2 Bullying Consequence Rubric The below consequences will be enforced after an investigation into each bullying incident or the bullying incident was directly observed by a school staff member. The incident will be documented into the school computer. The below stated consequences may be applied to any or all students who is actively taking part in incident (i.e. leader, henchmen, followers, etc ). 3 Bullying Behavior Referral Student name Date: Teacher This form is part of the parent contact communication we are using when a student has bullied someone at School Name. The categories of bullying behavior are listed below: VERBAL: teasing, making fun of, calling mean names, telling lies, spreading false rumors, gossiping or threatening. EXCLUSION: leaving someone out of a group or activity on purpose. PHYSICAL: hitting, kicking, pushing, shoving, locking in room, forcing someone to do what they don t want to do, making gestures with your hands or face. WRITTEN: sending mean notes, e mails, internet messages, Zanga, Facebook, or other electronic messages meant to hurt someone s feelings. PROPERTY: taking, hiding, damaging or breaking anything that belongs to s someone. OTHER: Teacher or Parent comments: Sign and date : student parent teacher/staff

4 DEFINITIONS: Teasing name-calling, insulting, or other behavior that would hurt others feelings or make them feel bad about themselves Exclusion starting rumors, telling others not to be friends with someone, or other actions that would cause someone to be without friends Hitting pushing, slapping, grabbing, flicking, poking, pinching, tripping, or other violations of personal space Severe hitting punching, kicking, and similar behavior that could result in injury to others Threat of serious violence threats of using a weapon, or other conduct which should be immediately reported to police Harassment racial, ethnic, or sexual name-calling or other severe harassment
