AMS) per athlete, per season. Please include your taxdeductible contribution with this form and return it to the Main Office. Donations will not be processed until final rosters have been set for each team. Checks/donations will be returned to those who do not make the team. We always appreciate and recognize voluntary donations from our parents and community members. Your generosity helps support our educational goals and programs and services for students. Please understand that any donations are truly voluntary and your student's ability to participate in a school activity is never conditioned on making a donation.
