1 WISHART STATE SCHOOL STUDENT ENROLMENT APPLICATION Student s Surname: Student s First Name:.. Enrolment Policy This enrolment application will only be accepted when accompanied by original documents (for sighting) and a copy of each document (to be retained by school), in the Student s name, listed below: Birth Certificate (an alternative to a birth certificate will be considered where it is not possible to obtain a birth certificate e.g. child born in a country without a birth registration system in which case passport and visa documents will suffice). This does not include failure to register a birth or reluctance to order a birth certificate. For international students, approved for enrolment by EQI, a passport and visa are required; AND Proof of Australian Citizenship or Australian Residency e.g. Australian Birth Certificate showing one or both parents were born in Australia; Australian or New Zealand Passport; Australian Citizenship Certificate; Passport and Australian Permanent Resident VISA; if you are unable to provide one of these listed documents please contact the school office to find out what other documents may be accepted; AND For students who reside within the school s catchment area, please see below for further documentation required at the time of application. Enrolments generally open as follows: In-Catchment Prep students: March of the year prior to commencement; In-Catchment Year 1 to 6 students: October of the year prior to commencement; Out-of-Catchment Prep to Year 6 applications: October of the year prior to commencement. Contact the school office closer to the time for exact dates. Student s Within-Catchment Parents or legal guardians who wish to enrol their child at the school as an in catchment enrolment, as per the school s Enrolment Management Plan, need to demonstrate that the student s principal place of residence is within the school s catchment area. Current proof of residency at the address on the enrolment form is to be provided by way of the following original documents (for sighting) and a copy of each document (to be retained by school), each showing the parent/legal guardian s name and the same address as the student: One primary document - current rental/lease agreement, or current rates notice, or unconditional sale agreement, and One secondary document current utility bill (e.g. electricity, gas) If the Principal is not satisfied that the documentation provided by an applicant demonstrates adequately that the address stated is the student s principal place of residence, then the Principal may request further sources of proof of residence. The Principal may also request a properly sworn Statutory Declaration from the enrolling parent or legal guardian attesting that the student s principal place of residence is the place nominated in the enrolment application. Please contact the school office if further clarification is required. Applicants should note that a false statement / assertion about the student s principal place of residence may amount to an offence and may be reported to police. The school Principal may repeal a decision to enrol a student in such circumstances. Student s Out-of-Catchment Application Applications for a student not meeting the criteria outlined above are considered an out-of-catchment enrolment application. Enrolment of students from outside the local catchment area is restricted to ensure that the total current and forecast enrolments do not exceed the Student Enrolment Capacity. This school can only enrol outof-catchment students: I. if there is sufficient spare capacity after reserving places for students who move into the catchment during the year; and II. after taking into account the school s projected future enrolment growth. 2 Out-of-catchment students applying for enrolment at this school are placed on a waiting list, and if a place becomes available will be assessed in order of receipt. Please note that enrolment applications only remain on the waiting list for the current year, except for applications for enrolment in the following year, in which case they remain on the waiting list until classes are finalised the following year (generally mid-february). Financial Delegation The parent listed as Parent/Carer 1 in the Student Family Details area of the Application for Student Enrolment Form (page 2 of 9) will receive student invoices raised in their name and will also be able to make payments from their QParents account. Please complete the parent details accordingly. Enrolment Agreement Wishart State School This enrolment agreement sets out the responsibilities of the student, parents or carers and the school staff about the education of students enrolled at Wishart State School. Responsibility of student to: attend school on every school day for the educational program in which they are enrolled, on time, ready to learn and take part in school activities act at all times with respect and show tolerance towards other students and staff work hard and comply with requests or directions from the teacher and principal abide by school rules, meet homework requirements and wear school s uniform respect the school environment. Responsibility of parents to: ensure your child attends school on every school day for the educational program in which they are enrolled attend open evenings for parents let the school know if there are any problems that may affect my child s ability to learn inform school of reason for any absence treat school staff with respect support the authority and discipline of the school enabling my child to achieve maturity, self-discipline and self-control not allow your child to bring dangerous or inappropriate items to school abide by school s instructions regarding access to school grounds before, during and after school hours advise Principal if your student is in the care of the State keep school informed of any changes to student s details, such as student s home address and phone number. 