1 1 APAC Forum 2017 eposter Guidelines This year at the APAC Forum Posters will be viewed in the Poster Gallery, with electronic posters displayed alongside printed posters and following on from last year s showcase, video posters. We are aiming to reach new audiences in different ways. Winning posters will be invited to present their posters at the conference. Display details eposters Posters will be displayed on portrait electronic display screens. A template is provided to ensure your poster is displayed to its best advantage Video Posters 10 selected posters Poster Awards Judging will take place in two steps. Twenty shortlisted eposters will be chosen across all themes and methodologies. The winners will then be selected by an international judging panel. The winners will be announced at the APAC Forum Plenary on 22 September 2017, where they will have the opportunity to do a short presentation of their poster. Poster submission This year, we are following the same acceptance process as last year, electronic submission via the poster portal. We require a draft poster rather than an abstract. Once accepted, posters can be updated and recent results included prior to the final submission date. You do not need to bring a printed copy of your poster. Deadlines Please ensure your draft eposter template document has been uploaded to the poster portal by Wednesday 12 th July. We will notify you by Wednesday 2 nd of August if your eposter has been selected for display at the APAC Forum You are then able to revise and update your poster before the final submission deadline, midnight on Tuesday 15 th of August Any posters received after this date will unfortunately not be eligible for awards. Feedback for improvement Poster authors have the opportunity to obtain advice and feedback on their draft posters from improvement advisors and research experts. During the submission process you are able to select this option. Feedback will be provided by Wednesday 2 nd of August Presenting poster author Each poster must have only one presenting author nominated during registration and submission. The presenting author must register and attend the conference to present the poster. Each presenting author may only present one poster. Please acknowledge wider team

2 2 involvement on the poster. A special conference rate for wider team members will be made available on the registration tab in the poster portal. Poster portal Step 1: Create an account by registering on the APAC Forum website Create an APAC Forum account and contacts via the poster portal (link provided on the webpage). Please note contact details will uploaded and used for posters searches and display information, so take care with abbreviations and spelling, give organisations and institutions in full and locations in full. Step 2: Submit your draft poster Create your eposter using the template provided on the webpage. Only eposters using the PowerPoint template will be accepted. Please DO NOT alter the sizing of the template, as the pre-set size optimises screen display. Please DO NOT cover or amend the size and position of the APAC Forum logo, Home or Back buttons, these are embedded in the display software and will cover any content you place there. Return to your APAC Forum account and complete the eposter details and upload the template document. Both the details and your eposter file will remain editable on the portal until you press the submit button. Please note the poster details will be used to categorise and search for your poster. Please also remember you are attending an international conference and local abbreviations may not be widely understood. During the submission process, there is an option to choose to have our local research and improvement experts provide feedback on your poster. If you wish to receive feedback, please indicate this during the submission process. Draft poster submissions close Wednesday 12 th July. Step 3: Submit your final poster Following draft poster acceptance there will be an opportunity for you to edit and finalise your poster, adding any additional data or results. Final posters must be submitted by 15 th August Poster presenter terms and conditions By submitting a poster to the APAC Forum 2017 you are agreeing to the following terms and conditions: I confirm that the content of my poster is factually correct and is my own work and I am authorised to share this information publicly. I understand that, while I retain ownership of copyright of my poster presentation, the APAC Forum organisers and the APAC Forum 2017 may publish my poster or video poster and any biographical information provided by me, on printed media, on the Ko Awatea website or on the APAC Forum 2017 website, video of proceedings, on the APAC Forum mobile app or via social media. I understand I should not use my poster as a means of selling my organisation's products or services.

3 3 I understand that each poster must be accompanied by at least one designated presenting author; this person must register and attend the conference and can only represent one poster. I understand that all successful submissions will automatically be entered into the competition for Best Poster across all categories. I understand that I will be notified of the outcome of my submission and will submit my final poster by the specified deadline. You will be asked to agree to these conditions during the submission process. Conference themes During the poster portal registration process you will be asked to select one or more themes that relate to your poster. These will be used to search and group your poster. LEAD INNOVATE GROW INTEGRATE CHANGE DESIGN Outfit yourself, and your people, with the authentic leadership qualities required to reach new heights. Chart a course for emerging leaders and create an environment that empowers people to tackle their own ascent. Hear from early adopters about the benefits and lessons learned from implementing digital solutions for clinical enablement. Are we using technology to its potential where it s improving healthcare processes, producing meaningful information, and ultimately improving patient outcomes? Cultivate your passion and resilience to enable yourself, and your teams, to compassionately contribute to the health of your population. Your wellbeing affects that of those in your care too. Allow yourself to reflect on what you do, why, and restore your energy. Hear from people who have spearheaded integration campaigns to close the gap between primary and tertiary health services, and those who are proactively working with our colleagues from related sectors. Map a patient pathway that delivers improvements in accessibility, reliability, and patient safety while reducing costs. Achieving the scale of change that is required in healthcare systems globally will require big thinking, cultural transformation, operational excellence, and durability. Hear from pioneers who are leading largescale disruptive change. At the centre of what we do is the desire to design services that provide excellence in quality and patient safety. Take a look at best practise examples from high performing organisations and learn how they have designed optimal services which result in excellence as measured by in quality standards and accreditation. Project approaches

4 4 During the poster portal registration process you will be asked to select one approach that best describes the approach used in your poster. This will be used to search, group and judge your poster. The judging criteria are based on these guidelines and aim to support sharing great work and best practice. Poster section suggestions The Problem Background Introduction The Question The Objective The Aim Interventions Strategy for Change Change Package Improvement Science Methodology Research Study Design Measurement Measures of Success Guideline reference Problem: What was the specific problem or challenge that you set out to address? How was it affecting patient/client care? Assessment of problem and analysis of its causes: How did you quantify the problem? Did you involve staff at this stage? How did you assess the causes of the problem? What solutions/changes were needed to make improvements? Intervention: Describe the intervention used with sufficient detail so that others could reproduce or adapt it. Strategy for change: How did you implement the proposed change? What staff or other groups were involved? How did you disseminate your plans for change to the groups involved? What was the timetable for change? Improvement Science Methodology: describe the methodology chosen (for example: the Model for Improvement, Lean, Patient Experience) why it was chosen and how it was applied. Study design: If your study was formal research, then describe the study design (for example, observational, quasiexperimental, experimental) chosen for measuring the impact and outcomes. Measurement of improvement: How did you measure the effects of your planned changes? Describe the analytical methods used and the results obtained.
