II. PREVIOUS YEAR SPP OUTCOMES AND REFLECTION In this section, document the outcomes from last year's goals and reflect on the root causes surrounding why your school met or did not meet those goals. Outcomes (Effect Data): Complete the chart below Last Year's Goals (auto-populate) Goal 1: By June 2015, 40% of students will score on average proficient or advanced on the Text Reading and Comprehension Assessment (TRC). Goal 2: By June 2015, 45% of students will score on average proficient or advanced as measured by the district's quarterly math interim assessments. Final Outcome (Do not analyze here, just state end outcome) 40% of students scored proficient or advanced on TRC as evident of end of year assessment. On interim 4, 43% of students were proficient or advance. Was the goal met or not met? Met Analyze and provide reflection on last year s outcome(s) Based on end of the year assessments 40% of students achieved proficiency in. Some contributing factors include, small group instruction teacher data analysis, and a positive culture and climate. Some factors affecting this outcome include student lateness and absences. Not Met Kindergarten-86.5%(Interim 4) Grade %(Interim 4) Grade (Interim 4) Grade %(Interim 4) Grade 4-15%(Interim 4) Grade 5-6.3%(Interim 4) Average proficient or advanced = 43% Students made progress throughout the school year.this was due to the continued utilization of the Common Core State Standards, teachers collaborating and using different strategies to engage students and increase School Performance Plan for Callaway Elementary 6 of 42

achievement. Additionally, teachers analyzed different forms to data to determine specific student needs. Goal 3: By June 2015, we will have decreased suspensions incidents by 25 from 78 in school year to 53. At the end of the school year 19 suspension incidents occurred. Met School climate increased as evident in student and teacher surveys last year. This had an impact on suspensions. This was due to the use of Restorative Practices to develop a strong school community and support students in making good choices. Teachers used restorative circles to build relationships with students and solve problems. Goal 4: By June 2015, 100% of teachers will implement small group instruction to provide interventions and enrichment in reading and math as evident in lesson plans, classroom arragement, class schedule and observations. As evident in walk through, informal observation and lesson plan review, 100% of teachers implemented small group instruction. Met All teachers utilized small group instruction to provide intervention and enrichment opportunities. This was evident during informal observations, walk through, and lesson plan review. Reflections on SER Visit: If your school had SER visit then reflect on your SER visit to answer the following questions. Reflection Questions Did the findings from the SER align with your perception of your school s performance at the time of the site visit? Which findings were surprising? Deflating? Encouraging? In reviewing the SER findings along with your performance data (qualitative and quantitative) what is the school identifying as an area of focus? Responses The findings aligned to the perception of the our school performance at the time of the visit. It was encouraging to know that our work is increasing student achievement and establishing a positive school culture and learning environment. However, less than half of the students are meeting with proficiency in reading and math. The school identified reading as an area of focus for the school year. Define your current state. What preferred state would you like to reach? We would like for most of the students to reach proficiency in reading to be able to access grade level material in reading and math. Currently 33% of students are proficient, 21% below proficient and 46% of students are in the School Performance Plan for Callaway Elementary 7 of 42

well below proficient level. What actions you would take to reach your preferred state? The actions to support our focus area include the following: 1. Institute a reading intervention/enrichment block 2. Utilize Fauntas and Pinnell Guided Reading to support reading intervention/enrichment 3. Develop a professional learning cycle to support teachers in utilizing guided reading effectively 4. Utilize scholastic leveled libraries to support student engagement and independent reading 5. Utilize a research based curriculum (Intervention by Desigh) to support students in need of intervention School Performance Plan for Callaway Elementary 8 of 42

III. INQUIRY In this section, document your questions that your Instructional Leadership Team has regarding teaching, learning, and leadership to support planning for the next academic school year. Keep a focus on past and future shifts in instructional programming in response to standards; and expectations regarding programmatic goals, scheduling and collaborative planning. This is a tool to support the activity of Inquiry which should come before looking at data. o Generating questions before looking at your school s data supports a focus for the data analysis. o Questions focused on a deeper understanding of contents and grade level expectations for MCCRC and PARCC assessments. o Ask questions directly related to raising the Bar for every student. Teaching (Adult Actions) Questions Learning (Student Outcomes) Questions Leadership (ILT Actions) Questions How do teachers manage to re-teach content while maintaining pace? How are teachers deepening their understanding and use of the Common Core State Standards? Are students engaging in lessons that meet their specific needs and help them move towards mastery of the content? Are students engaged at the same level of rigor as expected in the CCSS? How is administration supporting teachers in managing the use of re-teach plans while maintaining pace? How is the ILT team supporting teachers in identifying opportunities to re-teach content without falling behind in the curriculum? How is administration and ILT supporting teachers to increase the level of rigor to match that of the CCSS? School Performance Plan for Callaway Elementary 9 of 42

IV. SCHOOL'S DATA SECTION In this section, provide additional data that supports your school's understanding of previous years' performance. This section will also contain the EOY ILT Tool provided by the district with the following data. o Climate Attendance, Chronic Absence, Suspensions as of 5/30 o HS Academic Data Bridge (# of students with 1 Bridge, # of projects by subject) GPA and At Risk indicators by grade HS Subject Exams (MOC/MT and EOC/Finals) % Proficient o K-8 Academic Data TRC/Dibels BOY and EOY (% Proficient, % Well Below) RISE Reading Comprehension BOY and EOY (% Proficient, % Warning) Math Units (% Proficient on all Post Tests) Literacy Modules (% Proficient) o Administration rates or # tested for all district assessments For previous years' State Assessment data, please refer to the student-level files on Data Link and/or Principal's Dashboard. 11 V. ANALYZE DATA TO PRIORITIZE STRENGTHS AND NEEDS Select the data that can answer your inquiry questions and analyze it to document top three strengths and needs within your school. Use this data to identify the problem(s) of practice by conducting a root cause analysis (strengths and needs based on evidence) Prioritize the problem of practice (needs) Prioritized Strengths based on Effect Data (Student Outcomes) Inferences (Why is this an area of strength?) 1. Students increased reading comprehension from 27% to 40%. Teachers utilized small group instruction to support individual student learning. Teachers collaborated and planned to support small group instruction. 2. Positive school culture and climate increased as evident in student and teacher surveys. 3. Math proficiency increased from 34% to 43% by the end of the school year as measured by interim assessments. Staff members utilized restorative practices to build a community enabling students to make better choices by reflecting on their actions and pro-actively solving problems. All teachers fully implement the City School Curriculum. Teachers have a deep understanding of the MCCR content standards as well as the Instructional Framework, which is reflected in the pedagogy within the classroom. Prioritized Needs based on Effect Data (Student Outcomes) 1. Only 40% of students are reading on grade level. 21% of students are reading below grade level and 36% are reading well below their grade level. 2. Students in all grades need to master math application skills to better demonstrate an understanding of the mathematical principles and concepts with in the City School Curriculum. Inferences (Why is this an area for growth?) Students lack the necessary fluency and comprehension skills to make enough progress from BOY to EOY to meet the grade level standards. There is a lack of resources needed to provide additional skill-specific intervention to this target population. Student absences, tardiness, inclement weather, and strict lesson plan pacing affect increased student mastery of CCSS Math skills % of students are at risk for chronic absence. Students who are chronically absent and tardy are not meeting the grade level standards and may not be effectively engaged in instruction. School Performance Plan for Callaway Elementary 11 of 42
