1 IF Public Discussions - Survey Monkey Instructions (February 2017) The Interactivity Foundation uses the Survey Monkey website for its online surveys. Sue Lea, an IF Fellow and our Education Coordinator, manages this online survey effort. These surveys provide systematic data about citizens experiences with our public discussions. We are also interested in hearing about your own experiences in facilitating discussions and there is a separate survey for facilitators. We believe that these online surveys will allow us to collect more information in less time from both discussion participants and our facilitators. We are counting on you to help us with this important effort. PARTICIPANT SURVEYS To help us to collect data about our public discussions, we ask that each of you direct your discussion participants to use the following link to our online survey at the conclusion of a discussion series: If they are able to link directly from this document, they can try the following link: Survey Monkey for Participants in IF Public Discussions But if for any reason the above link doesn t work, then here is the full URL address that can be copied and pasted into their web browser: Please ask all participants to complete the survey after each discussion series, even if they might have participated in another, prior public discussion series that you facilitated. You can send participants an or give them a small handout that provides the survey link and the required pass code (see next page) that they'll be asked to enter at the beginning of the survey. FACILITATOR SURVEYS After each public discussion series, IF now requires that ALL FACILITATORS AND FELLOWS complete a separate facilitator survey using the following link: [if clicking/linking directly from this document:] Facilitator Survey [copy/paste the following for the full link:] You will enter the same passcode as your participants are using for the participant surveys (the passcode correlates to the discussion series rather to any particular individual). It is very helpful for us to get direct feedback from you about your facilitation experience so that we can improve upon our reports and our process. This feedback is sought so that IF can have better guidance as to potentially revising reports. You may also be asked to submit a separate written report or blog post about your discussion series to your IF manager/contact, particularly if you are a newer facilitator. Even if you submit a written report, however, you will still need to complete the online facilitator survey. Page 1 of 5

2 SURVEY PASSCODES Passcodes are comprised of seven (7) digits in the following format: ### - ## - ##. Passcodes are designed simply as a way for us to index data in different ways, i.e., by facilitator, report, or number of times a particular facilitator has led a discussion of a particular report. The first three numbers identity the facilitator. Facilitator ID numbers are listed below. Additional names and numbers will be assigned as needed. 5 The last two numbers represent the number of times that you personally have facilitated a particular topic. For instance, if this is the 2 nd time you have facilitated a discussion of the Rewarding Work topic, the last two digits of the passcode for your discussion would be 02. Note that the preceding zero (0) is required. As an example, if Sue Lea were to conduct a discussion of the Property report, which is a report that she has not yet facilitated, she would give the participants the following passcode at the end of the discussion when she directed them to the website link for the participant survey form: Sue s facilitator passcode number is 106, the report number is 07, and this would be the first time that she would have facilitated this particular report. WHAT IF MY PARTICIPANTS DON'T HAVE COMPUTERS? Some of your participants may be unwilling or unable to complete the participant survey online. If this is the case, you can give them a paper copy of the survey, which you can obtain by downloading the document from either of the following links: ng Discussion-Participant-Survey-Form-PAS-draft-2013_1106.pdf and then printing it out; or Requesting a printed copy from the Interactivity Foundation (Ph or office@interactivityfoundation.org); or Requesting an electronic copy from Sue Lea.
