Choose a class session that works with your schedule from the options listed in this packet. Check our website to verify dates, times, prices, and availability. 2. Read and complete entire registration packet and make a copy for your records. 3. If it s prior to 7 days before the course begins, mail pages 3-5 of the registration packet and the tuition payment for the course to our office located at: Central Driving School P.O Box 231 If it s within 7 days of the class beginning, please drop off pages 3-5 of the completed registration packet and the tuition payment at our office located at 3945 Okemos Road, Suite A-5 in Okemos. Please call 517-881-0990 if you have any questions or problems. 4. You re Registered! If there is a problem, we will contact you. Plan to attend your chosen course the first day. Courses are held in room B-116 at Okemos High School unless noted differently. 5. ALL students attending Segment 2 MUST bring the following on the first day of class: - Their Level 1 License they received from the Secretary of State (making sure it has been at least 3 FULL months since they received it) - Their driving log reflecting at least 30 hours have been driven SINCE they received their Level 1 License. At least 2 of the hours must be night hours. The log may be on paper or in the Road Ready app. Save by Registering Early! Central Driving School Inc. has been educating and preparing young drivers in the mid-michigan area since 1981, and is dedicated to providing quality instruction in a safe and friendly environment. Segment 1 and Segment 2 courses are offered at Okemos High School. Our free standing office is located at 3945 Okemos Road, Suite A-5, Okemos, MI 48864. CDS is also a State of Michigan Certified Third Party Road Testing Provider. Central Driving School proudly supports all mid-michigan students. Students from ANY area high school, home school, private school, or parochial school are eligible and welcome to enroll in any of our courses, regardless of location. The Segment 2 program includes 6 hours of classroom instruction. Students must have a minimum of 30 hours of supervised driving, including 2 hours of night driving on a Level 1 license (this is not a Segment 1 certificate) that has been valid for at least 3 continuous months BY THE FIRST DAY OF THE SEGMENT 2 CLASS. ($75.00 for credit card payments). Early Registration/Payment Incentive: -$60.00 payable by cash/check/money order if signed registration forms and payment in full are submitted by ONE FULL WEEK in advance of class ($65.00 for credit card payments. Availability for walk-ins may be limited based on pre-registered class size. Courses with less than 10 registered students may be cancelled or rescheduled. (Prices subject to change; please verify current pricing on our website). Classroom instruction shall not exceed 2 hours per day. Students must be present all 3 days of the class in order to receive a certificate of completion. REQUIRED LANGUAGE NOTICE This provider is required to be certified by the Secretary of State. If you have any complaint which you cannot settle with this provider, please complete the Driver Education Complaint form found on the Department of State website; www.michigan.gov/teendriver. Completion of driver education instruction does not guarantee qualification for a driver license. TERMS 1. The parent or guardian agrees to pay the amount of $70.00 payable by cash/check/or money order by the first day of class ($75 for credit card payments). Submit signed registration forms and payment in full more than one week before class and a $10.00 cash credit will be applied to your account (making the tuition $60.00 for cash/check/money order payments, $65.00 for credit card payments). Checks or Money Orders should be made payable to: Central Driving School, and mailed to the address above. Returned checks from lending institutions will be charged a $25 NSF fee. This fee must be paid before a Certificate of Completion will be issued. 2. Central Driving School, Inc. accepts cash, personal check, money order, or most credit cards. 3. Requirements to pass the course: a. Attendance for ALL 6 hours of classroom instruction. b. Successful completion (70% or better) of the STATE TEST. c. Certificate of Completion to be issued after completion of all requirements. 4. The cost of materials and supplies for the class is included in the course fee, however students should bring something to write with to Rev. 10.03.2018 --OVER-- Page 3 of 5

class each day. 5. In case of a student s absence due to emergency the school's policy is: Students are expected to be present for ALL scheduled classroom sessions. 6. In the event of a school closing (e.g., a snow day), classes at that high school location will also be cancelled. 7. The make-up day policy is as follows: Only in case of emergency, students will be allowed to attend the day missed in the next scheduled Segment 2 class immediately thereafter. There cannot be gaps between classes to complete the class requirements. 8. Replacement certificates will be at a charge of $10. 9. No electronic devices of any kind may be used in or during class unless requested by the instructor. This includes, but is not limited to: computers, cell phones (including text messaging), or tablets. All devices must be turned off and stored away if brought into the classroom. The instructor may request all devices be collected before the lesson. 10. Any student, who comes into class more than 10 minutes late, returns to class late after the short break, or leaves the classroom before the designated break time(s); will be marked as tardy. Two (2) tardies will count as one absence. 11. All conversations between students that are not part of the classroom lecture or topic discussions will not be permitted. Those who continue to talk, interrupt, or disrespect the class or guest speakers will be marked as inappropriately or not participating in classroom lectures or discussions. Any continued interruptions will result in dismissal from the course. Parents will be notified. 12. Sleeping in class or working on other activities while in class will be considered as being absent. You must be both physically and mentally present in class. 13. The instructor retains the authority to devise a room seating chart or ask students to sit in certain seats at any time during the course. 14. Any student who is disrespectful to an instructor, guest speaker, another student, or an employee of the school may be dismissed from the class. 15. Please pick students up on time, as instructors will not stay late and will need to lock the classroom and leave at the end of the class time. REFUND POLICY If for any reason you decide to withdraw from the course before its completion, your refund will be based on the following: a. A full refund minus a $20.00 administrative fee will be issued if the student withdraws from the class prior to the start of the first classroom session. b. A partial refund will be issued based on the following: Withdrawal during or after the first class date, $25 refund Withdrawal during or after the second class date, NO refund I have read and agree to the terms and policies outlined above.
