Reagan Athletics 2016-17 Quick Info and Checklist Coversheet Name: Sport: ID#: 2016-17 Grade: School Attended Last Year and City: Download all forms at http://rrhs.schoolwires.net/athletics Before you can practice or try out for a team you must be cleared by the Athletic Director. Please use the checklist below. The following must be completed and submitted to Room 101C to be cleared: 1. This 2016-17 Quick Info and Checklist Coversheet. 2. Receipt showing payment of the $25.00 Athletic fee ($30.00 if cleared after team has begun practicing. This is per sport and is non-refundable. This fee is paid in the main office before submitting completed forms to Room 142. Personal checks are accepted from August 1 st to March 1 st. Cash or Money Orders only from March 1 st to July 31 st 3. Reagan High School Student Athlete Handbook Verification. (Once per school year.) 4. Waiver and Release of Liability Agreement. (Once per school year.) 5. Code of Conduct Agreement. (Once per school year.) 6. Parent & Athlete Concussion Agreement, two-sided. (Once per school year.) 7. Physical Examination card signed by a physician. (Physicals for incoming 9 th grade students must be dated after 1/1/16. Physicals taken before 4/1/16 are good only for the 2016-2017 school year. Physicals taken on or after 4/1/16 are good for the 2016-2017 and 2017-2018 school years.) 8. Authorization for Release of Medical Information. (Once per school year.) 9. Athletic Permit Card and City-Wide Rules and Regulations, two-sided. (Once per school year.) 10. Athletic Emergency Information. (Once per each sport.) 11. Copy of your family health insurance card. (Once per school year. An option to purchase student-athlete insurance is www.studentinsurance-kk.com. Print and submit the proof of coverage.) 12. Academic Eligibility: Students are allowed a maximum of one U in the previous mark period. 11 th and 12 th graders must also meet a minimum GPA of 2.0 in the previous mark period. Academically ineligible athletes may practice, but must recertify to participate in contests. 13. Any unfinished business must be cleared with the office. This includes student fees, which must be paid in order to be cleared for practice. 12 th graders must make payment towards senior fees. MPS-Department of Academic Excellence OFFICE OF INTERSCHOLASTIC ATHLETICS WAIVER AND RELEASE OF LIABILITY AGREEMENT In choosing to participate in (hereinafter referred to as the the Activity ), I fully accept and assume all risks, whether before, or during the the Activity and its related events. These include, without limitation, physical injury, mental injury, emotional distress, trauma, sickness, illness, death, contact with other participants, equipment failure, inadequate safety equipment, the effects of weather including extreme temperature or conditions. All risks are appreciated by me. I waive any and all specific notice of the existence of risks. I shall assume and pay my own medical and emergency expenses in the event of injury, illness, or other incapacity regardless of whether I authorized such expenses. I realize that this activity requires good physical health and I represent that I am in sound medical condition capable of participating in this activity without risk to others or myself. I have no medical impediment that would endanger others or me. I understand and agree that a situation may arise during the Activity which may be beyond the control of the sponsors, promoters or organizers. Knowing these facts and in consideration of acceptance of my participation in the Activity and its related events, I for myself and anyone acting on behalf, release, waive, discharge, covenant not to sue and agree to hold the Milwaukee Public Schools and any of their agents, officers, directors, representatives and employees, harmless from any and all claims, demands and actions of any and every kind I have, may have or may hereafter accrue against the released parties directly or indirectly arising out of or relating to any respect to my participating in the Activity and its related events. My waiver and release of all claims, demands, actions and liability shall include without limitation, any injury, damage or loss to my person or property which may be caused by any act or failure to act by the above-identified persons and entities. I further agree to indemnify and hold the parties released above harmless from any and all loses, damages, claims and expenses, including attorneys fees, arising from or relating in any respect to my participation in the Activity. I have read this agreement, waiver and release, and agree to and accept its terms. Sport: Code of Conduct Agreement This is a contract that will be signed by all athletes if they wish to try out for the any team at Ronald Wilson Reagan College Preparatory High School. As a member of a Ronald Wilson Reagan High School Team, I agree to the following: 1. I agree to develop as a team player that wants to be successful. 2. I pledge to have a desire to improve my self-discipline and eventually play Varsity. 3. I agree to follow all the coach s directions while at practice or in games. 4. I will work hard at practice and have a strong competitive desire no matter what situation my team is in. 5. I will be coach-able recognizing that I may disagree with but not disobey the coach s decisions regarding training or game management. 6. I will communicate with the coaches at a proper time and in a proper place. 7. I agree that all practices, games, and team meetings are mandatory. Which means that missing more than 2 unexcused will mean termination from the team. Work is not an excusable absence. All emergency absences will be communicated to the coach. 8. I agree to maintain at least a 2.00 grade point average. If any of my teachers contact the coach informing him that my grades are slipping it will be grounds for immediate suspension from the team. Asking for grade changes will not be tolerated. 9. I agree to attend all of my classes. Any unexcused absences from class will be grounds for immediate dismissal from the team. Chronic truancy will be grounds for being denied a try out. 10. I agree to inform the coach of any academic difficulty I am having which will be an acceptable excuse for missing practice. 11. I agree to refrain from using any type of smoking materials, drinking any alcoholic beverages or any use of anybody altering substances. Violation of this will be immediate termination from the team. 12. I agree to represent Ronald Wilson Reagan College Prep H.S. and in doing so take responsibility for my entire behavior, especially when visiting other schools. 13. I also agree to adhere to all W.I.A.A. as well as MPS rules and regulations for eligibility guidelines.
