Bauman, Pharm.D., FCCP, FACC Editor-in-Chief, Journal of the American College of Clinical Pharmacy As editor-in-chief of ACCP s new journal, JACCP, it is my pleasure to give you a glimpse of the plans we have for it. First, I want to make it clear how different JACCP will be from Pharmacotherapy. Although JACCP will mirror Pharmacotherapy s quality of manuscripts and reading audience, the scope of the manuscripts will differ significantly. Pharmacotherapy will continue to focus on human pharmacology and drug therapy, whereas JACCP will publish papers across the spectrum of clinical pharmacy practice. This will include original research and review articles on practice-based implementation science, innovations in practice, medication optimization and patient outcomes, economic outcomes, clinical pharmacy education and training, international clinical pharmacy practice, health care policy influencing clinical pharmacists, and care delivery models. Second, we hope that JACCP will provide a forum for discussion and debate about clinical pharmacy issues, often through editorials and letters to the editor. To be clear, JACCP will be completely dedicated to the discipline of clinical pharmacy practice and factors that affect it. It is entirely appropriate that we also publish official ACCP statements and papers, ACCP poster abstracts, and Board of Pharmacy Specialties recertification modules. These are essential components for many clinical pharmacists practicing at the top of their training. As a scientific editor for Pharmacotherapy for more than a decade, I can appreciate that it takes a team to publish a journal. I am thrilled to have three associate editors on my team: Robert Parker at the University of Tennessee, Beth Bryles Phillips at the University of Georgia, and Alan Zillich at Purdue University. Together, we are assembling an editorial board that will be committed to producing a journal that will benefit you in your clinical pharmacy practice. We anticipate introducing the editorial board to you soon. JACCP will be online only and distributed to ACCP members with no increase in membership dues. We will begin taking submissions in January 2018. We look forward to receiving your best work! Highlights from the CMM Effectiveness and Implementation Grant: A Report from the Study Team Each quarter, the ACCP Report features a column, written by the study team, to provide ACCP members with highlights and learnings from the comprehensive medication management (CMM) project. Study Updates To revisit the aims of the study, you can find them outlined here: www.accp.com/061601 In this quarterly installment, we highlight two aspects of the study related to assessing the value proposition for CMM in primary care: the patient experience with CMM and the provider experience with CMM. The Triple Aim enhancing patient experience, improving population health, and reducing costs is widely accepted as a roadmap for improving health care. Many have advocated for expansion of the Triple Aim to the Quadruple Aim to add the goal of improving the health care provider experience. Toward this end, we will be evaluating all components of the Quadruple Aim in ACCP Report 2
seeking to demonstrate the value proposition for medication optimization in improving national health care. While work continues in our effort to measure the impact of CMM on important quality metrics and cost, we also seek to assess the impact of CMM on patient and provider experience. This work is key toward building the business case for CMM. These efforts continue through spring 2018 with results available late springearly summer. We provide a brief overview on the assessment of patient and provider experience. Patient Experience with CMM Patient experience with CMM is measured according to a construct we refer to as patient responsiveness. In implementation efforts, it is important to assess the participant s/patient s responsiveness to the intervention or service. We define patient responsiveness around three core components of the patient experience: Satisfaction with care: overall satisfaction with the comprehensive medication management service. Receiving care that meets the needs of the patient: the patient s evaluation of the CMM service and how well it meets their needs. Quality of the patient-pharmacist interaction and relationship: the patient s perception of the quality of their interaction and relationship with the pharmacist. This assessment is administered as a survey to patients after they have had at least one CMM visit with a clinical pharmacist in a primary care physician office. In addition to assessing impact on care consistent with the Quadruple Aim, the patient experience is serving as an essential element of our multi-faceted approach to assessing fidelity to the CMM intervention. With fidelity of the CMM intervention defined as assuring that the CMM intervention is implemented as intended. A critical aspect of a well-implemented intervention is the patient s perspective, hence, the patient experience or patient responsiveness