2 McKinney-Vento Homeless Assistance Program Enrollment Forms The Mckinney-Vento Homeless Assistance Act requires all LEAs to Include the Louisiana Student Residency Questionnaire Form in all enrollment packets. IDENTIFYING/ENROLLING HOMELESS STUDENTS: Louisiana Student Residency Questionnaire Form One separate form for each student of homeless family Complete all sections of the form Signatures are required by the parent/guardian and a school contact person List all underage siblings at home on the form also A Spanish version of the form is enclosed if needed Mckinney-Vento Confidential Referral Form One separate form for each student of the homeless family Complete all sections of the form that apply Signature of school personnel required If school fees are needed indicate amount that is due If uniforms are needed indicate sizes Homeless Underage Siblings Form Complete all sections of the form on each underage sibling of homeless family FAX completed forms to April Theriot, Homeless Liaison at Homeless Tracking System Homeless student will now be tracked in a state system Service activity logs must be completed monthly on each homeless student Service activity logs mush be faxed at the end of the month to April Nelson at HOMELESS POSTERS must be displayed where it is visible to parents/guardians If you have any questions regarding the homeless forms or homeless eligibility, please contact April extension Thank you for your assistance in ensuring that homeless students are identified to address their needs. IMPORTANT: Students must be enrolled immediately regardless of whether they have uniforms or appropriate documentation (i.e. birth certificate, immunization record, social security card.)
3 ST. MARTIN PARISH HOMELESS POLICY The mission of the St. Martin Parish School District Homeless program is to provide homeless children and youth equal access to its educational programs and an opportunity t o meet the same challenging S t a t e of Louisiana and District academic standards. Students will be protected from discrimination a n d will not be segregated on the bas.is of their homelessness. Homeless students are identified during enrollment. The students shall be immediately enrolled even if the student lacks records normally r e q u i r e d for enrollment. School secretaries and/or guidance counselors submit referral forms to the homeless liaison, who, in turn, sends forms to the homeless advocate. The homeless advocate completes a home visit to assess living arrangements a n d assist the parent/guardian i n completing t h e homeless forms to ensure needs are being identified. Once the residency questionnaire form is completed, i t along with the referral form, is sent to the homeless liaison's office. The homeless liaison reviews and approves the forms. The student is then identified as homeless on the parish mainframe b y entering t h e student's primary nighttime residence. A copy of the referral form is sent to the School Food Service Supervisor t o keep on file and to the school to be placed in the student's cumulative r e c o r d. Homeless students automatically q u a l i f y for free lunch. The Title I Homeless Program provides school fees, supplies and uniforms for each registered homeless child. Referral forms are sent to the School Based Health Center coordinator. A social worker shall contact the homeless family to see if any assistance i s needed. The school based h e a l t h centers offer preventive and primary health services that address the physical, emotional a n d educational needs of students. For students requesting to stay in the school of origin, the homeless liaison contacts the transportation supervisor who then makes arrangements for transportation to school of origin when feasible. Transportation s h a l l be provided in accordance w i t h District transportation guidelines. If a family chooses to stay in the school of origin after permanent housing has been found, transportation (when possible) will be provided for the duration of that school year, if requested by the family and if feasible. Each homeless child/youth s h a l l be provided services comparable to other students in the school. Student data is used to determine t h e need for further academic services. The District uses Dibels, RTl and tutors paid with Title I. The district also provides access to Virtual School to high school students. Student folders are kept in the Homeless Liaison s o f f i c e with homeless forms and documentation that the students needs were met. Grievances may be filed, by the student's parent or guardian, if they feel that student rights have not been served appropriately. Grievances shall b e processed in a timely manner utilizing the following steps: (1) A grievance is filed with school. Administrator and Supervisor attempt to r e s o l v e grievance. (2) If grievant i s not satisfied, may appeal t o the Superintendent o r designee. Superintendent shall c o n d u c t a full hearing on grievance. A written recommendation is provided to grievant. (4) If unsatisfied, grievant may appeal to St. Martin Parish School Board. (5) If unsatisfied, grievant may appeal to the State Department of Education. It is the goal of the District to provide the best education possible to every student and to blend homeless students into the general school population without stigma of their home situation.
