1 DELHI PUBLIC SCHOOL -DWARKA Provisional Admission to Class XI [ ] (For External Candidates only) Further to the interview held on Thursday, 13 April 2017 for admission to Class XI (Science/Commerce/Humanities stream) as Day Scholar/Hosteller, the following candidates are selected for admission and are required to complete the admission process as per the schedule appended below:- Admission is purely provisional and subject to scoring a minimum of grades or percentage as notified in Class X Board Examination Parents to submit an undertaking in this regard. Stream Category Day/Date/Time Science (Non-Medical) Day Scholars Monday, 17 April 2017, a.m. to noon Science/Commerce/Humanities Hostellers Tuesday 18 April 2017, a.m noon Commerce, Humanities & Medical Day Scholars Wednesday, 19 April 2017, am to12.00 noon Fee Details (Applicable for General category students only) School Fee schedule (for both Day scholars and Hostellers): - A demand draft for Rs. 58,400/- (Rupees Fifty eight thousand four hundred only) in favour of Delhi Public School. Dwarka payable at New Delhi is to be submitted at the time of admission and the balance amount will be paid in equal installments. Hostel Fee schedule (Only for Hostellers): - A separate demand draft of Rs. 1, 27,750/- to be submitted in favour of Delhi Public School Dwarka Hostel payable at New Delhi towards the hostel fee for the year The balance amount will be paid in equal installments. Documents and other essential items to be submitted (Only applicable to the candidates selected for admission) Enrolment Form for school duly filled and signed by the Parent and candidate Hostel Admission Form duly filled and signed by the Parent and candidate. A copy of Class X (SA I) mark -sheet, duly attested. Original to be produced for verification. Admission will not be granted if SA I mark sheet not submitted for verification 04 passport size photographs of the candidate and 01 each of parents. Attested copy of valid Proof of residence. Original to be produced for verification. Medical Fitness certificate duly signed by the Medical Officer. Character Certificate issued by the Head of the Institution last attended. Transfer Certificate duly countersigned by the Inspector of Schools of the respective zone. (to be submitted after the completion of admission process)

2 General Instructions Seats in the school hostel are limited and will be granted only to meritorious students. Once admission is granted to the hostel, students will not be permitted to withdraw during the course of the academic session and No refund will be made during the mid-session. The school provides the transport facilities, but there is no guarantee that a seat in the school bus will be made available if buses are full to the capacity or do not ply in the area of residence. Transport once provided will not be discontinued during the academic session. Any claim for the admission from the candidate will stand cancelled, if the candidate fails to follow the schedule mentioned without prior intimation and permission of the Principal. Registration of the candidate does not ensure admission. All decisions of the Admission Committee will be final and binding Please check the school website in the 3 rd week of April for the date for commencement of the session for class XI. The school will not entertain any request for change of stream after selection of admission. INSTRUCTIONS FOR HOSTELLERS ONLY Before a student joins the hostel, the following documents/formalities (including information on the student s week end out pass schedule) must be completed on separate forms. An authority letter from the parent giving details of two persons from different families who will act as the local guardian indicating their names, address, phone numbers, address along with photographs. A health certificate regarding medical history etc must be duly checked and signed by the Registered Medical Officer, parents and the student. If a student is allergic to any medicine or has a special medical history, please mention it in the medical certificate. School/hostel uniform requirements must be met by the parents. A list of articles of clothing and other items are mentioned in the Hostel Rules booklet. A letter of indemnity against any damage, sickness, accident etc caused in the course of normal functioning of the school. It is obligatory for the parents/local guardians to attend PTA meetings (PTA schedule is mentioned in the School Almanac) Parents/Local guardians are required to leave their telephone number and address in Delhi, for immediate contact, in case of any emergency. Both the local guardians should be present at the time of admission and reporting to the hostel. The No. of photographs as mentioned below must be submitted by the hostellers as part of the admission formalities. Student 4 copies, Local Guardian 4 copies, Father & Mother 4 copies of each

3 Kindly note that:- 1 All candidates selected for hostel accommodation will have to undergo a medical checkup by the school Doctor. 2 Date of joining of the selected hostel students will be notified through school website during 3 rd week of April It is mandatory for the local guardians to take the complete responsibility for his ward in time of sickness/emergency/week-ends/holidays and travel etc. 4 Only married couples living in a family set-up with telephone facilities will be allowed to act as local guardians. Only Hostellers - can download Hostel Admission Form, Undertaking by the Parents, undertaking by the local Guardians, Medical Emergency Information Form.
