Identify key elements of evidence-based and best practices that support success in bundled payment and CJR care delivery models. Connect with content experts currently in partnerships/developing partnerships in BPCI. Interpret and utilize publicly reported outcome and satisfaction data to enhance practice patterns. 4 Meet Today s Panel CJR Demonstration Project Your agency provides care within 1 of CMS CJR Medicare Service Areas (MSAs). The local hospital is interested in working with a limited number of post-acute providers and you have been approached to present to the large group of orthopedic surgeons. You are being asked to provide information on the value of HH as a 1 st post-acute setting of choice. What key information can position your agency as a preferred partner relationship? 3

6 Bundled Payment Initiatives Your organization is positioning itself to participate in bundled payment care initiatives (BPCI). As the director of therapy clinical services, you are asked to assess department/therapy clinical staff readiness for this transition. What are relevant factors that support success in a bundled payment model? What drivers will be important to evaluate? What characteristics are important in clinical staff?
