What would constitute success in your mind at the end of Year 1? 2. Please tell us about a time when you attempted to implement a new program or institute a change (at your current school or in previous roles) and it was NOT successful. What is your assessment of why this effort failed? What did you learn from this experience? 3. What do you think will be the biggest challenge for you and the teachers involved in the program in adopting the Achievement First approach to elementary math?

PART III: DATA TO SUBMIT Please complete the attached data template with student achievement results, by grade, in math and ELA for the 2016-17 school year and respond to the following questions: Is the data what you expected? Why or why not? What, if anything, concerns you about this data? What are the key ways you have acted on the year-end achievement data this year? If your school uses MAP, please submit the standardized MAP report for all grades tested from your most recent assessment. If your school does not use MAP, please submit the standardized report and explanation of results for the assessment used for all grades tested. Please submit a current resume for your principal and math leader. Please have a conversation with the principal's supervisor and have that person sign the attached agreement to confirm their support for your participation in the program.
