9 Registerig Your Child for Middle School CURRET ARLIGTO STUDETS SELECTIO OF COURSES/ACADEMIC PLAIG To ease the trasitio from elemetary to middle school, middle school couselors visit the elemetary schools to meet with fifth grade studets i late February ad early March. Paret iformatio sessios are also held to assist parets with academic plaig ad uderstadig middle school courses. The Middle School Program of Studies (POS) will be available i February at the elemetary ad middle schools ad o the Parets sectio of the website at The POS gives geeral ad specific iformatio about curricular offerigs ad services at the middle school level. Each fifth grader will work with the classroom teacher, school couselor ad parets to select courses for sixth grade. A Course Request Form (CRF) will be completed, approved by the paret ad set to the middle school. Studets develop a 4 6 Year Academic Pla at the middle school level that follows them to high school. Each studet s pla is idividualized ad outlies a pathway to meet rigorous requiremets to prepare the studet for high school. The pla is updated each year by the studet durig a meetig with the school couselor. Parets are strogly ecouraged to participate. Medical iformatio that icludes immuizatio records ad documetatio of a tuberculosis screeig doe withi the 12 moths prior to etry (See Health Requiremets) For additioal iformatio ad all forms eeded for registratio, go to the Registratio sectio of the APS website, HEALTH REQUIREMETS The studet s medical iformatio (immuizatios ad physical exam, if available) must be submitted to the school. A physical examiatio withi the 12 moths prior to school etry is recommeded, but ot required uless the studet is participatig i middle school sports. The immuizatio record is required ad should list the moth, day ad year of each immuizatio. Documetatio of tuberculosis (TB) screeig doe withi the 12 moths prior to school etry is required, except whe trasferrig from aother Virgiia public school system where TB screeig was previously completed. TB screeig ad/or testig must be completed i the Uited States or at a U.S. istallatio ad be verified ad siged by a licesed medical professioal (physicia, urse practitioer, registered urse or physicia s assistat). The screeig must iclude a review of symptoms, a risk assessmet, ad if idicated, the results of either a TB ski test or a TB blood test (commoly referred to as a IGRA ). REQUIRED IMMUIZATIOS Every studet eterig grade six through grade 12 is required by the state of Virgiia to receive a booster dose of Tdap (tetaus, diphtheria, acellular pertussis) vaccie. Three doses of Huma Papilloma Virus (HPV) vaccie are required for girls ages with oe dose prior to sixth grade etry. For HPV, parets may opt out of the HPV requiremet by simply ot gettig the vaccie. o letter or certificatio of opt out status is required. RECOMMEDED IMMUIZATIOS Talk with your health care provider or public health urse about recommeded vaccies for pretees ad adolescets. The Ceter for Disease Cotrol (CDC) strogly recommeds a meigococcal (meigitis) vacciatio for all adolescets betwee the ages of 11 ad 13 ad iflueza vaccie. Check-I is a daily after-school program which provides supervised, asset-buildig activities for erolled sixth-, seveth- ad eighth-grade studets. This program operates o all regularly-scheduled school days at all five middle schools from school dismissal to 6 p.m. Should schools close earlier tha the regularly-scheduled time, Check-I will close at 4 p.m. Check-I provides studets a positive eviromet i which to study, relax or participate i a variety of activities. The studets ca also participate i ay of the school s regularly-scheduled after-school activities, such as itramural programs ad clubs.
