3 1.0 INTRODUCTION Brief introduction of: (Example) The CLIENT. CLIENT objectives Scope / Objectives of this HAZOP Location of the HAZOP meeting The objective of the HAZOP is to assess and evaluate potential hazards related to the Implementation of the planned modifications. The HAZOP has been conducted at the Client s office location at. PCB Consultants has provided the HAZOP chairman. [CLIENT] has provided the HAZOP secretary. Participants of the HAZOP are listed in Attachment 1. The objective of this HAZOP is to assess the effect of the modifications on safety and additional potential hazards that are associated with the modifications. The scope of this HAZOP is to review the changes of modes of operation identified and any deviation from the normal, i.e. existing mode of operation. Procedural issues, when identified, have been recorded as action items together with the deviation of the intended use of the system and the system modifications as they became apparent during the HAZOP exercise. Adequacy of system safeguards has been reviewd only where it constitutes an additional hazard as a result of the modifications. When it is perceived that conditions have no impact on the original design and do not constitute an additional hazard, it was considered as Not part of this HAZOP or has been recorded as part of the normal existing operation. 2.0 METHODOLOGY Brief description of the methodology. [Typical] For performing the HAZOP, the PCB Consultants HAZOP worksheet format (Attachment 2) has been used. [CLIENT] documents, listed in Attachment 4, have been used during the HAZOP for reviewing the modifications and include: Process and Instrument Diagrams (P&ID s) [CLIENT] Technical notes if/ when applicable (Referred to but not necessarily included in the attachments) Pre-selected guidewords have been used (refer to paragraph 2.2) for identifying perceived conditions, relevant to the proposed modifications, which may occur when operating the system Page 3 of 5

4 after the implementation of the modifications. During the HAZOP session Page 4 of 5

5 Guidewords have been rephrased to suit specific issues as they became apparent. The actual Guidewords that have been applied are listed in paragraph 2.2. During the HAZOP, review each Node/Guideword was projected on a screen by the secretary. Once a conclusion/consensus was reached by the participants on how to deal with the issue, the consensus was recorded, projected, and saved. 3.0 REVIEW OF HAZOP RESULTS. No issues requiring major changes to the proposed modifications have been identified. The Recommended Actions were mainly related to engineering verification and preparation of procedures required for operation. Total number of action items: 15.
