Children and families are referred to the Family Intervention Support Team by preschool classroom teachers and early childhood screening results. Children who do not qualify for special education services may also be referred to the team. The problem solving process is a strategy available to the team. The Family Intervention Support Team provides support to children and families by offering school-based academic and behavioral interventions, home-based behavior consultation, home visits by a public health nurse, and/or by providing scholarship or transportation assistance for preschool. Children who continue to demonstrate concerns may be referred to the Student Support Team for a special education evaluation.

2 Overview of the Student Support Team procedures: The team meets weekly. Students are referred to the Early Childhood Student Support Team through doctor referrals, parent requests for evaluation, the Help Me Grow program, early childhood screening results, and preschool teacher referral. The team discusses the referrals and determines whether evaluation for special education services is appropriate. The evaluation process is then monitored and discussed at Student Support Team meetings. Students are referred to the team because they have needed continued support from a, parents have contacted the school regarding an ongoing custody issue, and/or the student s attendance records indicate a high number of absences. Students who have on-going concerns that impact their academic and/or behavioral functioning at school may be referred to the Problem Solving Team. Overview of the Problem Solving Team procedures: The team meets three times a month and implements the problem solving process for addressing student needs. Students are referred to the Problem Solving Team via a review of school-wide assessment data, teacher referrals, or by members of the Coordination Team. Student needs are determined by a review of a variety of data sources. The team plans academic and behavioral supports and interventions for at-risk students. Student progress in interventions is monitored using curriculum-based measurement for academic concerns. Students whose academic and behavioral data indicate that adequate progress is not being made are often referred to the Student Support Team for a special education evaluation or may be recommended for a 504 Accommodation Plan, if appropriate.

The Core Team meets weekly to review teacher referrals and to implement the problem solving process in order to address student academic and behavior needs. Students will be referred to the Problem Solving Team by teacher referral and the review of school-wide assessment and behavior data. The function of the team will be to review student data, identify the primary problem, and plan academic and behavior interventions accordingly. This team will also review student progress toward achieving their goals. If students are not making adequate progress despite intervention, additional interventions will be implemented or they may be referred to the Student Support Team for a special education evaluation. The Implementation Teams meet monthly to review progress of students in Tier II Intervention classes. In addition, these teams are working to build the capacity for Tier III interventions within the building. Overview of the Student Services Team (SST) procedures: This team meets weekly. Students are referred to the Student Services Team through school team referral or a parent request for an evaluation. The team discusses the referral, reviews student data, creates an evaluation plan that addresses all identified concerns, and executes the evaluation, including the determination of whether or not special education eligibility criteria has been met. In addition, this team reviews services and needs for students currently receiving special education services, which includes conducting three-year reevaluations and monitoring due process compliance.

PST meets weekly on Tuesday mornings from 8:00 to 9:30 am. Students are brought forward to the team through the Teacher Request for Assistance Form and referral to guidance counselors. Problem Identification, Problem Analysis, Plan Development, Plan Implementation, Plan Evaluation procedures are followed per Problem Solving Team training Overview of the Student Services Team (SST) procedures: Team meets Thursday mornings from 7:15 to 7:45 Every other month, there is a meeting with Nicole Woodward to discuss department and district issues Students who are in the process of an initial evaluation, or have already qualified for Special Education services are discussed at the meetings Due process topics are discussed at meetings. New procedures and information from I-Team are discussed at the meetings. New students are discussed and case managers are assigned. Students with behavior or academic concerns are discussed at the meetings
