2 Minooka Community High School Thank you very much for your interest in our camps at Minooka Community High School. We take a lot of pride in each of our camps. Our coaching staff works hard to assure that your camp experience is the very best that it can be. Minooka Community High School camps are for children Kindergarten through 12th grades. We offer various camps to fit the needs and interests of players of all ages and skill levels. Please take the time to browse through this brochure to find out more about each of our camp offerings. We think that each and every one of you will find that we offer a camp that is just right for you. Our camps will be conducted at our high schools, the locations are listed with the camp information. The addresses of our school locations are: Central Campus 301 S. Wabena Ave. Minooka, IL South Campus, Door 20 Channahon, IL Three easy ways to register and pay for camp 1. You may register and pay on-line using our secure registration system. We accept credit/debit MasterCard, Visa and Discover cards located at the Web Store at 2. You may mail your completed registration form in with payment to: MCHS, Athletics 301 S. Wabena Ave. Minooka, IL CAMP REFUND POLICY A full refund of registration and participation fees and charges will be made for all camps cancelled or adjusted by MCHS. For all other situations where refunds may be requested, the following guidelines shall apply. The Athletic Director shall have the authority to make decisions on all requests not specifically covered herein. 1. If a participant requests a refund, in writing, 10 calendar days or more in advance of the first day of camp, a full refund, minus a $5.00 processing fee per camp, will be issued. 2. If a participant requests a refund, in writing less than 10 calendar days prior to the first day of camp, a full refund, minus a $10.00 processing fee per camp. 3. No refund will be issued on the first day or after the first day of camp. Exceptions: A full refund of all fees paid, less a $5.00 processing fee per camp, will be made if: Prior to the first day of camp, a written excuse, from a licensed medical doctor, is provided indicating that the participant should not participate due to medical concerns or physical limitations. PARENT/GUARDIAN AGREEMENTS: 1. I acknowledge that every effort will be made to contact parents/guardians in the case of a medical emergency. If I cannot be reached, I authorize MCHS staff to seek appropriate medical care. 2. I understand that MCHS staff may not administer any medications or sunscreen. 3. I understand that MCHS is not responsible for lost or stolen items and that my child should not bring personal belongings such as toys, jewelry or money to camp. 4. My child will be dressed appropriately for camp and will wear athletic shoes or sneakers. 5. I will drop off and pick up my child at the designated times listed for each camp. Signature is required on Registration Form 3. You may complete the registration form and pay in person at our Central Campus. All Camps Include Camp T-Shirt (unless noted) Training, drills, games and directed instruction!
