The Staff and PTA have been working hard all summer to prepare for the coming school year. Thank you for allowing us to share in this experience with your children as they grow, expand their minds and create incredible memories. We will continue with the following goals this year: Provide - effective communication between school, parents and students. Respect - each other, students and staff Involve - parents in PTA sponsored activities Develop - an exciting, welcoming learning environment Engage - students with positive programs and events Our PTA continues to have a strong presence and a favorable reputation with the faculty at our school. 2 To: Parents of 7th Graders Re: Immunization Requirements for Students Entering 7 th Grade The Texas Legislature passed legislation in 2009 requiring additional immunizations for students entering 7 th grade. Parents are required to provide proof of these immunizations prior to the start of school in August **Students who do not have proof of these immunizations will not receive their schedules at orientation in August 2017.** Two additional immunizations are required: Tdap booster (if at least 5 years have passed since the last dose) Meningococcal (MCV4) Some may have already received these vaccines. Please review your child s immunization record(s) to see if he/she has received the required immunizations. A legal copy of the updated record from the clinic/physician is required and should be provided to our school nurse as soon as possible if you have not already done so. 3 LISD Health Services Information for Middle School Students NOTE: A pass from a teacher is required for ALL Health Room visits unless it is an emergency Health Screenings - 7th Grade: Vision, Hearing and AN* (Acanthosis Nigricans) Fall 6th Grade: Spinal Screening Spring *AN or Acanthosis Nigricans This state mandates that screening is required of all students in the 3rd, 5th, and 7th grades which is done by assessment or observation for skin changes which may include a velvety brown, rough skin usually found at the back or nape of the neck. Parents will only be notified if there are concerns such as positive AN findings. Medications - Students in Middle School may carry over-the-counter medications (Tylenol, ibuprofen, Midol, etc.), inhalers and Epi-pens, and most prescription medications, excluding controlled substances (Ritalin, Adderall, Vicodin, etc.). All controlled substances are required to be kept in the nurse s office. Below are the requirements for these medications. Forms are available from the Nurse and the Nurse s SRMS website. Over-the-Counter (OTC) Medications / Some Prescription Medications 1. Medication must be in the original container. 2. Students are required to carry a completed SRMS OTC/Rx Medication Permission form along with the medication, or a signed note from a parent listing the medication, instructions for its use, and a contact number. 3. STUDENTS ARE NOT PERMITTED TO SHARE MEDICATION WITH ANYONE. Inhalers and Epi-pens 1. Provide the School Nurse with a completed Medication Administration form or Allergy Action Plan. 2. Have a prescription label from pharmacy attached to or with the inhaler/epi-pen with student s name and instructions for use. Bring inhaler/epi-pen to nurse for documentation and review of use and instructions. Controlled Substances and Treatments 1. Provide the School Nurse with a Medication Administration form completed and signed by parent/guardian and physician along with any additional pertinent information from your child s doctor. 2. Medication must have a prescription label with student s name, dose, specific time & instructions for use. The School Nurse DOES NOT GIVE OUT OVER THE COUNTER MEDICATIONS. We do not stock or administer any medications which are not provided by parent(s). Parents must provide all medications as directed above. *STUDENTS ARE NOT PERMITTED TO TEXT/CALL A PARENT TO BE PICKED UP REGARDING INJURY/ILLNESS WITHOUT VISITING THE NURSES OFFICE FIRST. THIS IS A DISTRICT POLICY. Note: AED s (Automated External Defibrillators) are in all LISD schools including SRMS

We are having a SOCK DRIVE to help LISD students in need BRING A NEW PAIR or TWO OF SOCKS to ORIENTATION Simply pick up a spare pair of socks when you and your children are shopping for back to school. Drop them off in the donation box during orientation and the PTA will take it from there. This simple gesture can make a BIG difference for many in our district. * ADULT SIZE SPORTS AND ATHLETIC SOCKS ARE GREATLY NEEDED! * Dressed To Learn is a partnership with LISD Council of PTAs and Communities In Schools of North Texas. They directly benefit students in our district who are in need of basic clothing items to ensure they are Dressed To Learn. Be sure to check them out and support their efforts.