4 Religious Instruction Queensland state schools embrace a multitude of cultural, religious and non-religious beliefs. Under the Education (General Provisions) Act 2006, schools are to provide RI if approached by a faith group seeking to provide RI and students of that faith attend the school. RI informs students about the beliefs and values of a particular religion. It is delivered by volunteers of a faith group using instructional materials approved by that faith group. All RI instructors must hold a blue card and participate in compulsory Student Protection and Code of Conduct training. RI instructors are only entitled to deliver the RI program outlined below. A school staff member will be present during the delivery of RI. Participation in RI is not compulsory. Students in Years One to Six may participate in RI if a parent has given consent and indicated the religion they are to attend on the Application for Student Enrolment Form or in other written advice to the Principal. A parent, regardless of their own faith (if any), may choose to give consent for their child to participate in one of the RI programs listed below. Any child not participating in RI, or whose nominated religion is not represented within the RI program at our school, will receive other instruction by a qualified member of school staff in a separate location during the time RI is held. Other instruction will relate to subject areas that have already been covered in class. At any time, parents may provide written instruction to the Principal to change their preference for their child to participate in or be withdrawn from RI. If a parent does not update their consent by completing and returning this form, their previous written instructions to the school will continue to apply. INFORMATION & COMMUNICATION TECHNOLOGY Acceptable Use Agreement for Wishart State School Information and Communications Technology (ICT) includes for example computers, printers, scanners, digital cameras, Internet, facilities and other associated electronic and mechanical hardware and software. STUDENTS I understand that the use of Information and Communication Technology resources at Wishart State School is a privilege, which involves the acceptance of certain responsibilities. I understand and agree to the following:- I will use ICT resources only for appropriate learning tasks. I will: be considerate of other users. I will respect their privacy, and not seek access to files or messages intended for, or belonging to others, not attempt to send, transfer or create computer viruses, immediately report any damage, errors or faults with ICT resources to my teacher, immediately report accidental access to inappropriate, offensive, controversial, dangerous or illegal material, use my own logon, only print material related to my education, only use my print account. I will not use ICT resources to:- access, copy or distribute any material that is inappropriate, offensive, controversial, dangerous or illegal, download or install any software, access streaming media such as sounds, music, videos and games without teacher consent, access the control panel, system configuration, or make any changes to settings, passwords (other than my own), directly or indirectly access drives other than those allocated to me, directly or indirectly move, delete, modify any files (other than those in my personal drive), be involved with electronic or physical vandalism, use, manipulate or transmit photos of members of Wishart State School, student or staff without their (and their teachers) prior approval, reveal my full name, personal address, phone numbers, or those of other students or staff, in any electronic communications. 19 Consequences for misuse of Wishart State School Information, Communication and Technology resources 1. Students found going to inappropriate web sites will (with or without downloading) a) The first time have their Internet access denied for 2 weeks. b) The second time have their Internet access denied for 6 weeks. c) The third time have their Internet access denied for one semester. d) If the offence involves e.g. sexist, racist or violent material then the consequence will be at the discretion of the Administration. 2. Students found with inappropriate files on their drives will: a) The first time have their Internet access denied for 2 weeks. b) The second time have their Internet access denied for 6 weeks. c) The third time have their computer access denied for one semester. d) If the offence involves e.g. sexist, racist or violent material then the consequence will be at the discretion of the Administration. 3. Students found inappropriately using other students logins, and/or Print Accounts will: a) The first time have their computer access denied for 2 weeks. b) The second time have their computer access denied for 6 weeks. c) The third time have their computer access denied for one semester. Note: If the offence involves the offending student using another student s print allocation, the student will be responsible for repaying the costs associated with the Print Account. If the offender bullied another student to gain access to passwords then the School Behaviour Policy will also apply. 4. Students found attempting to access unauthorised sections of the computer systems (this includes the use of hacking and logging programs) will: a) The first time suspension (to be determined by Administration) and access to computer system denied for 3 weeks. b) The second time and subsequent times suspension (to be determined by Administration) and computer access denied for a semester c) Computer hacking is a criminal offence in Queensland and maybe reported as such. 5. Students found using inappropriately (this includes using Spam, using for social reasons during class time, using inappropriate language in s such as swearing ) will: a) The first time have their Internet access denied for 2 weeks. b) The second time have their Internet access denied for 6 weeks. c) The third time have their Internet access denied for one semester. d) If the offence involves serious allegations, offensive or threats, the consequence will be at the discretion of the Administration. 6. Students found tampering with or damaging computer equipment will: For Minor Damage - a) The first time complete a lunchtime detention and replace damaged item. b) The second time complete 3 lunchtime detentions and replace the item. c) The third time as determined by the Administration. For Major Damage a) The first time suspension (to be determined by Administration) and payment for replacement equipment. b) The second and subsequent times suspension or exclusion as determined by Administration. c) Payment for repairs and or replacement equipment. 20 Consent for Voluntary Student Participation in Chaplaincy Program at Wishart State School Student s Name (in full): 1. This school community provides a chaplaincy program endorsed by the school s Parents and Citizens Association and available on a voluntary basis to all students. The chaplain is involved in a range of activities which happen at this school which are free of religious, spiritual and/or ethical content. These activities, which include the following, are available to all students on a voluntary basis unless a parent or guardian requests in writing that this is not to occur for their child/ren: part of playground duty roster; supervise lunch-time clubs/games with children (no religious content); attend camps, excursions; support drug education programs within school; support our social skills program; be part of our student welfare team when required; be available for student or family trauma support if required.
