Training Agreement The fllwing training agreement needs t be cmpleted by the student, a parent r guardian f the student, training facility supervisr, and teacher. Training Site/Supervisr 1. The supervisr at the training site will cmplete evaluatins as scheduled by the teacher. 2. If the student is emplyed, the student emplyment will be within the prvisins f all state and federal child labr laws and existing labr management agreements. The emplyer agrees t cmply with all WBL regulatins and if utilizing cperative educatin fllw all student-learner applicable state and federal regulatins, will prvide student trainees equal pprtunity emplyment and will nt discriminate n the basis f race, clr, natinal rigin, including limited English prficiency, sex r handicapping cnditins. 3. The term f agreement shuld be fr an agreed upn length. Either party shall have the right t terminate this agreement upn written ntice. 4. The emplyer/facility will prvide adequate staffing in the instructinal areas s that n student will be expected t perfrm duties withut supervisin. 5. The emplyer/facility will prvide an pprtunity fr the student t keep up t date with plicies and new technlgy by ntifying the schl f changes in plicies and technlgy. 6. The emplyer/facility will nt emply r cntract fr the services f students r faculty members during established schl hurs. 7. T assure that the emplyer/facility has sufficient resurces t meet its bligatins under the agreement, bth parties shall cnfer prir t the start f each semester regarding the students wh will participate in the prgram at the facility and their apprximate schedule fr the semester. 8. The emplyer and the schl will prvide instructin and experience at the training site and in the classrm. 4. The schl assumes full respnsibility fr ffering an accredited educatin prgram. 5. The instructr will plan the schedule and assist with assigning students t training sites. 6. Students and faculty will abide by existing rules and regulatins f the facility insfar as they may pertain t their activities while in the facilities building. The facility supervisr and/r crdinatr may remve students immediately that are believed t nt be cnducting their behavir in the best interest f the safety f themselves r thers. 7. The schl will require students and faculty t maintain current health recrds and immunizatins. Student Agreement: Students will 1. Cmplete designated instructinal time and curriculum while maintaining academic grades, attendance and graduatin requirements t prgress t wrk based learning experience. 2.
