Instructions for Completing Your Application NOTE: Be sure your application is computer generated or typed. Application Process Check List. 2 VERIFICATION OF FINANCIAL NEED 1. If your parents claimed you as a dependent, please submit a copy of your parents 2016 Federal Income Tax Return. If you filed independently, please submit a copy of your personal 2016 Federal Income Tax Return. 2. If you do not provide us with a Federal Income Tax Report, please submit a statement explaining the absence of a tax form and outlining your means of support. 3. Attach any supporting documentation that verifies other income substitutes. PERSONAL STATEMENT Please use two separate sheets. Your statement format is typed with one inch margins, top, bottom and sides; name and ID in the top right-hand corner; Times New Roman 11 font; double-spaced; two pages maximum. Address all of the following items: 1. Please write about what is important to you, your educational and career goals, and the ways in which they are reflected in your accomplishments. Specifically give information about yourself, your family and background, your current activities, interests, employment, educational achievements, special needs, and financial circumstances. 2. Please write about your plans for the future. Discuss your interest in your intended major, explain how your interest in the field developed and describe any related work or volunteer experience. Include information about the steps you are taking to reach your goals. 3. Please explain any excessive W s that appear on your transcript. 4. In addition to content, personal statements are evaluated on organization, creativity and grammar. UNOFFICIAL TRANSCRIPT Please submit an unofficial CCSF transcript. The transcript should be printed after the last day to drop classes. You may print these forms for yourself by accessing the CCSF WEB4. Please also submit unofficial transcripts from any other colleges you have attended. EDUCATION PLAN 1. Please submit your current Educational Plan from either Spring 2017 or Fall 2016 that reflects your declared major. If you do not have a current education plan, please make a one hour appointment with your academic counselor or program adviser to prepare your plan. 2. Submit a list of coursework taken in your major area or submit an explanation for the lack of courses completed in your declared major. ESSAY If you pass the eligibility screening, you will be required to write an essay on a topic to be determined by the Yvonne Stoupe Scholarship Committee as part of the application process. You will have two hours to complete this assignment, which is to be hand-written. No electronic devices or reference materials will be allowed. You are to write the essay in one sitting with a proctor present. The topic will be made known to you when you arrive. Bring a Blue Book for your essay. There will be no other opportunity to write the essay. If you require an accommodation, contact the Disabled Student Programs and Services (DSPS) office to make the necessary arrangements: 452-5481. Photo identification will be required. PERSONAL INTERVIEW Finalists will be interviewed by the Yvonne Stoupe Scholarship Committee, if necessary.

3 Eligibility Requirements Who: CCSF students who are enrolled in a program that leads to a bachelor s degree from an accredited baccalaureate institution. Award: One (1) renewable scholarship of $5,000 per semester, depending upon available funds. Renewability is contingent upon completion of additional 12 transferable units per semester with a 3.0 GPA or higher, satisfactory pursuit of declared educational plan, continuing enrollment at CCSF in 12 transferable units and continued financial need. Scholarship disbursed in two installments each semester, after verification of eligibility. Requirements: 1. Have completed 12 units at CCSF transferable to a baccalaureate institution with a minimum 3.0 cumulative GPA 2. Be currently enrolled at CCSF in 12 units transferable to an accredited baccalaureate institution 3. Be continuing at CCSF the following semester in 12 transferable units. 4. Plan to transfer to an accredited baccalaureate institution to earn a degree in one of the following approved majors: astronomy, biology, chemistry, earth sciences, engineering (not CIS), English, history, journalism, romance languages, mathematics, music, physics, theater arts, visual arts (art, art history, sculpture, painting, philosophy, drawing, ceramics, printmaking, photography, or film.) 4. Be a U.S. citizen or permanent resident 5. Financial need will be considered. Note: Recipients of the Yvonne Z. Stoupe Scholarship will not be eligible for General Scholarship awards.
