3 EXPLANATION OF IMMUNIZATION AND EXAM REQUIREMENTS Key Points: Physical: Completed within one year prior to initial entry into a Kentucky Public School and/or sixth grade reported on Kentucky form or meets KY physical requirement components. Valid physicals received on students attending public Head Start in Kentucky do count towards meeting this requirement and do not need to be repeated for entry up to fifth grade. All students must have an additional physical completed at sixth-grade entry or at initial enrollment to KY schools after sixth grade. Physical must be obtained within 30 days of starting school. Eye Exams: Must be completed by an eye doctor and placed on the Kentucky Eye Exam form. Eye exams from an eye doctor are required one time between the ages of 3 to 6 for initial entry to school. Vision exams done while attending Head Start do meet this requirement and do not need to be repeated for entry into kindergarten. Dental Exams/Screening: Must be reported on the Kentucky Dental Screen form and completed during the same calendar year the student starts kindergarten. Screenings obtain while in Head Start do not meet this requirement unless completed during same calendar year student starts kindergarten. certificate that shows shot dates, type of certificate, and expiration date. Out-of- State immunization certificates can be accepted if the shots listed meet Kentucky requirements and has shots dates, expiration date, and type of certificate. A Kentucky immunization certificate will be required whenever a student is needing to obtain additional shots. Religious and Medical exemptions must be submitted on the new immunization certificate revised 1/2017. Commonwealth of KY declination on religious grounds form is to be completed with the provider and notarized. Only the certificate is required to be turned in to the school. This section may be revised in the future. KYIR (Kentucky Immunization Registry) is a state wide system that medical providers and health departments can use to access immunization history on someone. Parents/guardians shall be asked to sign an authorization form to allow the school to enter shot information for the student and issue a Kentucky immunization certificate if one is needed. If the child is not up-to-date,

4 a two-week temporary certificate can be issued to allow time for student to receive the immunizations. Any provider that has access to KYIR will be able to update shot information. The school system can enter any missing shot history they have in their records and issue a certificate if the provider did not do so. A declination for the school to enter information into KYIR will possibly mean more work for the parent/guardian and medical provider. The parent/guardian will become solely responsible for providing and obtaining immunization certificates. Some providers require office visits and charge additional fees to issue immunization certificates.

5 IMMUNIZATION AND EXAM REQUIREMENTS BY AGE/GRADE New immunization law now requires: Two Hepatitis A immunizations in addition to all other previously required shots for all students Age 16 and up must obtain a Meningitis booster Preschool up to Age 5: Physical within 1 year prior to initial entry into Preschool and yearly afterwards Eye Exams on KY form - According to Head Start Guidelines: If prescribed glasses, must have yearly Eye Exam. If not prescribed glasses, must have yearly eye screening. certificate that includes the new immunization requirements in addition to all previously required shots for age. Dental Exams/Screening as per Head Start Guidelines These will not meet the requirement for entry into kindergarten unless done the same calendar year student is to start kindergarten. Kindergarten to age 10: Physical within one year prior to initial entry into a Kentucky Public School or within 30 days of starting school. Children who are age 7 or older do not need this exam. Age 11 and Up Physical: Students entering sixth grade or entering KY schools after sixth grade must have a physical completed within one year of entry. certificate that includes the new immunization requirements in addition to all previously required shots for age. Sixth grade shots are now required for age 11 regardless of the grade student is in. (Tdap and Meningitis) New immunization law now requires two Hepatitis A on all students. Students age 16 and older must obtain meningitis booster regardless of whether they did or did not get one between the ages of 11 and 15.

7 Dear Parent/Guardian, New immunization laws have been passed by the State for school age children. These new requirements were passed in June 2017 and are set to be enforced starting the school year. The law will require almost every student in the state of Kentucky to obtain new shots and immunization certificates. Here are the key points: Upon enrollment, all students must present a current immunization certificate or have one on file at the school that shows the new immunization requirements. Regardless of previous expiration date on immunization certificates, students must have the new required shots listed on the certificate for it to be valid. NEW Hepatitis A: All students, regardless of age, are required to have 2 Hepatitis A vaccinations given at least 6 months apart. NEW Meningitis Booster: All students age 16 and up are to have a Meningitis shot regardless if they got one between ages 11 and 15. Sixth grade shots are now called Age 11 shots. This means regardless of the grade your student is in, they must have the Tdap and Meningitis immunizations when they turn age 11. We encourage you to contact your health care provider about the new immunizations now! It will take 6 months to complete one of the requirements. Take a copy of any immunization records your student currently has with you to the visit. Students are to have a current immunization certificate that shows the new shots when they enroll next fall.
