Further infrmatin abut services fr students with disabilities and the accmmdatin prcess is available upn request at this number. SMCC Pay-fr-Print Plicy Each semester students receive a $20 printing credit. The balance resets at the end f the semester and any remaining credits are remved. The cst varies depending upn page size and whether printing is dne in black and white r clr. Add-Drp Plicy Students wh drp a curse during the ne-week add/drp perid in the fall and spring semesters and the first three days f summer sessins receive a 100% refund f the tuitin and assciated fees fr that curse. Please nte any curse that meets fr less than the traditinal semester length, i.e., 15 weeks, has a pr-rated add/drp perid. There is n refund fr nn-attendance. Withdrawal Plicy A student may withdraw frm a curse nly during the semester in which s/he is registered fr that curse. The withdrawal perid is the secnd thrugh twelfth week f the fall and spring semesters and the secnd thrugh ninth week f twelve-week summer curses. This perid is pr-rated fr shrter-length curses. T withdraw frm a curse, a student must cmplete and submit the apprpriate curse withdrawal frm, available at the Enrllment Service Center (n phne calls, please). The designatin W will appear n the transcript after a student has fficially withdrawn. A curse withdrawal is an uncmpleted curse and may adversely affect financial aid eligibility. Failure t attend r ceasing t attend class des nt cnstitute withdrawal frm the curse. There is n refund assciated with a withdrawal. Plagiarism Statement Adherence t ethical academic standards is bligatry. Cheating is a serius ffense, whether it cnsists f taking credit fr wrk dne by anther persn r ding wrk fr which anther persn will receive credit. Taking and using the ideas r writings f anther persn withut clearly and fully crediting the surce is plagiarism and vilates the academic cde as well as the Student Cde f Cnduct. If it is suspected that a student in any curse in which s/he is enrlled has knwingly cmmitted such a vilatin, the faculty member shuld refer the matter t the Cllege s Disciplinary Officer and apprpriate actin will be taken under the Student Cde f Cnduct. Sanctins may include suspensin frm the curse and a failing grade in the curse. Students have the right t appeal these actins t the Disciplinary Cmmittee under the terms utlined in the Student Cde f Cnduct. Path Thrugh Math (PTM) Schlarship If yu are a Maine resident and have demnstrated financial need, yu may be eligible fr a schlarship f up t $500 by investing in yur math studies thrugh participatin in the Path Thrugh Math prgram. The prcess is simple: fllwing a tw-hur diagnstic test yu will be prvided with a persnalized nline math study curse in a prgram called My Fundatins Lab. After 20 hurs f mnitred nline study, yu retake the diagnstic test. When all steps have been cmpleted yu will be awarded a $500 schlarship. See yur advisr fr additinal details.
5 Student Cntract Numerical Math MAT-020 This cntract is written acknwledgement that the student has read and understands the criteria fr successful cmpletin f this curse as described in the curse syllabus. It is als the first assignment f the semester. A Cmmunity Cllege is designed t be a practical preparatin fr success in the wrkplace. As such, there are several aspects f classrm expectatins f the student designed t mirrr cmmn wrkplace expectatins f the emplyee. Classrm expectatins: The classrm is a place fr learning, and learning must be the fcus while in class. All cell phnes must be turned ff during class. Texting in class is grunds fr dismissal! Student-Teacher interactin is tw-way. The teacher will deliver lecture-based explanatin f the curse material, but that is nly the fundatin f the curse. The majrity f cmmunicatin shuld be questins and discussin f the material. Students are expected t ask and answer questins and t engage in discussin f the underlying cncepts. Althugh student questining is HIGHLY encuraged, questins must be asked f the instructr. All students are encuraged t participate in the answering and discussin f any and all questins, but side questins and discussins are nt allwed. A student wh feels like he/she is falling behind must ask the teacher fr help and nt engage a neighbring student in a side-discussin which impacts bth students' ability t remain fcused n the lecture. Althugh the grade fr the curse is based n quiz and hmewrk perfrmance, Attendance/Participatin/Attitude will be tracked ffering the pprtunity fr bnus pints t be awarded at the end f the semester. Students must ask and answer questins t earn full credit. Attendance & Punctuality Expectatins: As with the wrkplace, students are expected t be present and engaged in the classrm during the entire perid. Cllege students are adults s it is up t each f yu -- nt the teacher -- t decide whether r nt circumstances warrant an absence. There are many justified reasns why Students may be absent, but very few justified reasns fr disappearing withut cntact via telephne r . Students must cntact the instructr when absent. (Failure t d s may result in n credit fr missed wrk.) The student is respnsible fr all lecture material cvered during an absence and due dates are still valid. If the student has cntacted the teacher, arrangements can be made fr text hmewrk r quizzes t be presented the day f return fr full credit. If a student is absent fr five (5) classes anytime thrughut the curse, an administrative withdrawal frm the curse may be initiated, and missed quizzes and hmewrk may nt be made-up. Punctuality t class is expected bth at the start f class and returning frm break. Tardiness t class (at the start r after the break) will result in reduced credit fr the perid. There will be NO exceptins. Hmewrk & Assignment Expectatins: Assignments are designed t present a full educatin in the curse. Students shuld nt expect t get r use alternative assignments fr credit in lieu f the actual assignments. Fr all assignments (HW, tests & quizzes, etc.) wrk must be shwn (neat & rderly) fr full credit. Demnstrating and shwing a mathematically sund prcess is as much a part f the assignment as finding an answer. Units f measurement, if applicable, are as much a part f the answer as the number. Units must be included fr full credit. Assignments and prject due dates will be strictly enfrced. Special circumstances may be taken int cnsideratin nly if the student has cntacted the instructr prir t a particular deadline. Otherwise, late wrk will nt receive full credit, and may nt be accepted. It is the student s respnsibility t pass in all assignments n time. If an assignment is nt passed in t the instructr then a grade f zer will assigned. Plagiarism, cpying, r in any way presenting wrk that is nt fully the student s wn wrk will result in lss f credit fr the assignment and/r remval frm the class with an administrative failure. I, (print yur name here), have read bth the full curse syllabus and the abve cntract. I understand them and agree t fully abide by their terms. Student s signature: Date:
