Professional Learning and Leadership Development 333 South Beaudry Avenue, 14 th Floor Los Angeles, California 90017 Telephone: (213) 241-3444 Fax: (213) 241-8920 2015-2016 EDST Stakeholder Feedback Survey The Stakeholder Feedback Survey is to be administered online to students (grades 3-12) of teachers participating in Educator Development and Support: Teachers (EDST). The survey administration window is from November 2 December 18, 2015. Students should use the following URL to access the Stakeholder Feedback Survey: https://survey.lausd.net. The following are step-by-step instructions to log into the survey site. Students have the option of taking the survey in English or Spanish. They will be asked to make their selection before beginning the log-in process.

Once students have selected their language of preference, the log in screen prompts students to select their school from a drop down menu. Students can type in the name of their school or scroll through the drop down menu. 2

Additional required fields will appear once students make their selection from the drop down menu. Required fields are identified by a red asterisk (*) icon. Students must select accurate information from the drop down menu to be able to log in successfully. 3

If a student s name is not found in the drop down menu in the Student field, select Student Not Listed, which is the first option in the drop down menu. A Student Name field becomes available for the student to enter his/her name. After entering their birthdate (month and day), students click the Log In button. 4

The system will display a warning message if a student fails to enter correct login information. Student birthdates can be confirmed by referring to the student s LAUSD ID. The first six digits of a student s LAUSD ID is their birthdate. Once logged in, the student s teacher, course and period (for secondary students) are displayed. Students verify the teacher and course by answering the required questions. Students click on the Next button to continue with the survey on the next screen. 5

If the teacher information is incorrect, students will be asked if they would like to enter their classroom information again. If they respond YES, they will be re-directed to the home log in page. If they respond NO, students will be directed to speak to their teacher or survey administrator to help find the correct classroom information. 6

Students have the option to use audio support in both the English and Spanish versions of the survey. The directions, response key, and all questions have audio buttons that will read the text to the students. It is recommended that headphones be provided to students who require audio support. Students are to respond to the survey statements by clicking on one answer button for each statement. Students can navigate to the next or previous set of statements by clicking Next or Back at the bottom of the page. Students can change answers by clicking on a new answer button. Student answers are saved once they click on Next. 7

Students may click on the Back button to review answers on previous pages. Students may edit their responses by clicking on a new answer button. Students click on the Complete & Submit button once they are ready to submit. The survey is not editable once submitted. The system will give the student a last reminder that they will not be able to log back into the survey once they submit. The student clicks Ok to continue or Cancel if not submitting. 8

Once the survey is completed and submitted, the following confirmation message displays: Thank you for taking the survey. Students are unable to log in and retake a survey once it has been completed and submitted. They will receive the following message if they try to log in again: This survey was previously submitted. Students can click the home and/or the lock icon to get back to the home page.
