1 PUH 5304, Principles of Health Behavior Course Syllabus Course Description Examines the psycho-social, behavioral, and educational principles that determine health behavior. Theoretical models synthesizing these principles are examined. The course also presents ethical principles of professional and personal concern to health educators. Course Textbook DiClemente, R. J., Salazar, L. F., & Crosby, R. A. (2013). Health behavior theory for public health: Principles, foundations, and applications. Burlington, MA: Jones & Bartlett. Course Learning Outcomes Upon completion of this course, students should be able to: 1. Appraise the theoretical models used to define health. 2. Recognize the role of the health educator in health behavior change. 3. Describe the principles that determine health behavior. 4. Identify behaviors that contribute to mortality and morbidity. 5. Analyze the concept of contributing behaviors to one s health status. 6. Analyze the concept of determinants of health behavior. 7. Examine health behavior intervention strategies. 8. Evaluate the professional and personal concerns of health educators regarding health behavior intervention strategies. Credits Upon completion of this course, the students will earn three (3) hours of college credit. Course Structure 1. Unit Learning Outcomes: Each unit contains Unit Learning Outcomes that specify the measurable skills and knowledge students should gain upon completion of the unit. 2. Unit Lesson: Each unit contains a Unit Lesson, which discusses unit material. 3. Reading Assignments: Each unit contains Reading Assignments from one or more chapters from the textbook. Suggested Readings are listed in the Unit I, II, and VI study guides to aid students in their course of study. The readings themselves may or may not be provided in the course, but students are encouraged to read the resources listed if the opportunity arises as they have valuable information that expands upon the lesson material. Students will not be tested on their knowledge of the Suggested Readings. 4. Unit Assessments: This course contains five Unit Assessments, one to be completed at the end of Units I, II, III, IV, and VII. Assessments are composed of written response questions. 5. Unit Assignments: Students are required to submit for grading Unit Assignments in Units I, II, V, VI, and VIII. Specific information and instructions regarding these assignments are provided below. Grading rubrics are included with the Unit I, II, V, VI, and VIII Assignments. Specific information about accessing these rubrics is provided below. PUH 5304, Principles of Health Behavior 1

To access the library, log into the mycsu Student Portal, and click on CSU Online Library. You can also access the CSU Online Library from the My Library button on the course menu for each course in Blackboard. Librarians can help you develop your research plan or assist you in finding relevant, appropriate, and timely information. Reference requests can include customized keyword search strategies, links to articles, database help, and other services. Unit Assignments Unit I Project Topic Within Unit VIII, you will submit a concept paper that will serve as the groundwork for a mock public health initiative project. For this unit, you will need to identify a public health concern either within your area or one that is currently trending that would act as a good foundation for the final project. Once you identify a public health concern, you should provide context for your project using the core questions identified in Table 2-1 on page 35 of the textbook. In addition to the core questions, please address each of the following: 1. What is the public health concern of interest? 2. Summarize the problem and behaviors that contribute to the problem. 3. What are past and current approaches used to address the problem? 4. What is your the proposed solution and approach to target the problem? Your project topic should be at least one page, double spaced, and Times New Roman 12 pt. font. You are required to utilize at least one resource from the CSU Online Library. All references and citations used must be in APA style. Unit II Assignment In continuing with the development of your concept paper, you will now construct a framework to assist you in your program planning efforts. Using the PRECEDE-PROCEED model described on pages of the textbook as a guide, assign a set of evaluation tasks to frame your concept paper. Keep in mind the purpose of your chosen program initiative when developing your framework. You are encouraged to do an Internet search for examples of public health program planning models. (Please use these as examples only. Do not plagiarize from these examples.) Make sure you present your framework so anyone within your fictional organization could pick up where you left off. Your document should be at least one page, Times New Roman 12 pt. font, and use APA style. PUH 5304, Principles of Health Behavior 2

