Please select your courses very carefully with the assistance of your teachers and counselors. Consider your past academic achievement, future goals beyond high school, and other related interests. You will carry 7 subjects and physical education & health. You must choose 1 alternate elective in case of a scheduling conflict. Please note that for every 1 semester class that you choose you must select an additional 1 semester class. Be sure you meet the required prerequisites for your course selections and graduation. Every effort will be made to accommodate your requests, however choosing a course is not a guarantee that you will receive that course. This completed form with course numbers, titles, teacher/counselor signatures, student and parent signatures, and phone number must be returned to your English teacher by Thursday, February 20, 2015. SCHEDULING PROCEDURE Scheduling for the following school year is done each spring. All students receive a copy of the CVHS Academic Handbook and course selection sheets. Students and parents have the opportunity to discuss scheduling with school counselors during the scheduled classroom sessions and parent events. Students will be permitted to make adjustments by submitting schedule change request forms according to the timetable that accompanies the scheduling information. All schedules are final on April 10, 2015. Counselors will work with students with scheduling conflicts prior to the school year.

SCHEDULING PROCEDURE (cont.) Counselors will work with students with scheduling conflicts prior to the school year. After the deadline, changes to schedules will only be made for the following reasons: (a): A computer scheduling error has occurred. (b): You failed a course, need to repeat it, and the change did not occur. (c): You desire to add an elective (if seat is available) in place of a study hall (same period). (d): There is an error in the schedule that affects graduation requirements. No changes of any kind will be made after the first 5 days of the school year. Changes requested after the 5 day window will result in a Withdraw/Fail grade and be placed on the student s report card and transcript. This grade will be factored into a student s grade point average and class rank. Students will be required to meet with their school counselor at the time of the request to discuss and design an intervention plan to be followed for a period of time before a withdraw/fail grade is approved. A course waiver can be requested for a student who does not qualify and is not recommended for a course. The deadline to request a waiver is April 10, 2015. A meeting with a building principal to review the student s academic history will be required prior to the waiver being approved and issued. Requests for specific teachers, classes, lunch periods, or similar requests will not be approved. Please contact your School Counselor for additional supports if necessary.

Additional Information Please refer to pages 15-86 of the Academic Handbook for all course descriptions and prerequisite info. Course Selection Sheets are due complete with all signatures to student s English teacher no later than February 20, 2015. In order to schedule a course, a current teacher s signature is required in the prerequisite line. Be sure to correctly write the course number. You could schedule the wrong class if the number is not accurate. All schedules are final after April 10, 2015. No additional change requests will be permitted.

Additional Information(cont.) Students are only permitted to have an Academic Study Hall (0001) ONE semester per year. They must choose a 1 semester course to take during the opposite semester. Students are also not able to choose which semester this is during.