becomes an important construct of a fidelity assessment. Not only will we deliver learnings on patient experience with CMM across our study sites, it is our goal to validate an instrument for more widespread use across the profession in assessing patient experience with CMM. Provider Experience with CMM Primary care providers (PCPs) (i.e., physicians, nurse practitioners, and physician assistants) are key stakeholders for the effective and efficient delivery of CMM and, as referenced above, provider experience is a key component of the Quadruple Aim for improving patient care and health care quality. PCPs are necessary partners in a patient-centered, team-based care delivery model and have unique insights into the impact of CMM services on their overall experience. Thus, knowing and understanding provider experience with CMM is important as PCPs may contribute to CMM practice advancement, effectiveness, and sustainability. As part of provider experience, provider wellbeing and burnout have emerged as important areas of emphasis across primary care practice in general. Thus, we are interested in understanding the impact of CMM services on PCP wellbeing and burnout. As part of this study, we will a) define the themes described by PCPs regarding how CMM services delivered by clinical pharmacists embedded in primary care clinics affect their clinical functions and professional wellbeing and, overtime, b) develop a validated instrument to assess provider experience with CMM. Similar to patient experience, our goal is to deliver learnings on provider experience with CMM across our study sites, and, eventually, validate an instrument for more widespread use across the profession in assessing provider experience with CMM in primary care. Educational Programming Please join us in February at the Updates in Therapeutics Meeting in Jacksonville, Florida for the new Patient- Centered Team-Based Practice Forum! We have planned an exciting three-day workshop to engage participants in CMM practice advancement and improvement, and will utilize learnings from the study to assist attendees in efforts to accelerate the advancement of CMM in primary care and other ambulatory care clinics. We will also be joined by a number of guest speakers, including clinical pharmacists practicing in ambulatory care clinics throughout the US, physician providers, and other key stakeholders who will speak to payment and policy reform in the context of optimizing medication use. Importantly, as a participant in this workshop, we will actively engage you in assessing your own CMM practice and provide you with tangible tools and opportunities to work alongside others to develop action-oriented plans to accelerate advancement of CMM back at your home practices and organizations. Registration Opens January 2018 for ACCP Regional Board Certification Prep Courses ACCP s newest educational offering regional Board Certification Prep Courses will be presented April 21 22, 2018. Registration will open January 10, 2018. The courses will be held at the Western University of Health Sciences College of Pharmacy in Pomona, California, and will include Ambulatory Care and Pharmacotherapy as well as two new courses, Cardiology and Infectious Diseases. ACCP will also offer prep courses at Updates in Therapeutics 2018, February 16 18, 2018, in Jacksonville, Florida. The regional ACCP Board Certification Prep Courses in Pomona are being introduced for the convenience of attendees in California and surrounding states who may be unable to travel across the country to attend Updates in Therapeutics 2018. ACCP Clinical Research Challenge Registration Deadline Is February 7, 2018: Plan Now to Participate Critically evaluating and applying primary literature is an essential skill for clinical pharmacists. Research and scholarship contributes to improved health outcomes for patients and advances in the profession of clinical pharmacy. This innovative and unique competition offers teams of three pharmacy students the chance to participate in an online journal club and submit a clinical research proposal. Eligibility Participation in the ACCP Clinical Research Challenge is voluntary, with no entry fee required. The competition is open to pharmacy students pursuing their first professional pharmacy degree from an accredited institution or one that has been granted candidate status who are in the first 2 years of their first professional pharmacy degree program (P1 and P2 students for 4-year programs; P1 and P2 students for 3-year accelerated programs and the first 2 professional years of 0-6 or 0-7 year programs). Only one team per institution may enter the competition. Institutions with branch campuses, distance satellites, or more than one interested team are encouraged to conduct a local competition. ACCP provides a written examination that institutions may use for their local competition, if they wish. Format All eligible teams will have the opportunity to compete in round 1: Online Journal Club. Teams achieving the top 40 scores after round 1 will advance to round 