3 Unit V Assignment In continuing the preparation for your concept paper, develop a strategic marketing concept to promote your public health initiative. You may consider the following marketing tools to deliver your message: Internet Brochures Flyers Radio Television For this assignment, you will need to provide an example of your chosen marketing tool in action. For example, if you choose brochure, you will create a brochure. If you choose radio, create a transcript. If you choose television, create a transcript that includes a description of the scene and action occurring. Next, you will need to provide a step-by-step overview of how you plan to carry out your marketing efforts. Please refer to Chapters 9 and 10 in the textbook. You may also find examples of public health marketing tools on the Internet. Your strategic marketing concept should utilize Times New Roman 12 pt. font with APA style. Unit VI Assignment For this assignment, you will select a measurement tool/instrument that you believe is most appropriate for your public health initiative project. You may either choose a tool or develop your own based upon the following instrument examples: Questionnaires Self-checklists Attitude Scales Personality Inventory Observation Form Rating Scale Survey You will need to describe the tool you selected or created, explain why you chose that tool, and provide a summary of how it will be utilized within your health initiative project. It is important that you are detail oriented within this assignment, as it will be part of your final course project. You may reference pages in the textbook to assist you with the selection or development of your tool. You may also find it helpful to access the site linked below, which provides tips for developing a measurement tool, as well as information concerning what the measurement tool should include based on the kind of data and outcomes you are seeking. It also provides additional resources that you can use to find further instructions/examples of measurement tools and instruments for your proposed public health initiative: Your assignment should be a minimum of two pages in Times New Roman 12 pt. font and should utilize APA style. Unit VIII Course Project You are now ready to compile all of the components that have created over the length of the course and prepare the final draft of your concept paper. In the conclusion of your concept paper, include how you will take all of the information that you have obtained through your research efforts and put it into practice to help further health promotion efforts. 4 The final submission of your concept paper should be a minimum of eight pages, double spaced, Times New Roman 12 pt. font, and utilize APA style. APA Guidelines The application of the APA writing style shall be practical, functional, and appropriate to each academic level, with the primary purpose being the documentation (citation) of sources. CSU requires that students use APA style for certain papers and projects. Students should always carefully read and follow assignment directions and review the associated grading rubric when available. Students can find CSU s Citation Guide by clicking here. This document includes examples and sample papers and provides information on how to contact the CSU Success Center. Grading Rubrics This course utilizes analytic grading rubrics as tools for your professor in assigning grades for all learning activities. Each rubric serves as a guide that communicates the expectations of the learning activity and describes the criteria for each level of achievement. In addition, a rubric is a reference tool that lists evaluation criteria and can help you organize your efforts to meet the requirements of that learning activity. It is imperative for you to familiarize yourself with these rubrics because these are the primary tools your professor uses for assessing learning activities. Rubric categories include: (1) Assessment (Written Response) and (2) Assignment. However, it is possible that not all of the listed rubric types will be used in a single course (e.g., some courses may not have Assessments). The Assessment (Written Response) rubric can be found embedded in a link within the directions for each Unit Assessment. However, these rubrics will only be used when written-response questions appear within the Assessment. Each Assignment type (e.g., article critique, case study, research paper) will have its own rubric. The Assignment rubrics are built into Blackboard, allowing students to review them prior to beginning the Assignment and again once the Assignment has been scored. This rubric can be accessed via the Assignment link located within the unit where it is to be submitted. Students may also access the rubric through the course menu by selecting Tools and then My Grades. Again, it is vitally important for you to become familiar with these rubrics because their application to your Assessments and Assignments is the method by which your instructor assigns all grades. Communication Forums These are non-graded discussion forums that allow you to communicate with your professor and other students. Participation in these discussion forums is encouraged, but not required. You can access these forums with the buttons in the Course Menu. Instructions for subscribing/unsubscribing to these forums are provided below. Once you have completed Unit VIII, you MUST unsubscribe from the forum; otherwise, you will continue to receive updates from the forum. You will not be able to unsubscribe after your course end date.