2: Letter of Intent (LOI) Submission. If ties occur among the teams, the top 40 scores will be determined by the time required to complete the online journal club exam. Teams advancing to round 2 will be provided information regarding the research environment, which will include a specified budget and a timeline in which the proposed research must be completed. Eligible teams will have 2½ weeks to develop and submit a LOI online following the criteria outlined here. Washington Report: Updates in Therapeutics 2018 Patient-Centered Team-Based Practice Forum Accelerating the Adoption and Sustainability of Comprehensive Medication Management John K. McGlew Director of Government Affairs ACCP is pleased to announce an important new educational program in conjunction with the annual Updates in Therapeutics Preparatory Review and Recertification Courses: the Patient-Centered Team-Based Practice Forum Accelerating the Adoption and Sustainability of Comprehensive Medication Management. In 2018, for the first time, ACCP will offer this multifaceted, workshop-style event that immerses ambulatory care practitioners in the foundational elements of comprehensive medication management (CMM) and prepares them for initial or continued implementation of CMM services. The forum, to be held February 16 18, 2018 at the Hyatt Regency Jacksonville Riverfront Hotel in Florida, will provide attendees with a first look at the tools, strategies, and lessons learned from ACCP s Implementation and Effectiveness of CMM in Primary Care study. In addition, the event will guide participants on how to drive the adoption and expansion of CMM within their home organizations and help them establish an initial action plan designed to put implementation efforts on a path that will achieve the defined aim. The forum will be moderated by Todd Sorensen, Pharm.D., FCCP, and Mary Roth McClurg, Pharm.D., MHS, FCCP, the principal investigators for ACCP s CMM grant. The event will also feature a keynote presentation from policy leaders at the Centers for Medicare & Medicaid Services (CMS) and consistent with the broader focus on team-based care representatives from the family medicine community. ACCP Statement to the CMS Related to a New Direction for the CMS Innovation Center On September 20, 2017, CMS announced an informal Request for Information (RFI) seeking feedback on a new direction for the Center for Medicare & Medicaid Innovation ( Innovation Center ) to promote patientcentered care and test market-driven reforms that empower beneficiaries as consumers, provide price transparency, and increase choices and competition to drive quality, reduce costs, and improve outcomes. The Innovation Center was established in 2010 under the Affordable Care Act to test innovative payment and service delivery models to reduce program expenditures while preserving or enhancing the quality of care for individuals who receive Medicare, Medicaid, or Children s Health Insurance Program (CHIP) benefits. Since then, the Innovation Center has played a critical role in implementing the Quality Payment Program, created as part of the Medicare Access and CHIP Reauthorization Act of 2015 (MACRA) to pay for physicians and other providers services. The CMS RFI called for stakeholder input on eight key focus areas: Increased participation in Advanced Alternative Payment Models (APMs) Consumer-Directed Care and Market-Based Innovation Models Physician Specialty Models Prescription Drug Models Medicare Advantage (MA) Innovation Models State-Based and Local Innovation, including Medicaid-Focused Models Mental and Behavioral Health Models and Program Integrity. In response to the RFI, ACCP submitted comments to CMS calling for inclusion of CMM practices and programs that optimize the use of medications by America s seniors, as part of the process of evaluating the future of the Innovation Center and its role in promoting patient-centered care and shifting Medicare payment policy for providers toward value of care and away from volume of services. ACCP s comments highlighted the fact that although CMM services are increasingly available in MA plans as part of its efforts to drive innovation, improve quality and outcomes, and lower costs, Medicare s current benefit structure lacks a team-based and patient-centered medication management service for the vast majority of beneficiaries. The comments also drew attention to the important work currently under way as part of ACCP s CMM Effectiveness and Implementation Grant. This outreach to CMS and the announcement of ACCP s Patient-Centered Team-Based Practice Forum in Jacksonville, Florida, are just two examples of the College s ongoing efforts to explore legislative and regulatory opportunities to secure the integration of CMM services within evolving Medicare APMs to help ensure medication optimization, enhance patient safety, promote value-based rather than volume-based care to patients, and contribute to greater affordability and sustainability for the Medicare program. The award is not limited to pharmacists or ACCP members.
